How To Treat Recurrent Genital Herpes

The Early Warning Signs Of Genital Herpes

Herpes virus treatment

Once you learn to identify the early warning signs of a genital herpes outbreak, you can take action to avoid the worst outbreak symptoms.

The most common sign of genital herpes is the development of blisters around the genital area. Herpes simplex blisters are often itchy and painful.

Other symptoms may accompany the blisters to indicate early warning signs of genital herpes:

  • -Difficult or painful urination

Testing For Genital Herpes

If you think you may have genital herpes you should make an appointment with your GP or local sexual health services.

If there are symptoms present such as blisters, sores and ulcers, your doctor or nurse may be able to make a diagnosis straight away.

If you have visible blisters, your doctor or nurse may take a swab for testing, to check if this is herpes and what type. This swab can also test for another STI which causes blisters called syphilis.

The genital herpes swab tests are very reliable, though if the ulcer is too dry then it may be less likely to find a positive result.

Treatment For Oral Herpes

The antiviral medications available in pill form have been specifically developed for the treatment of genital herpes. However, it is not uncommon for healthcare providers to prescribe the antiviral drugs to those who have frequent or severe outbreaks of oral herpes.

A recent study found valacyclovir to be effective for treating oral herpes in a one-day treatment of 2 grams taken at the first sign of a cold sore, and then again about 12 hours later.

There are two topical antiviral medications prescribed for the treatment of oral HSV symptoms: acyclovir ointment and penciclovir cream. Both work to speed up the healing process and reduce the viral activity. These topical drugs are put directly on the lesions themselves, but can also be used at the onset of prodrome.

Other topical treatments for oral herpes are available over-the-counter , but are not antiviral compounds like acyclovir and penciclovir. Some also contain ingredients that numb the area and induce temporary relief from the discomfort of an outbreak. Unfortunately, some OTC treatments may actually delay the healing time of symptoms because they can further irritate the area with repeated applications. There is only one OTC FDA-approved cream, called Abreva®, which has been clinically proven to help speed the healing process.

History Of Primary Genital Herpes

Symptoms may appear 2-20 days following exposure to infection with the virus. However, initial symptoms of genital herpes may not be recognised or may not occur until months to years later. Symptom severity differs markedly with severe cases having lesions lasting up to 3 weeks.

The prodrome is signalled by flu-like symptoms of fever, headache and general myalgia, accompanied by local tingling, irritation and/or pruritus or pain in the genital region. Rapidly, pruritic erythematous papules appear, followed by multiple small vesicles that contain clear to a cloudy fluid. These vesicles rupture within 1-2 days to form painful, sloughy, shallow ulcers with irregular margins, which may become confluent. The area may be oedematous and can be extremely tender. Pain on urination is typical, particularly in women and spontaneous urination may be impossible. The ulcers dry to form crusts and later heal, leaving a transient red macule with minimal scarring . Less commonly, lesions can pass through the blister phase quickly and blisters may not be noticed. Involvement of the cervix occurs but speculum examination may not be possible. Lesions may also appear extra-genitally, commonly on thighs and buttocks and less commonly on hands, lips, face and breasts. Local lymph nodes, i.e. inguinal nodes with genital infection, are usually enlarged and tender.

Women are more severely affected than men. Immunosuppressed people may develop a very extensive disease.

What Are The Complications Of Genital Herpes

Genital Herpes Treatment

Genital herpes may cause painful genital ulcers that can be severe and persistent in persons with suppressed immune systems, such as HIV-infected persons. 5 Both HSV-1 and HSV-2 can also cause rare but serious complications such as aseptic meningitis . 5 Development of extragenital lesions may occur during the course of infection. 5

There are also potential complications for a pregnant woman and her newborn child. See How does herpes infection affect a pregnant woman and her baby? below for information about this.

Screening And Early Detection

Unlike some STIs, routine blood testing is not recommended for people who dont have symptoms of herpes. However, if there is concern about exposure or new symptoms, this testing may be recommended. People should talk with a healthcare provider to determine whether blood testing is necessary.

Although blood testing is not routinely done, screening for STIs should include a thorough exam to check for herpes lesions on the genitals .

The CDC recommends blood testing if you have symptoms of genital herpes. Testing should also be pursued if you are pregnant and are having sex with someone with herpes.

Testing may also be useful if:

  • You are not pregnant but are having sex with someone with herpes.

If You Have Genital Herpes Already Can It Be Spread To Other Parts Of The Body Such As The Arms Or Legs

No. Genital herpes cannot be transmitted to another part of your body such as your arm, leg or hand after the first infection occurs. If you have genital HSV II, you will not get HSV II at another site in your body. The immune system produces antibodies that protect other parts of your body from infection. However, there are cases where a person has multiple site infections from the same virus. This is usually acquired at the time of the first infection. For example, if someone has never had herpes but then has oral and genital sex with an infected partner, they can acquire the infection at both sites.

Is There A Cure Or Treatment For Herpes

Herpes (oral & genital) – causes, symptoms, diagnosis, treatment, pathology

There is no cure for herpes. Antiviral medications can, however, prevent or shorten outbreaks during the period of time the person takes the medication.11 In addition, daily suppressive therapy for herpes can reduce the likelihood of transmission to partners.11

There is currently no commercially available vaccine that is protective against genital herpes infection. Candidate vaccines are in clinical trials.

Psychological Impact Of Recurrent Genital Herpes

Although genital herpes does not typically result in severe physical disability in immunocompetent patients, it can have a devastating psychological impact. The relationship between recurrent genital herpes and psychological morbidity has been well documented in the literature. Patients frequently worry about the social implications of having a chronic sexually transmitted disease. Furthermore, the risk of transmitting herpes to partners or loved ones and the fear of revealing their condition to new partners often leads to depression and social isolation. Patients also experience a loss of self-esteem and feel less sexually desirable. Finally, one study suggests that many patients feel that the disease significantly hampers their performance at school or at work.

Although these studies clearly demonstrate the negative impact of genital herpes on QOL, an assessment scale specific to herpes was not developed until recently .

So What Exactly Is Herpecillin And Herpecillin Plus

Herpecillin Plus is truly a game changer, because some people suffer from very aggressive strains of the genital herpes virus. Believe me when I say that we all know at least a few days before when another genital herpes outbreak is about to occur. It is at the first sign that you are about to have another genital herpes outbreak that you are taking 2 Herpecillin Plus supplements 3 times a day for 5 days and my promise to you is that on day 5 your genital herpes outbreak will have passed with no physical outbreak ever occurring.

Myth #: People Cannot Spread Herpes To Others Unless They Have Sores Or Blisters

Genital Herpes Treatment

Fact: People can spread herpes to others at any time, including when they do not have any symptoms.

People with herpes may experience outbreaks and remissions. During an outbreak, a person has active sores or blisters, but in remission, they may have no symptoms at all.

The virus is usually more contagious when a person has an outbreak, but it can also spread when it is in remission.

People of any age can contract herpes simplex. You are more likely to get the virus if you:

  • Were assigned female at birth.
  • Have had multiple sex partners.
  • Started having sex at a young age.
  • Have a history of any sexually transmitted infection .
  • Have a weakened immune system.
  • Dont use condoms for intercourse and dental dams for oral sex.

Who gets HSV-1, commonly known as oral herpes?

Anyone can get HSV-1. Most people contract HSV-1 during childhood. It spreads when an adult who has the virus has close contact with a child, such as when a family member kisses a child.

Who gets HSV-2, commonly known as genital herpes?

Genital herpes affects sexually active teens and adults of all genders and races. It can spread if you have multiple sexual partners and dont use condoms or dental dams.

People assigned female at birth are more at risk. Delicate vaginal tissue can tear, making it easier for the infection to get in. Black people who were AFAB are especially vulnerable, with an estimated 1 in 2 people AFAB between the ages of 14 and 49 infected with HSV-2.

Herpes Symptoms And Signs

Shingles (Herpes Zoster): Pathophysiology, Risk Factors, Phases of Infection, Symptoms, Treatment

In order to diagnose herpes, a health care provider can swab an area of visibly active herpes infection or, if symptoms arent active, a blood test can be given that measures the number of herpes antibodies present in the body. The antibodies dont indicate herpes itself, but rather show the immune systems response to the presence of the virus in the body. Its important to note that sometimes a swab can give false negative results since herpes lesions need to be large enough to yield enough detectable virus and if the outbreak is already healing it also may not be detected in a swab.

Antiviral Tablets To Stop The Herpes Virus Multiplying

Antiviral medicines taken by mouth, such as valaciclovir or acyclovir, are the most effective in controlling symptoms, although they cant cure genital herpes. They stop the virus from multiplying in body cells where the virus is present. The medicine only works while you are taking it. The infection can come back when you stop taking it.

Antiviral tablets are used in the following two ways:

  • To treat outbreaks as they happen : The aim is to shorten the time each outbreak lasts and to relieve symptoms. This works best when taken as soon as you get symptoms and before the blisters appear.
  • To prevent or reduce recurrences : If you get frequent or severe recurrent outbreaks, your doctor may recommend you take antiviral tablets every day to help prevent recurrences. Suppressive therapy is taken continuously, every day, for months or years. Suppressive antiviral therapy may help reduce the risk of transmitting the virus to sexual partners.

Antiviral creams bought over the counter from the pharmacy are not recommended as a treatment for a first episode or recurrent genital herpes as they are of little benefit. Antiviral tablets are the preferred treatment.

Suppressive Therapy For Recurrent Hsv

Suppressive therapy reduces frequency of genital herpes recurrences by 70%80% among patients who have frequent recurrences . Persons receiving such therapy often report having experienced no symptomatic outbreaks. Suppressive therapy also is effective for patients with less frequent recurrences. Long-term safety and efficacy have been documented among patients receiving daily acyclovir, valacyclovir, and famciclovir . Quality of life is improved for many patients with frequent recurrences who receive suppressive therapy rather than episodic treatment . Providers should discuss with patients on an annual basis whether they want to continue suppressive therapy because frequency of genital HSV-2 recurrence diminishes over time for many persons. However, neither treatment discontinuation nor laboratory monitoring is necessary because adverse events and development of HSV antiviral resistance related to long-term antiviral use are uncommon.

Valacyclovir 1 gm orally once daily for 5 days

*Acyclovir 400 mg orally 3 times/day is also effective, but are not recommended because of frequency of dosing.

Treatment Of Genital Herpes

Currently, the only commercially available treatments for recurrent genital herpes are the antiviral agents aciclovir, famciclovir and valaciclovir. These agents act to prevent viral replication, and thereby limit viral spread to other cells. They may be used for the primary outbreak, on an episodic basis for recurrences, or chronically as suppressive therapy.

Cold Sores On Lips Vs Angular Cheilitis

10 Ways to Prevent Herpes Outbreaks

These two are:

Cold sores or herpes labialis

img source: prod-carehubs.net

What causes cold sores or fever blisters? They are caused mainly by the herpes simplex virus type 1 .

Symptoms of HSV-1 on lips usually start with a burning, itchiness and tingling or tautness on lips or around the mouth. Due to irritation, the area is likely to turn red and small blister or tiny clusters of blisters that itch.

How long do cold sores last? They usually heal after a period of 2 to 3 weeks from infection time or when transmission of the HSV -1 virus occurs.

The symptoms will tend to vary with each stage of herpes simplex on lips. For instance, if individuals show the early signs of fever blisters during the first episode, they usually develop fever or illness depending on how severe the infection is.

If you have recurring fever blisters or cold sore symptoms, the following may be responsible for the activation of the virus after initial breakouts on your lips or face.

  • Medical conditions like anemia, and Down syndrome
  • Overactive salivary glands or if your mouth holds a lot of saliva

However, angular cheilitis is not a symptom of HIV since it is due to the compromised immune system that puts one at risk of it. Angular cheilitis can also last for days or possibly years.

Angular cheilitis symptoms

Like cold sores on lips, AC can also be symptomized by blistering, redness and swelling on the mouth corners.
