Yes. To prevent transmission of herpes, we recommend that condoms be used 100% of the time. Many patients will shed the virus and be contagious when they dont have symptoms. Studies have shown that asymptomatic shedding occurs between 1% and 3% of the time in patients with HSV II genital infections. Many new herpes infections occur from partners who are shedding the virus asymptomatically, so condoms are highly recommended.

Condoms may not be an attractive option for monogamous couples or for couples who desire to become pregnant. Couples may opt to have serological tests to determine if either partner has an asymptomatic infection. In close monogamous relationships, the risks of transmission can be weighed against other relationship issues, such as intimacy and pregnancy.

Unfortunately, there is no known cure for HPV or herpes. Once youve contracted either, it will stay with you for life even when signs arent present. The good news is chancre outbreaks and genital warts caused by both HPV and herpes can be treated. Keep in mind, these medications will only treat the outbreaks themselves and will not rid your body of the virus entirely.

In Vitro Susceptibility To Acyclovir

A total of 160 HSV isolates from 25 patients were randomly selected to test for resistance to acyclovir by the dye-uptake method. Of these, 25 isolates were HSV-1 and 135 were HSV-2. Of the isolates subjected to sensitivity testing, 121 were from the rectum, 6 were from the genitals, and 33 were from the oropharynx. The mean IC50 for these 160 isolates was 1.37 g/mL . There were no significant differences in the mean IC50 for isolates obtained from the oropharynx, genitals, or rectum or for isolates that were HSV-1 compared with those that were HSV-2 . Overall, 3 of 160 isolates, obtained from 3 different patients, were found to have reduced sensitivity to acyclovir in vitro . Two of the patients had used acyclovir during the study . Acyclovir-resistant HSV-1 was isolated from the oropharynx of a patient with antibodies to both HSV-1 and HSV-2 whose CD4 cell count was 510/mm3. Acyclovir-sensitive HSV-1 was isolated from the same site 6 days before and 11 days after the resistant isolate was obtained . Acyclovir-resistant HSV-2 was isolated from the rectum of 2 different patients with CD4 cell counts of 46 and 608/mm3, respectively. In both patients, acyclovir-sensitive HSV-2 was isolated from the same site both before and after isolation of the acyclovir-resistant HSV-2.

What Are The Types Of Herpes Viruses

The varicella-zoster virus is a type of herpes virus that causes chickenpox and shingles . Anyone who contracts the virus will experience chickenpox, with shingles possibly occurring decades later. Only people whove had chickenpox can develop shingles.

The risk of getting shingles increases as we get older, especially after age 50. Part of the reason for this is that our immune system weakens with age.

Clinical Management Of Zoster And Varicella

Herpes Simplex

Acute management of zoster lesions involves pain management with analgesics, local treatment and specific antivirals.

Antivirals

In general higher doses of antivirals are required for zoster than HSV infections and should be started within 72 hours of rash onset. Similar to treatment of HSV, antivirals for VZV reduce viral shedding and duration of disease. They may also reduce the risk of postherpetic neuralgia. Acyclovir, valacyclovir and famciclovir are the preferred first line drugs. Treatment is continued for 7-10 days or until lesions have crusted. Although no comparative trials have been performed, acyclovir is considered inferior because of its relatively poor bioavailability and more frequent dosing.

The comparative doses of the commonly used antivirals against zoster are listed in Box 25.4. Most HIV infected patients with localised zoster require only oral treatment as an outpatient. However, cutaneous dissemination and visceral involvement require intravenous acyclovir for treatment.

Acyclovir resistant zoster

Resistant zoster, generally presenting as persistent lesions refractory to treatment, is usually due to deficient or altered thymidine kinase. Cross resistance among the first line antivirals is expected. In this case, IV foscarnet at 40 mg/kg q8h may be effective.

Your physician can prescribe different medications to help reduce your symptoms and speed up the healing of an outbreak. These medicines work best when you start them soon after an outbreak occurs.

To reduce pain during an outbreak:

  • Sit in warm water in a portable bath or bathtub for about 20 minutes. Avoid bubble baths.
  • Keep your genital area clean and dry, and avoid tight clothes.
  • Take over-the-counter medications, such as acetaminophen or ibuprofen. Avoid aspirin.

Let your physician know if you are worried about your genital herpes. He or she can recommend a support group to help you cope with the virus.
