How Common Is Genital Herpes

Genital herpes infection is common in the United States. CDC estimated that there were 572,000 new genital herpes infections in the United States in a single year.1 Nationwide, 11.9 % of persons aged 14 to 49 years have HSV-2 infection .2 However, the prevalence of genital herpes infection is higher than that because an increasing number of genital herpes infections are caused by HSV-1. 3 Oral HSV-1 infection is typically acquired in childhood because the prevalence of oral HSV-1 infection has declined in recent decades, people may have become more susceptible to contracting a genital herpes infection from HSV-1. 4

HSV-2 infection is more common among women than among men the percentages of those infected during 2015-2016 were 15.9% versus 8.2% respectively, among 14 to 49 year olds. 2 This is possibly because genital infection is more easily transmitted from men to women than from women to men during penile-vaginal sex. 5 HSV-2 infection is more common among non-Hispanic blacks than among non-Hispanic whites . 2 A previous analysis found that these disparities, exist even among persons with similar numbers of lifetime sexual partners. Most infected persons may be unaware of their infection in the United States, an estimated 87.4% of 14 to 49 year olds infected with HSV-2 have never received a clinical diagnosis. 6

The age-adjusted percentage of persons in the United States infected with HSV-2 decreased from 18.0% in 19992000 to 12.1% in 2015-2016. 2

When To Call The Doctor

Even though HSV sores can hurt and be unsightly, most cases of herpes do not cause serious illness. The sores go away in a few days. Contact the health provider if:

  • Sores are present and you think it might be a first herpes infection. The diagnosis is usually made by examining the sores. If the sores have not healed, sometimes a laboratory test is done.
  • Sores do not heal by themselves or show signs of infection such as pus, spreading redness or fever.

Testing For Genital Herpes

If there are symptoms present such as blisters, sores and ulcers, your doctor or nurse may be able to make a diagnosis straight away.

If you have visible blisters, your doctor or nurse may take a swab for testing, to check if this is herpes and what type. This swab can also test for another STI which causes blisters called syphilis.

The genital herpes swab tests are very reliable, though if the ulcer is too dry then it may be less likely to find a positive result.

What Does The Test Result Mean

Direct detection of the virus

  • A positive HSV DNA test or culture indicates that you have an active herpes infection, and the result may indicate if it is HSV-1 or HSV-2.
  • A negative PCR test or culture indicates that the herpes simplex virus was not detected, but this does not definitely rule out an infection. False negatives can occur if there is not enough active virus in the sample. This may happen if the lesion is cultured more than 48 hours after the symptoms appear. If suspicion of herpes remains high, repeat testing may be done.

Antibody blood tests

  • A positive HSV-1 or HSV-2 IgG antibody test means antibodies are present in your blood, which means you have had an infection in the past even if you havent noticed any symptoms. The type of antibody detected indicates the type of HSV infection.
  • Negative HSV antibody results mean that it is unlikely that you have been exposed to HSV or it may mean that your body has not had time to begin producing HSV antibodies.

Where Do Symptoms Appear

When a person has genital herpes, the virus lies dormant in the bundle of nerves at the base of the spine. When the virus reactivates , it travels nerve paths to the surface of the skin, sometimes causing an outbreak.

Most people infected with HSV-2 are not aware of their infection. However, the signs and symptoms of herpes during an initial outbreak can be quite pronounced. The first outbreak usually occurs within two weeks after the virus is transmitted, but the sores will also typically heal within two-to-four weeks. Other signs and symptoms during the primary episode may include a second crop of sores, and flu-like symptoms, such as fever and swollen glands. However, most individuals with HSV-2 infection never have sores, or they have very mild signs that they do not even notice or that they mistake for insect bites or other skin conditions like ingrown hairs or acne.

Type Of Tests Used To Diagnose Herpes

There are four main types of diagnostic tests that can be used to diagnose herpes. Your doctor will determine which type of test to use based on whether an outbreak is present or not.

If youre experiencing what you believe to be a herpes outbreak, your doctor can use a viral culture test or virus antigen detection test. If youre not experiencing symptoms, you can have an antibody test.

There Are Two Common Blood Tests Used To Identify Herpes And Theyre Often Misleading

I had a patient once who underwent this test, which was positive, and was told she has herpes, which to her was devastating and life-altering, Friedman says. She burst into tears when I explained the significance or lack thereof with this test, which is very misleading. There are two common blood tests used to identify herpes: IgG and IgM. The IgM test is still used by some doctors even though it has been denounced by the Centers for Disease Control and Prevention . According to the American Sexual Health Association , IgM tests can lead to deceptive results, as well as false assumptions about how and when a person acquired HSV.

And thats not all: IgM tests fail to accurately distinguish between HSV-1 and HSV-2 antibodies, leading them to provide false-positive results for HSV-2.
