What Happens During A Herpes Antibody Test

Discontinuation of Herpes Simplex virus (HSV) IgM Testing

There are different tests for HSV. Doctors will choose a test based on a persons health history and symptoms. Options include the following:

  • Swab test: A healthcare professional uses a swab to take a sample of fluid and cells from a herpes sore. They then order a PCR test or viral culture to determine whether HSV is present. This is the most reliable way to test for HSV. Doctors normally do this as standard procedure before ordering any other sort of test.
  • Blood test: A healthcare professional uses a needle to take a small amount of blood from a vein in a persons arm.
  • Lumbar puncture: Doctors may use this method if they suspect a person has an infection in the brain or spinal cord. It involves numbing the skin of the lower spine and inserting a thin, hollow needle between two vertebrae to collect cerebrospinal fluid.

A doctor will send samples to a laboratory for testing and a person should get their results within a few days.

Analysis Of Children’s Sera By Peptide

Alternative methods for specific detection of HSV-1 or HSV-2 antibodies include different ELISAs, both commercial methods and recently developed methods based upon the antigenicities of two different oligopeptides specific for HSV-1 and HSV-2 . However, the performances of these methods in a pediatric population have not been evaluated. The two peptide-ELISA methods developed in our laboratories were of particular interest and were used to analyze the sera that were already characterized by the WB-UB method.

How Does The Herpes Antibodies Test Work

A herpes antibodies test works by looking in your blood for HSV-1 antibodies or HSV-2 antibodies â typically the IgG type but sometimes the IgM type.

These tests only indicate exposure to either HSV-1 or HSV-2 at some point in your past. This does not necessarily mean that HSV is causing any current symptoms that you may have â since the exposure could have been years ago.

If you are worried about a new infection, it might be useful to take the test again around 21 days later to make sure that your body has had enough time to produce the antibodies.

The tests can give false-positive results. You may want to repeat the test or try other forms of testing to confirm the results.

Performance Of Western Blot Method

Western blotting has been reported to be an accurate and sensitive test for detecting HSV antibodies and is considered the gold standard. Initially, we analyzed the performance of the WB-UB method. The detection of gG-1 and gG-2 proteins with this method, using type-specific monoclonal antibodies, is shown in Fig. Fig.1.1. Prominent bands were observed, and in some cases, additional bands of different molecular masses were also seen. It has been described previously that the monoclonal antibody LP10 also reacts with unglycosylated and fast-migrating forms of gG-1. Figure Figure11 also shows typical results from an analysis of human serum. Bands were detected at the positions for gG-1 and gG-2. Whenever such bands were observed, the serum was recorded as containing HSV-1 or HSV-2 antibodies, respectively. Human sera also reacted more or less strongly with proteins with molecular masses different from those of gG-1 and gG-2, which were most likely other glycoproteins of HSV-1 or HSV-2, but these reactions were not recorded or studied further.

To evaluate the performance of the WB-UB method, a total of 105 sera were selected as described in Results and also analyzed with a HerpeSelect 1 and 2 Immunoblot IgG kit from MRL Diagnostics. The specific antigens were recombinant gG-1 and gG-2 proteins on nitrocellulose strips. According to the manufacturer, the sensitivity and specificity of the immunoblot strips relative to Western blotting are 99% and 92%, respectively, for HSV-1, and 98% and 94%, respectively, for HSV-2.

Testing With No Symptoms

Blood tests can be used when a person has no visible symptoms but has concerns about having herpes. Blood tests do not actually detect the virus instead, they look for antibodies in the blood.

IgM vs. IgG

When an individual contracts herpes, the immune system responds by developing antibodies to fight the virus: IgG and IgM. Blood tests can look for and detect these antibodiesnot the virus itself. IgG appears soon after infection and stays in the blood for life. IgM is actually the first antibody that appears after infection, but it may disappear thereafter.

IgM tests are not recommended because of three serious problems:

The accurate herpes blood tests detect IgG antibodies. Unlike IgM, IgG antibodies can be accurately broken down to either HSV-1 or HSV-2. The challenge here is that the time it takes for IgG antibodies to reach detectable levels can vary from person to person. For one person, it could take just a few weeks, while it could take a few months for another. So even with the accurate tests, a person could receive a false negative if the test is taken too soon after contracting the virus.

For the most accurate test result, it is recommended to wait 12 16 weeks from the last possible date of exposure before getting an accurate, type-specific blood test in order to allow enough time for antibodies to reach detectable levels.

Genital Herpes Is Common Shouldnt Cdc Recommend Testing For Everyone

CDC recommends herpes testing for people who have genital symptoms to confirm if they have it. Testing allows a healthcare provider to talk with patients about what to expect in the future. This includes talking about medications that help with symptoms. Providers can also tell patients how to lower the risk of transmitting herpes to sex partner.

CDC does not recommend herpes testing for people without symptoms in most situations. This is because of the limits of a herpes blood test and the possibility of a wrong test result. The chances of wrong test results are higher for people who are at low risk of infection.

Blood tests might be useful if:

  • You have genital symptoms that could be related to herpes, or
  • You have a sex partner with genital herpes, or
  • Your provider found signs of herpes, but you still need a test to confirm it.

If you are sexually active, talk openly and honestly with your healthcare provider about what tests are right for you. These tips can help.

The Centers for Disease Control and Prevention, the Infectious Diseases Society of America, and the American Society for Microbiology all recommend the use of the IgG antibodies test over the IgM HSV test.

IgG tests are preferred because:

  • For herpes, IgG and IgM antibodies show up around the same time â normally, IgM antibodies appear first.
  • IgM antibodies may only last a few months and may not be made after the first outbreak â but IgG antibodies last indefinitely and are made during all outbreaks.
  • IgG antibodies can distinguish between HSV-1 and HSV-2 and IgM antibodies canât.

How Does Genital Herpes Affect A Pregnant Woman And Her Baby

Neonatal herpes is one of the most serious complications of genital herpes.5,16 Healthcare providers should ask all pregnant women if they have a history of genital herpes.11 Herpes infection can be passed from mother to child during pregnancy or childbirth, or babies may be infected shortly after birth, resulting in a potentially fatal neonatal herpes infection. 17 Infants born to women who acquire genital herpes close to the time of delivery and are shedding virus at delivery are at a much higher risk for developing neonatal herpes, compared with women who have recurrent genital herpes . 16,18-20 Thus, it is important that women avoid contracting herpes during pregnancy. Women should be counseled to abstain from intercourse during the third trimester with partners known to have or suspected of having genital herpes. 5,11

When Do You Need A Herpes Antibodies Test

Herpes simplex virus gD antibody

Herpes antibodies tests are not recommended when your doctor wants to diagnose an active infection. The best tests, in this case, are to collect a sample from you and attempt to culture the virus or test for the presence of its genome using a technique called polymerase chain reaction .

Should an active infection be detected with a PCR test, your doctor may prescribe antiviral medications to help prevent outbreaks or shorten their severity. When outbreaks occur when you are on these medications, theyâre typically shorter and create fewer lesions than when you arenât taking them.

Common antiviral medications include acyclovir, famciclovir, and valacyclovir.

Your doctor may recommend an antibody test in addition to these other tests for an active infection. But it is more likely that they will recommend this test for another reason, including if:

  • You have recurring symptoms but active test results are negative.
  • You believe you were recently exposed but donât yet have symptoms.
  • Youâre pregnant or trying to become pregnant.
  • You have HIV or have a high risk of encountering HIV.

Finding Out The Hard Way

No good data exist on how often patients with questionable positive results are actually re-tested. Until the 2015 update, CDC herpes testing guidelines had no mention of confirmatory testing for low-positive results, said Johnston. So patients often discovered the option not through their doctors, but through searching the web and reading online herpes forums.

That was the experience of Bryan, a 40-year-old man who lives in Indiana, who wrongly believed he had herpes for about two months in 2011. The misunderstanding actually put him at higher risk, he said: During those months he considered joining the hundreds of thousands of Americans on dating sites for herpes-positive people. Exclusively dating people with herpes would have increased his likelihood of contracting the virus.

The experience of YT, a 33-year-old mom who has suffered from frequent herpes symptoms over the last year, shows another side of the testing breakdown. She believes she was given HSV by a partner who didnt realize herpes wasnt included in his previous STD tests, she told STAT. Having herpes has caused her significant emotional trauma, and has driven her to permanently swear off dating. Had her partner known his true status, she wonders if her story would have been different.

People call and say, I just dont know whats going on, he said. Weve almost become de facto counselors.

Study Setting And Patient Recruitment

Patients who presented to the Sydney Sexual Health Centre between September 1995 and November 1998 within 4 weeks of the first onset of genital symptoms suggestive of genital herpes were asked to participate in the study. The study was approved by the South Eastern Sydney Area Health Service research ethics committee and written informed consent was obtained from all participants. A self administered questionnaire was completed, to obtain details of diagnosed or suspected previous oral and/or genital herpes infections, other past sexually transmitted infections , and recent sexual practices including condom use. In addition, information regarding recent sexual partner was obtained including whether the partner had oral and/or genital herpes or symptoms suggestive of herpes and whether antiviral medication was being used. The SSHC clinical staff performed a genital examination and a swab for viral culture was taken from genital vesicles and/or ulcers. Serum was taken for type specific HSV serology. Patients who were HIV antibody positive were excluded. Tests to exclude other STIs were performed if appropriate.

Testing for genital and oral herpes may be performed with a blood sample or a sample of fluid swabbed or scraped from a sore. Both types of samples are collected by a health professional when conducted at a doctors offices, clinic, or community organization.

If a patient is experiencing an outbreak, a doctor can collect a sample for testing by swabbing or scraping a sore. Material from the sore can be used for a herpes viral culture, PCR testing,

A Tzanck smear or an antibody test.

If a patient is not currently experiencing an outbreak, a blood test may be used to identify HSV antibodies. If a doctor suspects a brain infection with HSV, a lumbar puncture may be performed to obtain a sample of cerebrospinal fluid for analysis.

How Can I Prevent Genital Herpes

If you are sexually active, you can do the following things to lower your chances of getting genital herpes:

  • Being in a long-term mutually monogamous relationship with a partner who does not have herpes.
  • Using condoms the right way every time you have sex.

Be aware that not all herpes sores occur in areas that a condom can cover. Also, the skin can release the virus from areas that do not have a visible herpes sore. For these reasons, condoms may not fully protect you from getting herpes.

If your sex partner has/have genital herpes, you can lower your risk of getting it if:

  • Your partner takes an anti-herpes medicine every day. This is something your partner should discuss with his or her healthcare provider.

Im Pregnant How Could Genital Herpes Affect My Baby

If you are pregnant and have genital herpes, prenatal care visits are very important. Some research suggest that a genital herpes infection may lead to miscarriage or make it more likely to deliver your baby too early. You can pass herpes to your unborn child before birth, but it more commonly passes during delivery. This can lead to a deadly infection in your baby . It is important that you avoid getting genital herpes during pregnancy. Tell your healthcare provider if you have ever had a genital herpes diagnosis or symptoms. Also tell them about any possible exposure to genital herpes.

If you have genital herpes, you may need to take anti-herpes medicine towards the end of your pregnancy. This medicine may reduce your risk of having signs or symptoms of genital herpes when you deliver. At the time of delivery, your healthcare provider should carefully examine you for herpes sores. If you have signs or symptoms of genital herpes at delivery, a C-section is likely to occur.

Is One Virus Better Than The Other

Although incurable, both viruses are treatable and manageable in day-to-day life. Beyond this fact, each persons body chemistry and immune system are unique, and although the virus strains have their preferences, they are not set in stone. Developing awareness of your own body and becoming educated about the herpes simplex virus strains can help you make more sensible decisions and help defeat the stigma of a member of the herpesviridae family.
