There are two strains of the herpes simplex virus, HSV-1 and HSV-2.

HSV-1 is transmitted through oral-to-oral contact – spread through kissing, sharing cutlery, and oral care items such as toothbrushes with an infected person. It causes sores around the mouth and lips which are commonly known as cold sores.

Lesions do not have to be present for both strains to spread. Many people are asymptomatic and do not show symptoms or have very mild symptoms, even if they have a herpes infection.

If you are pregnant, follow these steps:

  • Tell your doctor if you have been exposed to genital herpes or have had an outbreak in the past.
  • Let your doctor know if you are currently having an outbreak, especially if you are in the last part of your pregnancy.
  • Avoid unsafe sex. Herpes is often transmitted by people who donât know they are infected and donât have symptoms. Use condoms.
  • Avoid receiving oral sex from partners who have cold sores. Herpes in newborns can be caused by HSV-1, the virus that most commonly causes cold sores. Most experts advise pregnant women not to receive oral sex in the last 3 months of their pregnancy. It increases their risk of genital infection with HSV-1.

Antiviral medicine can be used safely in pregnancy to reduce the risk of an outbreak at the time of delivery. This lower risk, in turn, makes it less likely that delivery by caesarean section will be needed.

How Is Genital Herpes Diagnosed

HSV nucleic acid amplification tests are the most sensitive and highly specific tests available for diagnosing herpes. However, in some settings viral culture is the only test available. The sensitivity of viral culture can be low, especially among people who have recurrent or healing lesions. Because viral shedding is intermittent, it is possible for someone to have a genital herpes infection even though it was not detected by NAAT or culture. 11

Type-specific virologic tests can be used for diagnosing genital herpes when a person has recurrent symptoms or lesion without a confirmatory NAAT, culture result, or has a partner with genital herpes. Both virologic tests and type-specific serologic tests should be available in clinical settings serving patients with, or at risk for, sexually transmitted infections. 11

Given performance limitations with commercially available type-specific serologic tests , a confirmatory test with a second method should be performed before test interpretation. If confirmatory tests are unavailable, patients should be counseled about the limitations of available testing before serologic testing. Healthcare providers should also be aware that false-positive results occur. In instances of suspected recent acquisition, serologic testing within 12 weeks after acquisition may be associated with false negative test results. 11

What Is The Treatment For Herpes Simplex

Mild, uncomplicated eruptions of herpes simplex require no treatment. Blisters may be covered if desired, for example with a hydrocolloid patch. Severe infection may require treatment with an antiviral agent.

Antiviral drugs used for herpes simplex and their usual doses are:

  • Aciclovir 200 mg 5 times daily for five days
  • Valaciclovir 500 mg twice daily for five days
  • Famciclovir as a single dose of 3 x 500 mg

In New Zealand, famciclovir is not currently funded by PHARMAC .

Higher doses and/or longer courses of antiviral drugs may be used for immunocompromised patients, eczema herpeticum, or for disseminated herpes simplex.

Topical aciclovir or penciclovir may shorten attacks of recurrent herpes simplex, provided the cream is started early enough.

In general, a person can only get HSV-2 infection during sexual contact with someone who has a genital HSV-2 infection. Both HSV-1 and HSV-2 can be spread even if sores are not present. How Can I Reduce My Risk Of Getting Genital Herpes

If you are sexually active, you can do the following things to lower your chances of getting genital herpes:

  • Using latex condoms the right way every time you have sex.

The incubation period is 212 days.

  • A thumb sucker may transmit the virus from their mouth to their thumb.
  • A health-care worker may develop herpetic whitlow

When the hair is not properly shaved on fragile skin, it can curl inside it, forming a bump, and can progress to pseudofolliculitis or even folliculitis.

The skin in the genital area is particularly fragile, and ingrown hair can easily occur there as a result of a nonoptimal shaving technique. The bump of ingrown hair is usually small and firm, but it can look like a blister, which makes it look like a herpes lesion. It can also be painful, but its pain does not resemble the distinctive burning pain of herpes. It also does not ulcerate, and if it pops open it usually heals very quickly, unlike herpes.

When there is recurrence of ingrown hair, inflammation can occur in that area and present as redness of the surface of the skin and a chronic recurrence of itchy buttons.
