If youâre reading this, itâs likely that you or someone you care about may have herpes. Well, first, we want to remind you that HSV-1 and HSV-2 are exceptionally manageable viruses, and that the anxiety and fear youâre experiencing right now, while normal, may be unwarranted. Depending on the type of herpes virus youre infected with and your symptomatic status, your herpes outbreak frequency can become a common occurrence or be something youll rarely, if ever, have to deal with.

Why Is This Medication Prescribed
Acyclovir is used to decrease pain and speed the healing of sores or blisters in people who have varicella , herpes zoster , and first-time or repeat outbreaks of genital herpes . Acyclovir is also sometimes used to prevent outbreaks of genital herpes in people who are infected with the virus. Acyclovir is in a class of antiviral medications called synthetic nucleoside analogues. It works by stopping the spread of the herpes virus in the body. Acyclovir will not cure genital herpes and may not stop the spread of genital herpes to other people.
The Virus And Pathogenesis
HSV is a double-stranded DNA virus that may enter the host through abraded skin or intact mucous membranes.1 Epithelial cells are the initial targets. Once infected, these cells die, releasing clear fluid intradermally to form vesicles and merging with other cells to create multinucleated giant cells.
Retrograde transport through adjacent neural tissue to sensory ganglia leads to lifelong latent infection.1 Reactivation of the virus is triggered by local or systemic stimuli such as immunodeficiency, trauma, fever, menstruation, ultraviolet light and sexual intercourse.13 Although emotional stress is assumed to trigger HSV recurrence, recent research fails to show a definite causative role.4 Once reactivated, the virus is transported by the neuron back to the epithelium, where more replication occurs, and another outbreak ensues.
HSV exists as two separate types, labeled 1 and 2, which have affinities for different body sites.2 Ninety percent of infections caused by HSV-2 are genital, and 90 percent of those caused by HSV-1 are oral the reason for this division is unknown.5 In addition, oral HSV-1 infection recurs more frequently than oral HSV-2, and genital HSV-2 recurs more often than genital HSV-1.

How Do I Know If I Have Genital Herpes
Most people who have genital herpes have no symptoms, or have very mild symptoms. You may not notice mild symptoms or you may mistake them for another skin condition, such as a pimple or ingrown hair. Because of this, most people who have herpes do not know it.
Herpes sores usually appear as one or more blisters on or around the genitals, rectum or mouth. The blisters break and leave painful sores that may take a week or more to heal. These symptoms are sometimes called having an outbreak. The first time someone has an outbreak they may also have flu-like symptoms such as fever, body aches, or swollen glands.
People who experience an initial outbreak of herpes can have repeated outbreaks, especially if they are infected with HSV-2. Repeat outbreaks are usually shorter and less severe than the first outbreak. Although the infection stays in the body for the rest of your life, the number of outbreaks may decrease over time.
You should be examined by your doctor if you notice any of these symptoms or if your partner has an STD or symptoms of an STD. STD symptoms can include an unusual sore, a smelly genital discharge, burning when urinating, or bleeding between periods.

Topical Otc Treatments For Genital Herpes
Several over-the-counter topical options are available for genital herpes treatment, but evidence suggests they arent all effective.
Start with a few simple DIY topical compresses. Research shows that topical therapy can effectively alleviate symptoms of genital herpes outbreaks and even contribute to the prevention of outbreaks when used promptly.
Here are a few tried-and-true natural compress ideas to get you started:
- -Dry, warm compress using warmed rice in a sock
- -Cold compress using an ice pack or soft washcloth filled with ice

Not all herpes infections are the same. Herpes zoster is an infection caused by the varicella-zoster virus, which is the virus that causes chickenpox. Genital herpes is an infection caused by the herpes simplex virus . Medications can help decrease the severity of the herpes lesions and prevent recurrences, but they do not cure the infection. The dose of the medication is different, depending on which herpes virus is being treated, and whether the medication is for an initial episode or a recurrent episode or chronic treatment to prevent outbreaks.
Herpes Zoster
After an attack of chickenpox, the virus stays in the nerve cells. It is called a dormant infection. Anyone who has had chickenpox carries the virus. Shingles is more common in people with immune systems that are weakened from HIV infection, chemotherapy or radiation treatment, transplant operations, or stress. A stress on the body may allow the virus to become active and produce an “outbreak,” causing a painful rash or blisters.
Herpes Simplex Virus
There are 2 types of herpes simplex virus. HSV-1 most commonly infects the lips and produces what is often referred to as “cold sores”or “fever blisters,” but it also can cause genital herpes. HSV-2 is the usual cause of genital herpes, but it also can infect the mouth. HSV remains in the infected nerve cell of the body for life and can produce symptoms off and on.

Can I Be Treated To Prevent Genital Herpes Transmission To My Partner
Yes. A large study showed that if one partner has herpes and the other partner is uninfected, treating the infected partner with suppressive therapy can prevent transmission of symptomatic herpes in over 90% of cases. This is an option for couples who are interested in having unprotected sex or who are planning to become pregnant.
If you are entering into a new relationship and are aware that you have herpes simplex infection, you owe it to your partner to notify them before having sex.
Treatment For Genital Herpes
There are three antiviral medications that are FDA-approved for the treatment of genital herpes:
- Acyclovir: The oldest antiviral medication for herpes is acyclovir. It has been available since 1982 in a topical form and sold since 1985 in pill form. Acyclovir has been shown to be safe in persons who have used it continuously for as long as 10 years.
- Valacyclovir: A newer drug, valacyclovir, actually uses acyclovir as its active ingredient. This medication delivers acyclovir more efficiently so that the body absorbs much of the drug, which has the advantage of taking the medication fewer times during the day.
- Famciclovir: Famciclovir uses penciclovir as its active ingredient to stop HSV from replicating. Like valacyclovir, it is well absorbed, persists for a longer time in the body, and can be taken less frequently than acyclovir.
Antiviral medication is commonly prescribed for patients having a first episode of genital herpes, but they can be used for recurrent episodes as well. There are two kinds of treatment regimens: episodic therapy and suppressive therapy.

How Can I Best Learn To Cope With Herpes Simplex

Is There A Cure Or Treatment For Genital Herpes
There is no cure for genital herpes. However, daily use of antiviral medicines can prevent or shorten outbreaks. Antiviral medicines also can reduce the chance of spreading it to others.
Though several clinical trials have tested vaccines against genital herpes, there is no vaccine currently available to prevent infection. One vaccine trial showed efficacy among women whose partners had HSV-2, but only among women not infected with HSV-1. Efficacy was not observed among men whose partners had HSV-2. Another trial testing the same vaccine showed some protection from genital HSV-1 infection, but none from HSV-2.
Management Of Sex Partners
The sex partners of persons who have symptomatic genital herpes can benefit from evaluation and counseling. Symptomatic sex partners should be evaluated and treated in the same manner as patients who have symptomatic genital herpes. Asymptomatic sex partners of patients who have symptomatic genital herpes should be asked about a history of genital symptoms and offered type-specific serologic testing for HSV-2. For partners without genital herpes, no data are available on which to base a recommendation for PEP or PrEP with antiviral medications or that they would prevent acquisition, and this should not be offered to patients as a prevention strategy.

If You Have Genital Herpes Already Can It Be Spread To Other Parts Of The Body Such As The Arms Or Legs
No. Genital herpes cannot be transmitted to another part of your body such as your arm, leg or hand after the first infection occurs. If you have genital HSV II, you will not get HSV II at another site in your body. The immune system produces antibodies that protect other parts of your body from infection. However, there are cases where a person has multiple site infections from the same virus. This is usually acquired at the time of the first infection. For example, if someone has never had herpes but then has oral and genital sex with an infected partner, they can acquire the infection at both sites.
Blisters Inside The Urethra
The urethra is the tube that connects the urinary bladder to the genitals. In both men and women with herpes type-2, painful sores can form on the inner lining of this tube. When urinating, a person may feel a burning or razor blade sensation when urine passes over these sores. Unlike genital or mouth sores, the doctor may need to conduct tests to confirm a herpes infection when the urethra is affected.

Suppressive Therapy For Herpes
If you have frequent recurrences of genital herpes
Taking medication every day for many months will stop recurrences completely, or their frequency and severity are greatly reduced. This is called suppressive or prophylactic treatment. Generally, after a year, it is recommended you stop the medication for a few months to see if recurrences have become less frequent. This type of continuous treatment can be repeated if necessary. This type of daily treatment tends to be prescribed if you have frequent or severe recurrences of genital herpes more than six times per year.
Suppressive treatment is valaciclovir 500mg taken daily. This may be increased to 500mg taken twice daily on individual basis of clinical presentation and/or if you are having breakthrough recurrences on 500mg daily. The alternative is oral aciclovir 400mg taken twice daily .
A Doctor can give you a prescription every 3 months for up to 12 months and ongoing if your recurrences are still frequent.
Effectiveness is optimised if taken regularly as prescribed.
For special events
A course of medication may help to prevent a recurrence during special times. This may be an option even if you do not have frequent recurrences, but want to have the least risk of a recurrence – for example, during a honeymoon, holiday or during exams.
Prevent The Spread Of Herpes
If you have herpes, . Talk about what it is and what you and your partner need to know and do to protect themselves. Avoid skin-to-skin contact during outbreaks and refrain from all sexual activity during these times. In between outbreaks, you should still because there is still the potential of spreading the virus.
