Treatment For Genital Herpes
There are three antiviral medications that are FDA-approved for the treatment of genital herpes:
- Acyclovir: The oldest antiviral medication for herpes is acyclovir. It has been available since 1982 in a topical form and sold since 1985 in pill form. Acyclovir has been shown to be safe in persons who have used it continuously for as long as 10 years.
- Valacyclovir: A newer drug, valacyclovir, actually uses acyclovir as its active ingredient. This medication delivers acyclovir more efficiently so that the body absorbs much of the drug, which has the advantage of taking the medication fewer times during the day.
- Famciclovir: Famciclovir uses penciclovir as its active ingredient to stop HSV from replicating. Like valacyclovir, it is well absorbed, persists for a longer time in the body, and can be taken less frequently than acyclovir.
Antiviral medication is commonly prescribed for patients having a first episode of genital herpes, but they can be used for recurrent episodes as well. There are two kinds of treatment regimens: episodic therapy and suppressive therapy.

How To Treat A Cold Sore
Last Updated July 2021 | This article was created by familydoctor.org editorial staff and reviewed by Robert Chuck Rich, Jr., MD, FAAFP
Cold sores are small, blister-like, fluid-filled lesions that usually appear around the border of the lips. They also may appear on other areas of your face, such as your chin or nose. Cold sores are caused by a virus called herpes simplex . They are very common and also very contagious.
Even though they are called cold sores, they have nothing to do with having a cold. You catch a cold sore by coming into close contact with someone who has a cold sore, such as by kissing. Also, not everyone who comes into contact with the virus will get a cold sore. For these people, the virus just never becomes active. But the majority of people who are exposed to a cold sore will have some sort of reaction. It may be that they get one cold sore and never have another. Or they may have cold sore outbreaks several times a year.
Because cold sores are caused by a virus, you cannot cure them. Thankfully, there are treatments and medicines that can help manage outbreaks when you get them.
How Does Genital Herpes Affect Pregnancy
Herpes simplex virus doesnt affect fertility or your ability to conceive. Pregnant women diagnosed with HSV-2 should start a daily antiviral at 36 weeks of pregnancy as prescribed, to prevent outbreaks during delivery. If you have an active infection at the time of childbirth, you can pass the herpes virus to your baby. Neonatal herpes puts a baby at risk for blindness, brain damage, skin infections and death. Your healthcare provider will perform a cesarean section to lower this risk.

What Are The Complications Of Hsvxiii
- Skin infections This happens if the virus comes into contact with broken skin such as cuts or eczema.
- Neonatal herpes If an expectant mother has genital herpes, the infection could pass to her baby and cause brain damage, blindness, or even death.
- Increased risk of contracting other STIs Genital herpes increases your risk of contracting other STIs, as the associated blisters can cause breaks in the skin, allowing certain microbes to enter the body more easily.
- Blindness If the infection is transferred to the eyes and spreads to the cornea, it can be sight-threatening.
How Is Herpes Simplex Diagnosed
Healthcare providers may diagnose herpes simplex based on how the sores look. Your provider may take a sample from the sore. Laboratory analysis of the sample can confirm or rule out the herpes virus.
If you dont have sores, your healthcare provider can use a blood test to check for HSV-1 and HSV-2 antibody, a marker showing youve been exposed to the virus. The blood test doesnt show an active infection . But it informs your provider whether youve been exposed to the herpes virus in the past. If this is your first infection, you may not test positive for herpes if there hasnt been enough time for your body to develop antibodies. The HSV-1and HSV-2 antibody test may be repeated in eight to 12 weeks.

What Are The Symptoms Of Herpes Simplex
Many people with the infection never experience any herpes symptoms. If you do notice symptoms, youll experience them differently depending on whether youre having your first herpes outbreak or a repeat outbreak. Recurring symptoms are usually milder than the first outbreak. Symptoms dont last as long with later outbreaks. Some people may only have one or two outbreaks during their lifetime. Others may have as many as four or five outbreaks a year.
People who do have herpes symptoms may experience:
- Cold sores around their lips, mouth or tongue. They may look crusty or like fluid-filled blisters.
- Tingling, itching or burning.
- Pain while urinating.
How Does Herpes Simplex Spread
Herpes spreads through close contact with a person who has the infection. The virus can be found in skin and saliva. If you have herpes simplex, you are most likely to pass the virus to another person when you have sores. But you can infect someone else even if you have no symptoms. Healthcare providers call this asymptomatic viral shedding.

Other Cold Sore Treatments
Beyond patches, creams, and gels, other over-the-counter medications and home remedies may help reduce cold sore symptoms. These include:
- Over-the-counter pain medications: Drugs such as acetaminophen or ibuprofen may help reduce the pain and swelling of a cold sore.
- Cold compresses: These may also help with pain or discoloration. A person can make a compress by covering an ice pack in a cloth and holding it against the affected area.

Cold Sores Kissing And Sex
If you get cold sores, that doesnt mean you should totally avoid intimacy, whether thats dating, kissing or sex. But the American Sexual Health Association advises that you should wait until a cold sore heals and the area looks normal again before performing oral sex or kissing someone directly on the mouth .
Because most adults have oral herpes, we do not advise that a person stop giving or receiving affection altogether between outbreaks simply because they have oral herpes, the ASHA says. However, using a barrier or condom when performing oral sex can reduce the risk of contracting genital herpes.
Treatment Options For Genital Herpes
Acyclovir was the first antiviral agent to be used in the treatment of genital herpes, and as such, is the most extensively studied of the antiviral agents currently available. This drug is available in topical, oral, and intravenous forms . In the distant past, topical acyclovir was used to treat recurrent genital herpes, but its ineffectiveness has been known for quite some time. A randomized, placebo-controlled, double-blind study found that after application of topical acyclovir 6 times daily for 5 days, only the duration of virus shedding from lesions was reduced compared to placebo. Differences between time to lesion crusting, lesion healing, new lesion formation, and cessation of pain were non-significant .
Intravenous acyclovir remains the most effective form of treatment for a primary genital herpes infection, leading to significant reduction in time to cessation of viral shedding and pain. Time to lesion healing is also 6 days faster than with placebo alone . However, as IV administration of acyclovir requires hospitalization, this route is reserved for patients with life-threatening disease or systemic complications .

What Is A Cold Sore
Cold sores, also known as fever blisters, are caused by the HSV-1 virus. HSV-2 is the form of the virus responsible for genital herpes, although you can contract either form of the virus on your mouth or genitals.
Cold sores generally take the form of blisters on or near the lips . They tend to appear in three stages:
As conspicuous as a cold sore might make you feel, youre far from alone: According to the World Health Organization, 67% of people worldwide are infected with HSV-1. Most people contract the virus as children, often from relatives a quick peck on the face can be enough to transmit it. But the majority of people who have HSV-1 dont show symptoms. Only about 30% of people with HSV-1 will get cold sores, usually once or twice a year.
There is no cure for HSV-1. After infection, it remains in the body forever. Usually, the first outbreak of cold sores is the worst in terms of symptoms. But after a cold sore outbreak fades, the virus remains in the bodys nerve endings, waiting to be triggered and resurface.
