Most people who are infected with HSV dont even know that they have the virus. In fact, nearly 80 percent of all babies who contract neonatal herpes are born to mothers who didnt report having HSV before.

The risk of passing HSV to the fetus varies greatly depending on when the mother was infected. If you were infected with herpes before your pregnancy and have a herpes outbreak while pregnant, the chance of passing the infection to your baby is less than 1 percent. There are many strains of the herpes virus, but the two that are commonly referred to as herpes are called herpes simplex virus type 1 and herpes simplex virus type 2. Approximately one third to half of all cases of neonatal herpes are caused by HSV-1. Its estimated that the incidence of neonatal herpes in the United States is 1 per 15,000 live births. However, these estimates vary greatly from location to location.

You may be offered antiviral treatment:

  • to treat outbreaks in pregnancy
  • from 36 weeks to reduce the chance of an outbreak during birth
  • from diagnosis until the birth if you first get herpes after 28 weeks of pregnancy

Many women with genital herpes have a vaginal delivery. You may be offered a caesarean, depending on your circumstances.

General Properties And Mechanism Of Action

Valacyclovir is well absorbed after oral administration absorption of a single dose of 1000 mg is 54% higher than that achieved after single 200- or 800-mg doses of oral acyclovir.7 Valacyclovir is rapidly metabolized to acyclovir, the triphosphorylated form of which selectively inhibits human HSV DNA polymerase, reducing viral DNA replication. Similarly, famciclovir is the oral prodrug of its active form penciclovir it is more stable than acyclovir triphosphate, as evidenced by longer half-lives intracellularly, which might account for the prolonged in vitro antiviral activity.8 Acyclovir 5% and penciclovir 1% creams are used to treat orolabial herpes . How Does Genital Herpes Affect Pregnancy

Herpes simplex virus doesnt affect fertility or your ability to conceive. Pregnant women diagnosed with HSV-2 should start a daily antiviral at 36 weeks of pregnancy as prescribed, to prevent outbreaks during delivery. If you have an active infection at the time of childbirth, you can pass the herpes virus to your baby. Neonatal herpes puts a baby at risk for blindness, brain damage, skin infections and death. Your healthcare provider will perform a cesarean section to lower this risk.

Herpes infections can be life-threatening for newborn babies. Because of this, many women who have genital herpes are afraid of passing herpes viruses on to their child while giving birth. But this only rarely happens.

How best to deal with genital herpes in pregnancy, and whether a natural birth is possible, will depend on

  • whether there are any signs of a genital herpes outbreak shortly before the birth and
  • when the woman became infected with the virus.

The risk of infection is much smaller if the woman already had genital herpes before, or at the beginning of, pregnancy. This is because her immune system will have had a chance to produce antibodies to the virus. The mother passes these antibodies on to her unborn child through her placenta, giving him or her a certain amount of protection.

How Can I Get Tested For Genital Herpes

If you have symptoms, the best test is a viral culture. To perform this test, your health care provider must take a sample from a herpes outbreak while it is active, preferably on the first day. Test results are available in about a week.

If you dont have symptoms, a blood test can tell you whether you carry HSV-2, the type of herpes that usually infects the genital tract. A blood test may also tell you whether you have HSV-1, but in many cases this simply means you have oral herpes.

Ask your doctor about these tests as they differ regionally and some tests are not accurate. Also the doctor needs to be aware how to interpret the test in light of the clinical presentation.

Neonatal herpes is a serious viral infection that can, sadly, be fatal . There are three subgroups of neonatal herpes depending on the site of infection:

  • skin, eye and/or mouth
  • affecting central nervous system
  • infection thats spread and involves multiple organs.

If neonatal herpes is just on the skin, eye and/or mouth, it has the best chance of being cured. If neonatal herpes has spread to multiple organs, thats when it can be much more serious .

How Can I Protect My Newborn From Getting Herpes

If your partner has herpes and you dont, do not have sex and oral sex the last few weeks of pregnancy. Condoms can reduce the risk of transmission but arent 100% effective. Theres no reason to risk a new infection close to delivery.

In the unlikely event your baby has been exposed, well treat the baby with antiviral medications. HSV cant be passed through breast milk, so unless you have sores or lesion on your breasts, breastfeeding is safe.

With visitors, do not allow anyone who has a cold sore or has had one recently to hold your baby. Same goes for people who have a cold or virus. Insist that anyone who wants to hold or touch your newborn wash their hands first this should be the norm anyway due to COVID-19.
