Left alone, a cold sore will normally last 10 to 14 days, says Kakita. But if youre bothered by frequent, severe cold sores, it makes sense to see your doctor. Even if the cold sore develops, most people find that the outbreak will be milder, less painful, and shorter if theyre taking medication. Prescription medications such as acyclovir are available to fight the herpes simplex virus 1 responsible for cold sores, and they can stop a cold sore in its tracks, says Kakita. If a cold sore develops pus, seek medical attention, advises Rooney. You probably have a bacterial infection, which can benefit from antibiotic treatment.

If you find out that you have herpes, try not to freak out. There are a few ways that you can stop it from spreading to your partners and other parts of your body.

  • Talk with your doctor about taking herpes medication every day, which can lower your chances of spreading herpes.

  • Dont have sex during a herpes outbreak, even with a condom. There may be sores on places the condom doesnt cover.

  • Learn how to tell when an outbreak is coming, and stop having sex right when you notice these signs. You may feel a burning, itching, or tingling feeling that lets you know youre about to get sores.

  • Dont have sex until your sores are totally gone, and the scabs heal and fall off.

  • Dont touch your herpes sores, because you can spread the infection to other parts of your body or other people. If you touch a sore, wash your hands with soap and water right after.

  • Dont wet contact lenses with spit this might spread your oral herpes to your eye.

  • If you have a cold sore on your mouth, don’t kiss anyone especially babies, children, or pregnant women.

  • Always tell your sexual partners that you have herpes before you have sex, so you can work together to prevent it from spreading. Telling someone you have an STD can be hard, but herpes is super common and doesnt lead to serious health problems. So try not to be too embarrassed or stressed out about it.

How Is Genital Herpes Diagnosed

HSV nucleic acid amplification tests are the most sensitive and highly specific tests available for diagnosing herpes. However, in some settings viral culture is the only test available. The sensitivity of viral culture can be low, especially among people who have recurrent or healing lesions. Because viral shedding is intermittent, it is possible for someone to have a genital herpes infection even though it was not detected by NAAT or culture. 11

Type-specific virologic tests can be used for diagnosing genital herpes when a person has recurrent symptoms or lesion without a confirmatory NAAT, culture result, or has a partner with genital herpes. Both virologic tests and type-specific serologic tests should be available in clinical settings serving patients with, or at risk for, sexually transmitted infections. 11

Given performance limitations with commercially available type-specific serologic tests , a confirmatory test with a second method should be performed before test interpretation. If confirmatory tests are unavailable, patients should be counseled about the limitations of available testing before serologic testing. Healthcare providers should also be aware that false-positive results occur. In instances of suspected recent acquisition, serologic testing within 12 weeks after acquisition may be associated with false negative test results. 11

How Is Genital Herpes Spread

  • Saliva from a partner with an oral herpes infection
  • Genital fluids from a partner with a genital herpes infection
  • Skin in the oral area of a partner with oral herpes or
  • Skin in the genital area of a partner with genital herpes.

You also can get genital herpes from a sex partner who does not have a visible sore or is unaware of their infection. It is also possible to get genital herpes if you receive oral sex from a partner with oral herpes.

You will not get herpes from toilet seats, bedding, or swimming pools. You also will not get it from touching objects, such as silverware, soap, or towels.

What Are The Symptoms Of Herpes Simplex

Many people with the infection never experience any herpes symptoms. If you do notice symptoms, youll experience them differently depending on whether youre having your first herpes outbreak or a repeat outbreak. Recurring symptoms are usually milder than the first outbreak. Symptoms dont last as long with later outbreaks. Some people may only have one or two outbreaks during their lifetime. Others may have as many as four or five outbreaks a year.

People who do have herpes symptoms may experience:

  • Cold sores around their lips, mouth or tongue. They may look crusty or like fluid-filled blisters.
  • Tingling, itching or burning.
  • Pain while urinating.

Herpes simplex encephalitis is inflammation of the brain caused by either HSV-1 or HSV-2. It is a rare but extremely serious brain disease. Untreated, herpes encephalitis is fatal most of the time. Respiratory arrest can occur within the first 24 to 72 hours. Fortunately, rapid diagnostic tests and treatment with acyclovir have significantly improved survival rates and reduced complication rates. Nearly all who recover have some impairment, ranging from very mild neurological changes to paralysis.

Herpes Meningitis

Herpes simplex meningitis is inflammation of the membranes that line the brain and spinal cord. It is mainly caused by HSV-2. Like encephalitis, meningitis symptoms include headache, fever, stiff neck, vomiting, and sensitivity to light. Fortunately, herpes meningitis usually resolves after about a week without complications, although symptoms can recur.

How Do People Get Genital Herpes

Infections are transmitted through contact with HSV in herpes lesions, mucosal surfaces, genital secretions, or oral secretions. 5 HSV-1 and HSV-2 can be shed from normal-appearing oral or genital mucosa or skin. 7,8 Generally, a person can only get HSV-2 infection during genital contact with someone who has a genital HSV-2 infection. However, receiving oral sex from a person with an oral HSV-1 infection can result in getting a genital HSV-1 infection. 4 Transmission commonly occurs from contact with an infected partner who does not have visible lesions and who may not know that he or she is infected. 7 In persons with asymptomatic HSV-2 infections, genital HSV shedding occurs on 10.2% of days, compared to 20.1% of days among those with symptomatic infections. 8

Episodic Therapy For Recurrences

For a recurrent episode, treatment takes 1 to 5 days, depending on the type of medication and dosage. You should begin the medication as soon as you notice any signs or symptoms of herpes, preferably during the prodrome stage that precedes the outbreak of lesions.

In order for episodic therapy to be effective, it must be taken no later than 1 day after a lesion appears. If taken during prodrome, episodic therapy may help prevent an outbreak from occurring or reduce its severity. If taken at the first sign of a lesion, it can help speed healing.

How Do Dermatologists Diagnose Herpes Simplex

During an outbreak, a dermatologist often can diagnose herpes simplex by looking at the sores. To confirm that a patient has herpes simplex, a dermatologist may take a swab from a sore and send this swab to a laboratory.

When sores are not present, other medical tests, such as blood tests, can find the herpes simplex virus.

Special Considerations With Regard To Starting Antiretroviral Therapy

Orolabial and genital HSV should not influence the decision on when to start ART in persons with HIV. Transient increases in HSV-2âassociated genital ulcers have been observed during the first 6 months after initiation of ART in HIV/HSV-2 coinfected persons. In such cases, suppressive anti-HSV therapy can be considered. The frequency and severity of clinical episodes of genital herpes is often reduced in individuals after immune reconstitution on ART. However, immune reconstitution does not reduce the frequency of genital HSV shedding.33

Treatment For Oral Herpes

The antiviral medications available in pill form have been specifically developed for the treatment of genital herpes. However, it is not uncommon for healthcare providers to prescribe the antiviral drugs to those who have frequent or severe outbreaks of oral herpes.

A recent study found valacyclovir to be effective for treating oral herpes in a one-day treatment of 2 grams taken at the first sign of a cold sore, and then again about 12 hours later.

There are two topical antiviral medications prescribed for the treatment of oral HSV symptoms: acyclovir ointment and penciclovir cream. Both work to speed up the healing process and reduce the viral activity. These topical drugs are put directly on the lesions themselves, but can also be used at the onset of prodrome.

Other topical treatments for oral herpes are available over-the-counter , but are not antiviral compounds like acyclovir and penciclovir. Some also contain ingredients that numb the area and induce temporary relief from the discomfort of an outbreak. Unfortunately, some OTC treatments may actually delay the healing time of symptoms because they can further irritate the area with repeated applications. There is only one OTC FDA-approved cream, called Abreva®, which has been clinically proven to help speed the healing process.

Herpesvirus And Other Infections

A herpes prodrome or active herpes infection is also an absolute contraindication. Price and colleagues27 describe a case of fulminant hepatic failure and death due to herpes simplex infection after hysteroscopy. This case report detailed the unfortunate sequelae of performing hysteroscopy in a patient with herpes without mucocutaneous lesions. Although this is a rare condition, it reminds us that hysteroscopic procedures should be abandoned if active herpes or a prodromal herpetic episode is suspected. Herpes is a possible cause of postsurgical infection, especially in the setting of high fever, leukopenia, and abnormal liver function. Gynecologists performing hysteroscopy must remain vigilant about asymptomatic herpes when hysteroscopy is performed.

Microscopically inspect the vaginal discharge if it appears mucopurulent or malodorous, and obtain cervical cultures. Patients who have trichomoniasis, which is a sexually transmitted disease, might have a higher incidence of other STDs such as gonorrhea or Chlamydia trachomatis. It is far better to postpone the hysteroscopy by 1 week than to cause an acute pelvic infection.

ABSTRACT: Genital herpes continues to be a relevant public-health concern because of its prevalence and its association with an increased risk of acquiring other sexually transmitted diseases. Genital herpes may present as vesicular lesions that can rupture and become painful however, more than 80% of individuals are asymptomatic, which makes it difficult to contain the spread of disease. The treatment of genital herpes serves to shorten symptom duration and improve quality of life, but it does not cure the disease. Strategies to prevent transmission include chronic suppressive therapy in serodiscordant couples, safe-sex practices, and avoidance of sexual intercourse during outbreaks. Currently available therapies are well tolerated and highly effective, and some newer therapeutic options are being investigated.

Herpes Symptoms And Signs

Herpes (Herpes Simplex)

In order to diagnose herpes, a health care provider can swab an area of visibly active herpes infection or, if symptoms arent active, a blood test can be given that measures the number of herpes antibodies present in the body. The antibodies dont indicate herpes itself, but rather show the immune systems response to the presence of the virus in the body. Its important to note that sometimes a swab can give false negative results since herpes lesions need to be large enough to yield enough detectable virus and if the outbreak is already healing it also may not be detected in a swab.

The most common herpes symptoms include:

  • Developing either a single cold sore or cluster of multiple cold sores that form on the lips, inside the mouth, or on the genitals, buttocks and upper thighs.
  • Sores can sometimes be severe, painful and rupture and cause fluid to be secreted out.
  • Some herpes canker sores develop a thin, white coating and burn when touched while they healing.
  • Around herpes cold sores, its common to feel pain, tenderness and other symptoms of a rash, such as redness or signs of swelling.
  • Some people are able to tell before an outbreak if one is going to occur because they feel tingling, itchy sensations near the affected area.
  • Some people develop other symptoms during herpes outbreaks that are similar to those caused by a cold or the flu. This may include fatigue, irritability, aches or a slight fever.

How Is Herpes Simplex Diagnosed

Healthcare providers may diagnose herpes simplex based on how the sores look. Your provider may take a sample from the sore. Laboratory analysis of the sample can confirm or rule out the herpes virus.

If you dont have sores, your healthcare provider can use a blood test to check for HSV-1 and HSV-2 antibody, a marker showing youve been exposed to the virus. The blood test doesnt show an active infection . But it informs your provider whether youve been exposed to the herpes virus in the past. If this is your first infection, you may not test positive for herpes if there hasnt been enough time for your body to develop antibodies. The HSV-1and HSV-2 antibody test may be repeated in eight to 12 weeks.
