Innovations in Testing for the Detection of Lyme Disease

Many doctors also don’t inform patients that the test is not included, so when people do have symptoms and receive a diagnosis, there can be feelings of confusion and worry about whether a partner was dishonest or cheated, when their partner simply may not have known about the infection. In fact, by the time some people are diagnosed, it can be very difficult to determine how long they’ve had the infection. This means they may not have contracted it from their current partner.

Just about every place that tests for STIs, can also check for HIV/AIDS. Sometimes its included in the cost, and others its an added fee. It usually depends on the type of test you selected, and the policy of the clinic.

The GMHC is passionate about ending the ravages of this virus. Education, prevention and adequate testing are a few of their goals. There are 4 locations in Manhattan, and hours extend to Saturday. Walk ins are encouraged, but appointments can be made on Thursdays. Testing really is free, if you qualify.

The Callen-Lorde Center has been serving Manhattans LGBTQ community for more than thirty years. Along with HIV care, testing and education are two of their specialties. The center is on West 18th, and you can just walk in to see a professional. If money is an issue for you it wont be here, though this can result in sitting for hours.

Ever since AIDS and HIV came to everyones attention in the 1980s, New York hospitals have diligently operated test and treatment centers throughout the Big Apple. Mount Sinai and Montefiore Medical Center are two of the more well-known, and unfortunately this also translates into extended waiting times. Fees though are often waived or reduced, and insurance is always readily accepted.

These are excellent options, but discretion isnt always a top priority. Private HIV testing ensures this, and it is affordable no matter your pay scale.

Question 2 How Does The Hsv

The HSV-2 IgG inhibition assay measures the differential abilities of lysates of cells infected with HSV-1 or HSV-2 to neutralize patient sample reactivity to recombinant gG2 protein , the HSV-2specific protein used in the HerpeSelect HSV-2 IgG ELISA.2 Separate aliquots of the patient sample are incubated with the 2 lysates, then tested in the HerpeSelect HSV-2 type-specific IgG ELISA. The index value of serum pre-incubated with HSV-2 lysate is then compared to the index value of serum pre-incubated with HSV-1 lysate. The HSV-1 lysate controls for any non-gG2 reactivity present in the serum sample thus, any inhibition that is detected reflects only the absorption of gG2 reactive antibodies by the native gG2 present in the HSV-2 lysate. An example of how the inhibition value is calculated is shown below:

Index value for serum pre-incubated with HSV-2 lysate = 0.23

Index value for serum pre-incubated with HSV-1 lysate = 2.45

Inhibition = x 100 = x 100 = 0.91 x 100 = 91%

Inhibition values > 60% indicate true HSV-2-specific IgG reactivity

Trypanosoma cruzi

Chagas is a serious disease caused by the parasite Trypanosoma cruzi. The agent is endemic in Latin America, but approximately 20 reports of transmission by blood transfusion have been published worldwide. The Red Cross blood donations are screened using the Ortho T. cruzi Enzyme-Linked Immunosorbent Assay for the qualitative detection of antibodies to T. cruzi in human serum or plasma samples. An FDA licensed enzyme strip immunoassay is used for confirmatory testing. The FDA has approved a T. cruzi reentry algorithm requiring a follow-up sample testing nonreactive by the two FDA licensed screening tests and the ESA. Although T. cruzi can be transmitted by blood transfusion, to date, the Red Cross has not identified any recipients infected by blood components from screen-negative donors or screened-negative donors who subsequently tested positive . All reports of transfusion transmission have been from unscreened platelets, except from one red cell case, or from whole blood from unscreened donors in Latin America. Because T. cruzi is not endemic in the United States, the Red Cross donors are tested only once. The frequency of detecting a positive donor is about 1 per 15,000 first-time donations screened.

Use Of Type Specific Tests In Paediatric Sera

Only the HSV-1/HSV-2 gG based Premier type specific test combination from Meridian was tested in paediatric populations in FDA clinical trials. The results prompted a warning in the kit against use of the tests for herpes diagnosis in children. Recently we conducted a blinded study between UW WB and Meridian Premier HSV-1 and HSV-2 kits in sera from 97 children ages 114. The Meridian kits had a 54% specificity for HSV-1 and a 47% specificity for HSV-2. The NHANES survey revealed an HSV-2 seroprevalence of 0.25% in children using immunodot enzyme assay. Eis-Hubinger et al found an HSV-2 seroprevalence of 4% among children with the Gull test and 3% with the Cobas test. Paediatric sera accounted for nearly all of the false positive results in this large comparison study. Performance in paediatric sera by the Focus, POCkit, and other commercial tests are not known. Thus, these tests should be used with caution, if at all, in children under 14.

How To Read Your Results

Your HSV test results will likely be reported as negative or positive :

  • Negative. The herpes virus was not found. However, you may still have an HSV infection if your results were negative. It may mean the sample didnt have enough virus to be detected. If you still have symptoms of herpes, you may need to order another test.
  • Positive. HSV was found in your sample. It may mean you have an active infection or contracted the virus in the past .

If your test results are positive for HSV, talk with a doctor about your next steps.

Importance Of Laboratory Diagnosis Or Testing For Genital Herpes

Genital herpetic infection is mainly diagnosed on clinical grounds, especially when the clinical picture is classical, with the presence of typical papular lesions progressing to vesicle and ulcerative lesions which finally crust, associated with local adenitis and in recurrent cases preceded by prodromal . However, clinical diagnosis of genital herpes may be limited in accuracy. The clinical differentiation of genital HSV infection from other infectious and non-infectious etiologies of genital ulceration is often difficult and laboratory confirmation of the infection should always be sought . Besides classic vesicular lesions, HSV genital infection may be associated with other clinically atypical presentations. These include either unusual sites or atypical morphological forms of genital disease . Meningitis may be observed during phases of primary infection and reactivation and can also confuse the diagnosis of genital HSV infection . Accordingly, exclusive reliance on clinical diagnosis could lead both to false positive and false negative diagnosis of the condition. Thus, a clinical diagnosis of genital herpes should be confirmed with laboratory tests .

Requesting A Genital Herpes Virus Test

Visit a physician and ask for a type-specific blood test. There are many herpes testing options but you are unlikely to have a false positive herpes test if you undergo one of the latest type-specific serological tests. A serological test for HSV can determine the difference between HSV-1 and HSV-2.

Anyone who fears they have received a herpes false positive test result may be exploring various other herpes testing options. Many experts agree that the molecular tests are best for diagnosing genital infections caused by the herpes simplex virus. Culture tests fail to detect the virus in many patients whereas the molecular test appears to confirm and clearly identify whether the person has HSV-1 or HSV-2.

Naturally, anyone who has been diagnosed with HSV will be concerned. If you feel that you have been handed a false positive herpes test result, you should discuss taking another test with your physician.

Indirect Serological Diagnosis Of Herpetic Infections

Detection of HSV-specific IgG antibodies can be done sensitively by several immunological methods. Serologic diagnosis of HSV infections and HSV type-specific antibody testing are summarized in Table , and commercially available assays approved by the Food and Drug Administration in Table . Accurate type-specific HSV serologic assays are based on the detection of HSV-specific gG1 and gG2 antibodies using native, purified or recombinant gG1 or gG2 as antigens. Serological assays based on antigen preparations from whole virus or from crude infected-cell protein mixtures detect predominantly type-common antibodies, may have low sensitivity in detecting HSV-2 antibodies in HSV-1seropositive patients, or may incorrectly type antibodies in patients with only HSV-1 or HSV-2 infection. Some commercial assays described as type-specific are actually based on relative reactivity of serum antibodies to crude preparations of HSV-1 versus HSV-2 antigens. The accuracy of such tests for HSV-2 antibody detection is low compared with glycoprotein Gbased tests, and their use is not recommended .

Finding Out The Hard Way

CBS2’s Dr. Max Gomez Infected With Lyme Disease

No good data exist on how often patients with questionable positive results are actually re-tested. Until the 2015 update, CDC herpes testing guidelines had no mention of confirmatory testing for low-positive results, said Johnston. So patients often discovered the option not through their doctors, but through searching the web and reading online herpes forums.

That was the experience of Bryan, a 40-year-old man who lives in Indiana, who wrongly believed he had herpes for about two months in 2011. The misunderstanding actually put him at higher risk, he said: During those months he considered joining the hundreds of thousands of Americans on dating sites for herpes-positive people. Exclusively dating people with herpes would have increased his likelihood of contracting the virus.

The experience of YT, a 33-year-old mom who has suffered from frequent herpes symptoms over the last year, shows another side of the testing breakdown. She believes she was given HSV by a partner who didnt realize herpes wasnt included in his previous STD tests, she told STAT. Having herpes has caused her significant emotional trauma, and has driven her to permanently swear off dating. Had her partner known his true status, she wonders if her story would have been different.

People call and say, I just dont know whats going on, he said. Weve almost become de facto counselors.

Reducing The Risk Of Genital Herpes

The herpes virus cant pass through a condom. However, if the virus is present and active on the skin in areas around the genitals not covered by the condom , infection may still occur. Therefore condoms are not 100% protective against the herpes virus.

If your partner has herpes, avoiding sex when they have visible sores reduces your risk of getting herpes.
