What Is Neonatal Herpes

Neonatal herpes can be a very serious infection. Babies generally contract the virus from their birthing parent during childbirth even if the parent doesnt have active lesions. They may also get HSV-1 if an adult with an active cold sore kisses them. Breastfeeding babies can also get HSV-1 from the breast if there are lesions present. But babies cant get herpes from breast milk, so its safe to pump and feed.

Herpes simplex can be more dangerous for young babies because they dont have a fully developed immune system. But most babies with neonatal herpes can recover fully with treatment.

The dangers of neonatal herpes are much higher if the infection spreads to the babys organs. If youre pregnant and have herpes, speak with your healthcare provider about how to lower the risk of passing the infection to your baby.

I was diagosed 13 years ago with Genital Herpes as the result of a very foolish short lived relationship.

The first attack was, as everyone says, the absolute worst . Since then I have had good years and bad years, sometimes many months with no attack. However over the last year I feel I have had back to back attacks. I am in misery, I have a couple of other health problems one of which is constant UTIâs and of course they are made worse by the herpes .

I know that plenty of people live with much worse conditions but this does seem to be a pretty bad thing to have.

Even when you do not have sores, you can pass the virus to someone during sexual or other close contact. To protect others:

  • Let any sexual partner know that you have herpes before having sex. Allow them to decide what to do.
  • Use latex or polyurethane condoms, and avoid sex during symptomatic outbreaks.
  • Do not kiss or have oral sex when you have a sore on the lips or inside the mouth.
  • Do not share your towels, toothbrush, or lipstick. Make sure dishes and utensils you use are washed well with detergent before others use them.
  • Wash your hands well with soap and water after touching a sore.
  • Consider using daily antiviral medicine to limit viral shedding and reduce the risk of passing the virus to your partner.
  • You may also want to consider getting your partner tested even if they have never had an outbreak. If you both have the herpes virus, there is no risk for transmission.

What’s Involved In Herpes Testing

If there are blisters or sores, a healthcare provider may gently swab the area to take a fluid sample and test it. If there are no sores, but you are concerned you may have been exposed, a blood test may done. Herpes tests are not normally recommended unless you have symptoms.

Use An Antiviral Medication

Topical over-the-counter meds dont work as well as prescription antivirals as cold sore treatments, says Bruce Robinson, M.D., a dermatologist in private practice in New York City and clinical instructor of dermatology at Lenox Hill Hospital. Over-the-counter creams may provide relief from the tingling and burning, but they arent the best option if you want sores gone fast.

The best way to treat cold sores is to start treatment earlyin that prodromal stagewith a prescription oral antiviral medication like valacyclovir , says Lorraine Young, M.D., co-chief of dermatology clinical services at UCLA.

A common regimen is to take 2,000 milligrams in the morning and again later on in the day. This “decreases the virus from replicating, so then it will help it to heal faster, says Dr. Young. It may even prevent the cold sore from erupting in the first place.

Theres even a newish way to get this medicine theres a form of acyclovir that you can put between your lip and gum on the side the cold sore is erupting on. It sits there and dissolves slowly throughout the day, offering your body a steady stream of the medicine to tame the virus.

Genital Herpes During Pregnancy

Prevention of neonatal herpes depends both on preventing acquisition of genital herpes during late pregnancy and avoiding exposure of the neonate to herpetic lesions and viral shedding during delivery. Mothers of newborns who acquire neonatal herpes often lack histories of clinically evident genital herpes . The risk for transmission to the neonate from an infected mother is high among women who acquire genital herpes near the time of delivery and low among women with prenatal histories of recurrent herpes or who acquire genital herpes during the first half of pregnancy . Women who acquire HSV in the second half of pregnancy should be managed in consultation with maternal-fetal medicine and infectious disease specialists.

All pregnant women should be asked whether they have a history of genital herpes or genital symptoms concerning for HSV infection. At the onset of labor, all women should be questioned thoroughly about symptoms of genital herpes, including prodromal symptoms , and all women should be examined thoroughly for herpetic lesions. Women without symptoms or signs of genital herpes or its prodrome can deliver vaginally. Although cesarean delivery does not eliminate the risk for HSV transmission to the neonate , women with recurrent genital herpetic lesions at the onset of labor should have a cesarean delivery to reduce the risk for neonatal HSV infection.

Acyclovir 400 mg orally 3 times/dayORValacyclovir 500 mg orally 2 times/day

What Do Genital Herpes Symptoms Look Like

Other symptoms can include:

  • tingling or burning around the genitals
  • feeling unwell, with aches, pains and flu-like symptoms
  • unusual vaginal discharge in women.

Many people with genital herpes wont get any symptoms, or may get symptoms for the first time months or even years after they were infected.

For most people, the blisters go away within one to two weeks. Although the outbreaks clear-up by themselves, the virus stays in the body. This means that people usually get blisters again which is called having a recurrent outbreak. Outbreaks usually become shorter and less severe over time.

Antibiotics strengthen a viral infection . Viruses naturally living in the body can store antibiotic-resistant genes, passing them out like candy to hungry bacteria and intensifying the problem of antibiotic resistance.

Antibiotic resistance is a very real public health crisis. When bacteria are resistant to antibiotic drugs, they are able to survive in the body in spite of antibiotic use.

According to a report from the Centers for Disease Control and Prevention in 2013, antibiotic resistance is directly responsible for at least 23,000 deaths per year.3 In addition to these entirely preventable deaths, antibiotic resistance has caused over 2 million illnesses per year, says the CDC. Unsurprisingly, antibiotic resistance has been referred to by CDC Director Tom Frieden as the next pandemic, where patients may enter the hospital with one disease and leave with another, as he described when speaking at a National Press Club event in 2014.4 In 2015, researchers confirmed in an article published in Pharmacy & Therapeutics that antibiotic resistance has become a global health crisis, as well as a national health and economic burden, due in large part to antibiotic overuse.5

When Is Treatment Necessary

Herpes can cause blisters to form on or around the mouth or genitals. These blisters then break, forming open, painful sores that may take at least 1 week to heal.

The symptoms may clear up on their own, and they may reappear later, in what some people call outbreaks or flare ups.

Once the virus enters the body, it never completely goes away. Some triggers that can cause herpes symptoms to reappear include:

  • friction in the genital area
  • a weakened immune system

Doctors may prescribe one of the following antiviral medications to help manage herpes symptoms. A person can also take some steps at home to ease the symptoms, and we describe these later in the article.

Can Antibiotics Cause Herpes Outbreak

Friction from intercourse can irritate tissue and trigger a herpes outbreak.

Obviously, that doesnt mean to avoid having sex forever or that youll break out if you do.

But it does mean if youre having early warning signs that an outbreak is coming on, like sensitivity or tingling, then wait for them to pass.

They may be prodrome symptoms that signal contagiousness and an impending outbreak. And making them worse might encourage the outbreak to happen.

So since sex can sometimes trigger outbreaks, just be careful to minimize irritation. Try using a water-based lubricant. However, lubricants that contain the spermicide nonoxynol-9 can irritate soft tissue of the vaginal walls.

What Happens If I Dont Get Treated

Genital herpes can cause painful genital sores and can be severe in people with suppressed immune systems.

If you touch your sores or the fluids from the sores, you may transfer herpes to another part of your body, such as your eyes. Do not touch the sores or fluids to avoid spreading herpes to another part of your body. If you do touch the sores or fluids, immediately wash your hands thoroughly to help avoid spreading your infection.

Mucocutaneous Herpes Simplex Infection

Lesions may appear anywhere on the skin or mucosa but are most frequent in the following locations:

  • Conjunctiva and cornea

Generally, after a prodromal period of tingling discomfort or itching, clusters of small, tense vesicles appear on an erythematous base. Clusters vary in size from 0.5 to 1.5 cm but may coalesce. Lesions on the nose, ears, eyes, fingers, or genitals may be particularly painful.

Vesicles typically persist for a few days, then rupture and dry, forming a thin, yellowish crust.

Healing generally occurs within 10 to 19 days after onset in primary infection or within 5 to 10 days in recurrent infection. Lesions usually heal completely, but recurrent lesions at the same site may cause atrophy and scarring. Skin lesions can develop secondary bacterial infection. In patients with depressed cell-mediated immunity due to HIV infection or other conditions, prolonged or progressive lesions may persist for weeks or longer. Localized infections can disseminate, particularlyâand often dramaticallyâin immunocompromised patients.

Herpes labialis is usually a recurrence of HSV. It develops as ulcers on the vermilion border of the lip or, much less commonly, as ulcerations of the mucosa of the hard palate.

Treatment If The Blisters Come Back

Go to a GP or sexual health clinic if you have been diagnosed with genital herpes and need treatment for an outbreak.

Antiviral medicine may help shorten an outbreak by 1 or 2 days if you start taking it as soon as symptoms appear.

But outbreaks usually settle by themselves, so you may not need treatment.

Recurrent outbreaks are usually milder than the first episode of genital herpes.

Over time, outbreaks tend to happen less often and be less severe. Some people never have outbreaks.

Some people who have more than 6 outbreaks in a year may benefit from taking antiviral medicine for 6 to 12 months.

If you still have outbreaks of genital herpes during this time, you may be referred to a specialist.

However, medication can help with symptoms, like sores, and outbreaks. Medication also lowers the risk of transmission to others. You can take most medications for herpes orally, though you may also apply some as a cream or receive them by injection.
