Since outbreaks arent always obvious, its not always clear when youre contagious. There are a few days a year when herpes is active and possibly transmitted without any symptoms present, explains Wyand. This is known as viral shedding. This doesnt happen on most days, but its tricky, because theres no real way to know.

All this sounds really scary, but what it boils down to is the importance of practicing safe sex. While the risk of transmission will never be zero, there are some steps you and your partner can take to significantly reduce the risk of transmission: avoiding sex during an outbreak, using condoms. The partner with herpes can also go on suppressive therapy .

Dr. Rosser agrees that condoms are your best friends for any kind of sex. People still look at me like I have two heads when I suggest condoms for oral sex, she says. But if theres any question, then use them.

Communication is not only essential when it comes to having great sex, adds Dr. Rosser, but also when preventing the spread of herpes. If youre infected, be honest and consideratelet your partner know.

Maternal And Neonatal Herpes

If a woman is infected with the HSV during pregnancy, it is possible for the virus to be passed on to the newborn baby, normally during delivery. Although this is rare, the resulting neonatal herpes can pose serious risks to newborns.

Signs that a pregnant woman is infected with the herpes simplex virus are the same as those in non-pregnant people. The risk of neonatal herpes is highest in cases where the pregnant woman develops herpes for the first time in the late stages of pregnancy, as the body is unlikely to have the necessary antibodies for combatting the virus. Women with HSV in a latent form, i.e. who have had the virus previously without recurrence, have a very low risk of passing the virus onto the newborn baby.

Good to know: When acute/florid HSV infection is present around the time of the expected date of delivery, a Cesarean birth will usually be recommended before the waters break to avoid the transmission of infection from mother to child.

HSV-2 infection is more common among women than among men the percentages of those infected during 2015-2016 were 15.9% versus 8.2% respectively, among 14 to 49 year olds. 2 This is possibly because genital infection is more easily transmitted from men to women than from women to men during penile-vaginal sex. 5 HSV-2 infection is more common among non-Hispanic blacks than among non-Hispanic whites . 2 A previous analysis found that these disparities, exist even among persons with similar numbers of lifetime sexual partners. Most infected persons may be unaware of their infection in the United States, an estimated 87.4% of 14 to 49 year olds infected with HSV-2 have never received a clinical diagnosis. 6

The age-adjusted percentage of persons in the United States infected with HSV-2 decreased from 18.0% in 19992000 to 12.1% in 2015-2016. 2

Recurrent Episodes Of Genital Herpes

Recurrences are usually less painful and shorter in duration than the first episode of genital herpes. Over time, episodes usually become less frequent and may eventually stop altogether. Infections caused by HSV1 are less likely to recur in the genital area than infections caused by HSV2.

How Is Genital Herpes Spread

If you do not have herpes, you can get infected if you come into contact with the herpes virus in:

  • Saliva or genital secretions

You can get herpes from a sex partner who does not have a visible sore or who may not know he or she is infected. It is also possible to get genital herpes if you receive oral sex from a sex partner who has oral herpes.

Both genital herpes and genital warts are STIs, are spread through skin-to-skin contact, and are caused by a virus. But the viruses that cause genital herpes and genital warts are different:

Treatment Of An Initial Outbreak

The first outbreak of genital herpes is usually much worse than recurrent outbreaks. Symptoms tend to be more severe and to last longer. Your provider will prescribe one of the three antiviral medications, which you will take for 7 to 10 days. If your symptoms persist, treatment may be extended. An acyclovir ointment may also be prescribed for topical treatment of initial genital herpes.

Genital Herpes Treatment In Pregnancy

You may be offered antiviral treatment:

  • to treat outbreaks in pregnancy
  • from 36 weeks to reduce the chance of an outbreak during birth
  • from diagnosis until the birth if you first get herpes after 28 weeks of pregnancy

Many women with genital herpes have a vaginal delivery. You may be offered a caesarean, depending on your circumstances.
