Herpes And Female Fertility
Herpes can impact female fertility only in an indirect way: a herpes infection may reduce your ability to get pregnant by making it more difficult to have unprotected sex. During an active herpes outbreak, having unprotected sex is not recommended, as the virus could spread. Fortunately, people with herpes can resume having unprotected sex after the outbreak has passed.
How Do I Know If I Have Genital Herpes
Most people with genital herpes have no symptoms or have very mild symptoms. Mild symptoms may go unnoticed or be mistaken for other skin conditions like a pimple or ingrown hair. Because of this, most people do not know they have a herpes infection.
Herpes sores usually appear as one or more blisters on or around the genitals, rectum or mouth. This is known as having an outbreak. The blisters break and leave painful sores that may take a week or more to heal. Flu-like symptoms also may occur during the first outbreak.
People who experience an initial outbreak of herpes can have repeated outbreaks, especially if they have HSV-2. However, repeat outbreaks are usually shorter and less severe than the first outbreak. Although genital herpes is a lifelong infection, the number of outbreaks may decrease over time.
Ask a healthcare provider to examine you if:
- You notice any symptoms or
- Your partner has an STD or symptoms of an STD.
STD symptoms can include an unusual sore, a smelly genital discharge, burning when peeing, or bleeding between periods .

Facts And Statistics About Herpes
- According to the World Health Organization, an estimated two-thirds of the global population under 50 years old are infected with herpes simplex virus type 1 .
- Estimates for HSV-1 prevalence in the U.S. among people aged 049 years are 178 million women and 142 million men .
- In total, about 75 percent or more of the American adult population has the HSV-1 virus that usually causes oral herpes , and about 20 percent to 30 percent have HSV-2 that usually causes genital herpes.
- Men and women are affected by HSV-1 about equally. Roughly 6 million women in the U.S. become infected with HSV-1 each year, along with 5 million men.
- Slightly more women than men are affected by genital herpes, whether from HSV-1 or HSV-2. Middle-aged women are most likely to acquire genital herpes.
- Transmission of HSV most often occurs without symptoms, so its estimated that 85 percent of people with genital herpes dont know it. Many dont have any symptoms at all after the initial infection, and only about 15 percent ever receive a HSV-2 infection diagnosis in their lifetimes.
- Most primary oral/facial HSV infections are caused by HSV-1 however, facial infection by HSV-2 is increasingly becoming common.
- Less children are becoming infected with HSV-1 than in the past in high-income countries, mostly due to better hygiene and living conditions.
- After someone comes into contact with an active herpes outbreak on another person, the incubation period takes about one week.

Does Herpes Need To Be Treated
Treating herpes simplex symptoms may not be necessary. Herpes simplex infections clear up even if no treatment is given. But treating genital herpes can be helpful particularly if there are severe symptoms. Some people are affected more than others. See below for the rare occasions when it can be serious.
Hepatitis C Virus Infection
Persons newly infected with HCV typically are either asymptomatic or have a mildclinical illness. HCV RNA can be detected in blood within 13 weeks afterexposure. The average time from exposure to antibody to HCV seroconversion is 410 weeks, and anti-HCV can be detected amongapproximately 97% of persons by 6 months after exposure .
Chronic HCV infection develops among 75%85% of persons with HCV infection, and10%20% of persons with chronic infection develop cirrhosis in2030 years of active liver disease . The majority of infected persons remainunaware of their infection because they are not clinically ill. However, infected persons are a source of transmission to others and are at risk forcirrhosis and hepatocellular carcinoma decades after infection.
Acute HCV infection is a reportable condition in 49 states. Matching viralhepatitis and HIV surveillance registries, and molecular epidemiologicassessments, can facilitate early detection of social networks of HCVtransmission among MSM with HIV infection.
CDC recommends hepatitis C screening at least once in a lifetime for all adultsaged 18 years and for all women during each pregnancy, except insettings where the prevalence of HCV infection is < 0.1% . One-time hepatitisC testing is also recommended regardless of age, setting, or recognizedconditions or exposures . Routine periodic HCV testing isrecommended for persons with ongoing risk factors .

If You Have Genital Herpes Can You Donate Blood
Before discussing whether you can donate blood when you have herpes, we need to answer the question: Can herpes be transmitted through blood donation?
Previously, blood collection facilities allowed herpes carriers to donate blood as long as they did not have an active infection. Thats because many believed that herpes could spread only during intercourse or via direct skin-to-skin contact.
However, recent studies suggest that herpes may spread via blood transfusions, regardless of whether the infected has an active infection. Since there isnt enough evidence to support this theory, many facilities still accept blood from herpes carriers.

Whether or not you take medicine to treat herpes, taking care of yourself by eating healthy foods, getting enough sleep, and avoiding stress might help keep future outbreaks from popping up.
No one knows for sure what triggers genital herpes outbreaks. Other infections, surgery, sex, your period, skin irritations, and stress may cause outbreaks. Sunburns, injuries to your lips, or other infections can cause oral herpes flare-ups. Try to avoid getting sunburned if you have oral herpes.

Healthcare providers call this asymptomatic viral shedding.

Gonococcal Infection Among Infants And Children
Sexual abuse is the most frequent cause of gonococcal infection among infants andchildren . For preadolescent girls, vaginitis is the most common manifestation of this infection gonococcal-associated PID after vaginal infection can be less common amongpreadolescents than adults. Among sexually abused children, anorectal andpharyngeal infections with N. gonorrhoeae are frequentlyasymptomatic.
Culture can be used to test urogenital and extragenital sites for girls andboys. NAAT can be used to test for N. gonorrhoeae fromvaginal and urine specimens from girls and urine for boys . Although data regarding NAAT fromextragenital sites among children are more limited, andperformance is test dependent, no evidence supports that performance of NAATfor detection of N. gonorrhoeae among children differs fromthat among adults . Because of the implications of aN. gonorrhoeae diagnosis in a child, only validatedFDA-cleared NAAT assays should be used with extragenital specimens.Consultation with an expert is necessary before using NAAT to minimize thepossibility of cross-reaction with nongonococcal Neisseriaspecies and other commensals and to ensure correct interpretation of results.
Gram stains are inadequate for evaluating prepubertal children for gonorrheaand should not be used to diagnose or exclude gonorrhea. If evidence of DGIexists, gonorrhea culture and antimicrobial susceptibility testing should beobtained from relevant clinical sites .

Antiviral Creams And Tablets
Antiviral creams such as aciclovir or penciclovir may speed up the healing time of a recurrent cold sore infection if used correctly.
Cold sore creams are widely available over the counter from pharmacies without a prescription.
Theyre only effective if you apply them as soon as the first signs of a cold sore appear, when the herpes simplex virus is spreading and replicating. Using an antiviral cream after this initial period is unlikely to have much effect.
If you have frequently recurring bouts of cold sores, use antiviral medication as soon as you feel the tingling sensation that indicates a cold sore is about to develop. Youll need to apply the cream up to five times a day for four to five days.
Antiviral creams can only help to heal a current outbreak of cold sores. They dont get rid of the herpes simplex virus or prevent future outbreaks of cold sores occurring.
Antiviral tablets are generally more effective than creams at treating cold sores, but are usually only prescribed for more severe cases.
Treatment Options For Genital Herpes
Acyclovir was the first antiviral agent to be used in the treatment of genital herpes, and as such, is the most extensively studied of the antiviral agents currently available. This drug is available in topical, oral, and intravenous forms . In the distant past, topical acyclovir was used to treat recurrent genital herpes, but its ineffectiveness has been known for quite some time. A randomized, placebo-controlled, double-blind study found that after application of topical acyclovir 6 times daily for 5 days, only the duration of virus shedding from lesions was reduced compared to placebo. Differences between time to lesion crusting, lesion healing, new lesion formation, and cessation of pain were non-significant .
Intravenous acyclovir remains the most effective form of treatment for a primary genital herpes infection, leading to significant reduction in time to cessation of viral shedding and pain. Time to lesion healing is also 6 days faster than with placebo alone . However, as IV administration of acyclovir requires hospitalization, this route is reserved for patients with life-threatening disease or systemic complications .

Sexual Assault Or Abuse Of Children
These guidelines are limited to the identification and treatment of STIs inprepubertal children. Management of the psychosocial or legal aspects of thesexual assault or abuse of children is beyond the scope of these guidelines.
Identification of STIs in children past the neonatal period strongly indicatessexual abuse .The importance of identifying a sexually transmitted organism for such childrenas evidence of possible child sexual abuse varies by pathogen. Postnatallyacquired gonorrhea, syphilis, chlamydia, and T. vaginalisinfection and nontransfusion, nonperinatally acquired HIV infection areindicative of sexual abuse. Sexual abuse should be suspected when anogenitalherpes or anogenital warts are diagnosed. Investigation of sexual abuse amongchildren who have an infection that might have been transmitted sexually shouldbe conducted in compliance with recommendations by clinicians who haveexperience and training in all elements of the evaluation of child abuse, neglect, and assault. The social significance of an infection that might havebeen acquired sexually varies by the specific organism, as does the thresholdfor reporting suspected child sexual abuse . When any STI has been diagnosed in a child, efforts shouldbe made in consultation with a specialist to evaluate the possibility of sexualabuse, including conducting a history and physical examination for evidence ofabuse and diagnostic testing for other commonly occurring STIs .

How Is It Treated
Although there is no cure, medicine can relieve pain and itching and help sores heal faster. If you have a lot of outbreaks, you may take medicine every day to limit the number of outbreaks.
After the first outbreak, some people have just a few more outbreaks over their lifetime, while others may have 4 to 6 outbreaks a year. Usually the number of outbreaks decreases after a few years.
Treatment works best if it is started as soon as possible after the start of an outbreak. This is especially true for outbreaks that come back again and again.
Finding out that you have herpes may cause you to feel bad about yourself or about sex. Counseling or a support group may help you feel better.
