In most cases of infection with the herpes simplex virus, you wonât have any symptoms at all or theyâll be very mild. Typically, your first outbreak is the most severe. Symptoms will periodically come back, but studies show that these bouts are shorter and less symptomatic than the first.

Breakouts typically happen in the genital region when you have HSV-2 and near your lips when you have HSV-1. But you can also get HSV-1 infections in your genital region.

Symptoms include:

  • Blisters that appear either individually or in clusters they can be quite painful and may ooze.
  • Soreness, pain, and burning at the infection site
  • Body aches and pains

Who Decides Which Diseases Are Notifiable And Why

The Council of State and Territorial Epidemiologists works with CDC to determine if a particular disease should be nationally notifiable. To make this decision, they follow a number of public health principles, for example, whether or not it is treatable whether or not it is preventable how common it is whether or not a public health response is needed whether or not there is a good source of information on the number of cases and whether or not it represents a significant public health threat . Information on HSV-1 and HSV-2 infections already exists in NHANES, so herpes infections have not been classified as notifiable diseases.

The following conditions are some of the most common reasons for the test. To figure out the exact reason behind the formation of painful blisters or sores around the lips or in the genital area. To check if the sexual partner of a person with genital herpes is also infected by the same. To check for neonatal herpes in new-borns, whose mothers are infected with genital herpes. The doctor will check the physical symptoms of the infection and order for the test. Some of the common symptoms are mentioned below. The occurrence of blistering sores in the mouth or on the genitals Any pain during urination. An itching or burning sensation around the affected area. These symptoms can be accompanied by other general symptoms like fever, swollen lymph nodes, headaches, tiredness and lack of appetite. In some cases, HSV can also affect the eyes and can cause symptoms like eye pain, discharge, and a gritty feeling in the eye. A few people have an increased risk of infection from the virus as mentioned below. People with recurrent genital sores or atypical symptoms who have negative results on HSV culture. People who have a sex partner who has been diagnosed with genital herpes. People who have been tested positive for other sexually transmitted diseases. Individuals diagnosed with an HIV infection.

Finding Out The Hard Way

No good data exist on how often patients with questionable positive results are actually re-tested. Until the 2015 update, CDC herpes testing guidelines had no mention of confirmatory testing for low-positive results, said Johnston. So patients often discovered the option not through their doctors, but through searching the web and reading online herpes forums.

That was the experience of Bryan, a 40-year-old man who lives in Indiana, who wrongly believed he had herpes for about two months in 2011. The misunderstanding actually put him at higher risk, he said: During those months he considered joining the hundreds of thousands of Americans on dating sites for herpes-positive people. Exclusively dating people with herpes would have increased his likelihood of contracting the virus.

The experience of YT, a 33-year-old mom who has suffered from frequent herpes symptoms over the last year, shows another side of the testing breakdown. She believes she was given HSV by a partner who didnt realize herpes wasnt included in his previous STD tests, she told STAT. Having herpes has caused her significant emotional trauma, and has driven her to permanently swear off dating. Had her partner known his true status, she wonders if her story would have been different.

Currently, the US Preventive Service Task Force advises against herpes screening in people without herpes symptoms, including pregnant women.

That doesn’t mean you shouldn’t get tested in certain circumstances. According to the Centers for Disease Control and Prevention, the HSV test can be used in people who are asymptomatic who:

  • Had sexual contact with someone with genital herpes
  • Will be tested for otherssexually transmitted infections
  • Do you have HIV and a history of genital herpes symptoms?
  • You are at increased risk of infection for any reason

Why The Test Is Performed

The test is done to find out whether a person has ever been infected with oral or genital herpes. It looks for antibodies to herpes simplex virus 1 and herpes simplex virus 2 . An antibody is a substance made by the bodys immune system when it detects harmful substances such as the herpes virus. This test does not detect the virus itself.

What Herpes Can Look Like

Firstly, the symptoms of herpes vary greatly from person to person. Herpes symptoms can sometimes resemble a paper cut, tear, pimple, blister, bump, cut, sore or a rash in the infected area.

Itching, burning and tingling is sometimes felt before any blister or sore appears. When the herpes virus is active and has broken out at the skins surface it typically develops into a fluid filled blister which ulcerates, begins to form a dry scab and then heals.

The herpes virus is normally only active on the skin for a short period of time before it retreats back inside the body, at which point the skin typically heals and returns to normal.

How Accurate Are The Herpes Tests

It can be incredibly stressful to wonder if you have herpes. Thats true whether youre concerned because of symptoms because you know youve just been exposed, or for any other reasons. One way to determine whether or not you have genital herpes or oral herpes infection is to get a herpes test.

Many people have questions about how accurate herpes tests are. The truth is that herpes blood tests work reasonably well, but no test is perfect.

As with all herpes viruses, once an individual is infected with the herpes simplex virus, or HSV, he or she is infected for life. The presence of antibodies against herpes simplex means an individual is currently infected with the virus.

The accuracy of a herpes blood test depends on two things: how many people in the tested population have herpes and which specific test was used.

This is a special early warning test you can get to detect Herpes as soon as possible. We draw a small amount of your blood for this test and check it for a certain type of antibody. This antibody is the front line soldier and holds back an infection while your body focuses on crafting the more specialized antibodies that come later. This front line soldier retreats after 4 weeks, as the main antibody army is finished. So this test WILL NOT WORK if its been over a month since your contact of concern.

How To Get Tested

Tests for genital and oral herpes are available at doctors offices, medical clinics, and community-based organizations that offer STD testing. Testing is often ordered by a doctor but may be available over the counter and at community-based organizations without a physicians orders.

You can order a herpes test from Testing.com with testing from a local lab. Or you can test for herpes as part of a complete STD panel.

Is An At Home Herpes Test Accurate

Herpes is a common sexually transmitted disease. Once youre exposed to this virus, it enters your nerve cells, where it can live for the rest of your life. Its possible for the virus to reactivate at any time. When its active, it may cause sores, and you may be able to pass the virus on to others. Even when you dont have sores, you may be able to pass on the virus.

In order to protect both themselves and their partners, many people are wondering how to check for herpes at home. How can you know whether youve been infected with this virus? Is there a rapid herpes test that you can get at home?

The problem is that blood tests for herpes when no lesions or symptoms are present are often bullshit. I realize that’s a pretty strong word, but after speaking with doctors who have administered these tests, it’s become more and more apparent to me how misunderstood and in some cases, just plain wrong the results they give are.

Part of the reason they’re flawed has to do with the fact that most people have been exposed to HSV-1 or oral herpes. “For HSV-1, close to 100 percent of people in the U.S. are antibody-positive because they have been exposed at some point in their lives, though only a percentage of these individuals will actually get cold sores,” says Adam Friedman, the residency program director at the George Washington University School of Medicine & Health Sciences in Washington, D.C. He adds that blood tests can be misleading because they usually don’t detect the herpes virus itself and instead look at the level of antibodies against the virus, which is merely a sign of immune response to it.

Herpes blood tests have a sensitivity level of about 80-98%. This type of test detects antibodies to the herpes virus, so it may not be as accurate when performed soon after infection. In general, false positives are a concern with herpes testing, so its not recommended unless you have symptoms or have a high risk for infection.

When tests fall between 1.1 and 3.5, more testing is necessary, said Johnston. This recommendation is also noted in the 2015 CDC Sexually Transmitted Diseases Treatment Guidelines. But some patients will never be referred for a second test.

I think most clinicians are unaware and perhaps labs dont have this available and/or it is not straightforward to order, Johnston said of second-step tests.

But while her facilitys Western blot is considered highly accurate, it is expensive and cumbersome to perform. Each test costs over $200 and the University of Washington is the only lab that provides it.

Other confirmatory tests also exist, for instance Biokits HSV-2 Rapid Test and Quests own HSV-2 IgG Inhibition assay. The latter, which adds only $4 to the price of the HerpeSelect test, performed well in a study conducted over a decade ago. Rick Pesano, the medical director for infectious disease at Quest, believes that with more awareness, the test could stand in for the Western blot. But the test was not mentioned in the USPSTF guidelines because it still has not been evaluated in asymptomatic individuals, according to Cindy Feltner, associate director of the RTI-UNC Evidence-based Practice Center, who helped prepare the science review for USPSTF.

We need better diagnostic testing. That is where we are stuck at this point, said Johnston. We dont have a good test thats inexpensive, high throughput, and reliable.

In The Absence Of Answers

Herpes tests arent the only ones with a risk of false positive results. False positives can occur for any test that diagnoses viral infection based on antibodies, i.e., your bodys immune reaction, rather than direct detection of the virus. For similar diagnostics like HIV and hepatitis C testing, protocols automatically call for a second test that directly detects the virus whenever an antibody test comes back positive, said Paul Swenson, laboratory director in the department of public health of King County, Washington. Herpes, however, is a particularly challenging infection to directly test for, because the virus spends most of its time hiding in nerves. Swab tests can sometimes detect the virus during outbreaks, but this isnt an option for people without symptoms. Thus even the Western blot relies on antibodies, and may give indeterminate results to a small number of people.

But two steps of antibody testing are still more reliable than one step todays diagnostics for Lyme disease and syphilis are a two-step antibody testing approach, said Dr. Edward Hook, a medical epidemiologist specializing in STI screening and prevention at the University of Alabama, Birmingham, who questioned why such a standardized two-step approach hasnt taken firm hold for herpes.

All of this tells us that sure, there are some currently available blood tests for herpes that can tell you your status or give you some idea of it but they’re still far from perfect. And when we’re dealing with a illness that’s highly and widely stigmatized the way herpes has become, most people are looking for airtight answers. This is precisely why so many doctors, as well as the CDC, do not recommend getting tested unless you have the common symptoms associated with herpes, such as itching, burning, or a blister.

What Do My Test Results Mean

There are two possible antibodies that your body can make to HSV-1 and HSV-2. These are IgM and IgG.

IgM is the antibody that is made first and typically represents a current or acute infection, although this may not always be the case.

IgG is made after the IgM antibody and typically will be present in the bloodstream for the rest of your life.

A negative test result is considered normal. This generally means that youve never contracted an HSV infection.

However, its possible for your results to come back negative even if youve contracted the infection within the past few months. This is referred to as a false negative.

Your body will typically take several weeks to develop IgG antibodies to HSV.

If youre tested earlier in your infection, its possible to have a false negative result. Your doctor may recommend that you return in 2 to 3 weeks to be retested.

A positive test result for HSV-1 or HSV-2 indicates that youve contracted either virus at some point.

The results also allow your doctor to differentiate between HSV-1 and HSV-2, which isnt always possible by visually examining the sores.

Depending on your results, you and your doctor can discuss ways to treat and prevent the transmission of your HSV infection.

When a serum antibody test is recommended for HSV, IgG detection is preferred. In fact, some laboratories are discontinuing their IgM tests in the future.

Are Test Results Accurate

Tests that identify evidence of HSV infections are routine and commonly used medical tests. The ability of a test to accurately diagnose or rule out HSV infection depends on the test used for analysis and the stage of a patients infection.

The Tzanck smear and viral culture are less able to accurately identify patients who have an HSV infection than other tests. PCR testing and antibody testing are more sensitive, which means that they miss fewer cases of infection.

Blood tests that detect antibodies to HSV can distinguish between HSV-1 and HSV-2 but not the site of infection. This means that blood tests cannot tell whether a patient has a genital or oral herpes infection.

Although these tests are valuable for identifying HSV infection, no test is perfect. When learning of test results, patients should talk to their doctor about the type of test conducted, the reliability of test results, and if additional tests are necessary to confirm or rule out a diagnosis.

What Abnormal Results Mean

A positive test means you have been infected with HSV recently or at some point in the past.

Tests can be done to help determine if you have a recent infection.

About 70% of adults have been infected by HSV-1 and have antibodies against the virus. About 20% to 50% of adults will have antibodies against the HSV-2 virus, which causes genital herpes.

HSV stays in your system once you have been infected. It may be asleep , and cause no symptoms, or it may flare up and cause symptoms. This test cannot tell whether you are having a flare-up.
