How Should This Medicine Be Used

Topical acyclovir comes as a cream and an ointment to apply to the skin. Acyclovir cream is usually applied five times a day for 4 days. Acyclovir cream may be applied at any time during a cold sore outbreak, but it works best when it is applied at the very beginning of a cold sore outbreak, when there is tingling, redness, itching, or a bump but the cold sore has not yet formed. Acyclovir ointment is usually applied six times a day for 7 days. It is best to begin using acyclovir ointment as soon as possible after you experience the first symptoms of infection. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Use topical acyclovir exactly as directed. Do not use more or less of it or use it more often than prescribed by your doctor.

Your symptoms should improve during your treatment with topical acyclovir. If your symptoms do not improve or if they get worse, call your doctor.

Acyclovir cream and ointment are for use only on the skin. Do not let acyclovir cream or ointment get into your eyes, or inside your mouth or nose, and do not swallow the medication.

Acyclovir cream should only be applied to skin where a cold sore has formed or seems likely to form. Do not apply acyclovir cream to any unaffected skin, or to genital herpes sores.

Herpes labialis affects 1 in 5 Americans each year and accounts for the largest reservoir of herpes simplex virus infections. The frequency of recurrence of herpes labialis has been estimated to be approximately 33%. In the United States, penciclovir is the only FDA-approved topical treatment option. Topical acyclovir is also used, but is not FDA approved. Abreva is an over-the-counter alternative to these agents. It is currently the only FDA approved nonprescription medication available for the treatment of cold sores.

Mechanism of action

Antiviral agents such as acyclovir and penciclovir prevent viral replication through their competitive inhibition of viral DNA polymerase. In contrast, docosanol appears to inhibit replication by blocking viral entry into cells. Incorporation of docosanol into the plasma membrane is believed to alter the normal membrane to inhibit the fusion of virons to the host cell. Unlike the nucleoside analogue agents, acyclovir and penciclovir, docosanol does not block replication shared by the virus and the infected target cell. This may possibly prevent the induction of drug-resistant viral mutant strains that can possible occur with the use of nucleoside analogues

If you have a stubborn case of genital herpes or are experiencing many recurrences, you may want to consider seeing your doctor for a prescription of valacyclovir , a drug that reduces frequency of attacks, limits their severity, and speeds healing time. This drug, approved by the FDA in 2001, stays in the body longer than acyclovir , so you have to take only one pill a day to fight off recurrences. If you are having your first attack or your recurrences are frequent, or if you believe them to be frequent, talk to your doctor. If you are pregnant, be sure to tell your doctor that you have herpes, since the virus can infect newborns. A strong link was once suspected between genital herpes and cervical cancer. That link is not as strong as once thought, but its still important for women with herpes to get a yearly Pap test.

Where Does The Herpes Virus Live In The Body

The herpes simplex viruses are latent, meaning they can live in the body without causing symptoms. After the initial infection, the virus gets into the nerve roots and spreads to the sensory nerve ganglia, the junctions where nerves from different parts of the body come together. For the genital area, the ganglia are adjacent to the spinal cord in the lower back. For orofacial herpes , the ganglia are located behind the cheek bone.

The virus can be spread when someone with HSV has an episode or an outbreak characterised by having a sore, blister, ulcer or skin split . It can also be spread between episodes, when there is no sore, blister, ulcer or skin split present called asymptomatic viral shedding.

However, it takes time to transition potential treatments from animals to humans. In a 2020 news article, one of the researchers that authored the Nature paper estimated that it would be at least 3 years before human trials.

Scientists are also researching the possibility of creating a vaccine to prevent herpes. A 2022 study outlined a new approach to developing an mRNA vaccine for herpes. The vaccine mentioned in the study worked well in mice and guinea pigs.

The researchers say that theyre aiming to conduct human trials before the end of 2022, and the vaccine could be available within the next 10 years.

I Had Lesions When I Was Evaluated But My Culture Was Negative Does This Mean That I Dont Have Herpes

Not necessarily. Although lesions can be caused by something other than herpes, false negative herpes tests can occur if the samples are not taken appropriately, if there is a long transport time between the clinic and the laboratory, or if cultures were taken late in the course of the lesions. Lesions that occur early in the course of a herpes outbreak are much more likely to have positive cultures than cultures taken after the lesions crust over.

How To Tell Your Partner You Have Herpes

Before we get into the practical side of having sex with herpes, its important to cover another topic that many infected people worry about telling their sexual partner about their infection status.

If you are aware that you have herpes, you need to tell your sexual partner. Even when you avoid sex during outbreaks, use condoms and follow other safe sex practices, theres still a risk of transmitting the virus. This makes it essential that your sexual partners are informed.

Many people feel anxious about telling their romantic interest that they have herpes, for reasons that are very understandable. Nobody likes to disclose that they have an STD, especially to the person theyre sexually and romantically interested in.

However, done the right way, letting your partner know that you have herpes doesnt have to be a stressful or negative experience.

First, before you disclose to anyone that you have herpes, its important to check which type of the herpes virus youre infected with. Our guide to herpes tests covers the most common herpes testing methods and explains how you can get tested to see if you have HSV-1 or HSV-2.

If you have HSV-1 and only develop herpes sores on your lips, disclosing your status to your sexual partner is fairly straightforward. After all, an estimated 67 percent of the worlds population under the age of 50 have HSV-1, meaning theres a good chance that your partner has the virus.

    How To Apply Aciclovir Cream

    • Before you start the treatment, read the manufacturer’s printed information leaflet from inside the pack. It will give you more information about using aciclovir cream and will provide you with a full list of the side-effects which you could experience from using it.
    • Apply the cream five times a day. The best way to do this is to use it every four hours during the day . Dab the cream on to the area rather than rubbing it in. This will minimise any damage to the blisters, which could cause pain and spread the virus around.
    • Start using the cream at the first sign of infection and continue to use it for five days. This is often sufficient time for the sore to heal but you can continue to use the cream for a further five days if needed. Remember to wash and dry your hands before and after using aciclovir, to prevent spreading any infection.
    • If you forget to use the cream, apply it as soon as you remember and then continue using it again at your usual times.
