Antiviral medications can help to control the symptoms and shorten the duration of initial herpes outbreaks by 2 to 4 days. The treatment of an initial outbreak should be started within the first three days the earlier, the better.
The medications are taken as tablets. The number of tablets per day depends on the active ingredient and the dose. The World Health Organization recommends the following doses and amounts for treating an initial outbreak:
- aciclovir: 200 mg five times per day or 400 mg three times per day,

Management Of Sex Partners
The sex partners of persons who have symptomatic genital herpes can benefit from evaluation and counseling. Symptomatic sex partners should be evaluated and treated in the same manner as patients who have symptomatic genital herpes. Asymptomatic sex partners of patients who have symptomatic genital herpes should be asked about a history of genital symptoms and offered type-specific serologic testing for HSV-2. For partners without genital herpes, no data are available on which to base a recommendation for PEP or PrEP with antiviral medications or that they would prevent acquisition, and this should not be offered to patients as a prevention strategy.
What Are The Causes Of Genital Herpes
Genital herpes is spread by skin-to-skin contact with someone who has the HSV virus, usually during genital or oral sex. It can also be spread through kissing, foreplay or non-penetrative sex.
- HSV1 commonly causes cold sores on the lips or face.
- HSV2 causes most genital herpes.
The HSV virus is most easily spread when there are blisters or sores, but it can still be passed even if a person has no current blisters or sores or other symptoms.

Even if youre not experiencing an outbreak, using condoms is a good idea to prevent transmission.
Also, if youre wondering if your uninfected partner can take Valtrex to avoid becoming affected, the answer is no. Someone without herpes simplex virus shouldnt take medication for something they dont have. This could cause serious side effects or health problems.

Taking Valtrex Once a Day Reduces Risk of Spreading Herpes
Dec. 31, 2003 People with herpes who take an antiviral drug once a day can reduce the risk of transmitting the virus to their sexual partners by nearly 50%.
A new study shows that daily therapy with the antiviral drug Valtrex significantly lowers the risk of transmitting genital herpes among heterosexual couples in which one partner is infected with the herpes simplex virus type 2 .
Valtrex is commonly used to treat outbreaks of genital herpes, but preliminary results of this study prompted the FDA to approve a second indication for Valtrex in reducing the risk of transmitting the disease to susceptible heterosexual partners. The complete results appear in the Jan. 1, 2004, issue of TheNew England Journal of Medicine.

When Is Preventive Treatment Considered
Some people have outbreaks of genital herpes very often or experience especially severe symptoms. Then it may be a good idea to take antiviral medication for a longer time including during symptom-free phases. The goal is to prevent further outbreaks and to keep the symptoms from becoming so severe. Also, the antiviral medication reduces the likelihood of infecting others. As preventive treatment, the WHO recommends the following:
- aciclovir: 400 mg two times per day,
- famciclovir: 250 mg two times per day or
- valaciclovir: 500 mg once per day.
Research shows that preventive treatment can considerably lower the risk of further outbreaks. The people participating had four or more herpes outbreaks per year before the start of the studies, and then took an antiviral medication or a fake treatment for up to one year. A comprehensive analysis of the study results shows that
- 96 out of 100 people who took a placebo had at least one further outbreak of genital herpes.
- 46 out of 100 people who were given antiviral medication had at least one further outbreak of genital herpes.
In other words, the medication prevented further outbreaks in 50 out of 100 people within one year.
People with very frequent herpes outbreaks also benefited from preventive treatment: In one study involving people with very frequent outbreaks, the medication lowered the number of episodes in one year from 11 to 2.
What Is Suppressive Treatment For Genital Herpes
Suppressive treatment involves taking medication on a daily basis to reduce the chance of a genital herpes outbreak.
Taking antiviral medicines on a daily basis also reduces the amount of virus that is shed, both when sores are present and in between outbreaks, when the skin looks normal.
Suppressive treatment does not guarantee that you will not get a herpes outbreak, but should reduce the severity and frequency of herpes outbreaks.
Over time, your immune system tends to exert better control over the herpes virus, so your doctor may suggest stopping treatment every 6 months to see whether you have any further outbreaks. Treatment can be re-started if you do have a recurrence.

If you have frequent recurrences of genital herpes
Taking medication every day for many months will stop recurrences completely, or their frequency and severity are greatly reduced. This is called suppressive or prophylactic treatment. Generally, after a year, it is recommended you stop the medication for a few months to see if recurrences have become less frequent. This type of continuous treatment can be repeated if necessary. This type of daily treatment tends to be prescribed if you have frequent or severe recurrences of genital herpes more than six times per year.
Suppressive treatment is valaciclovir 500mg taken daily. This may be increased to 500mg taken twice daily on individual basis of clinical presentation and/or if you are having breakthrough recurrences on 500mg daily. The alternative is oral aciclovir 400mg taken twice daily .
A Doctor can give you a prescription every 3 months for up to 12 months and ongoing if your recurrences are still frequent.
Effectiveness is optimised if taken regularly as prescribed.
