Herpes Simplex Virus Infection
Epidemiology
Anogenital herpes is caused by herpes simplex virus type 1 or type 2 . Infections caused by this virus are common across the world but there are variations in prevalence between countries and population groups.
HSV disease is not a notifiable disease in Australia so there are limited data regarding disease prevalence. A nationwide population seroprevalence survey of HSV infection from 1999-2000 found an overall HSV2 seroprevalence of 12% . HSV2 seroprevalence was two-fold higher in females compared with males , 18% amongst indigenous Australians, and higher in metropolitan compared with remote areas . The overall seroprevalence of HSV1 infection was 76% .
Historically, HSV1 was the most common cause of orolabial herpes and HSV2 the most common cause of anogenital herpes. However, recent data from a large sexual health clinic in Melbourne suggests that HSV1 is now the most common cause of anogenital herpes among women, people younger than 28 years and MSM .
Clinical manifestations
Genital HSV disease typically presents in the anogenital area with blistering, ulceration and lymphangitis . It is a common cause of proctitis in MSM, although only one-third of men will have externally visible ulcers . Herpes proctitis is more common among HIV-positive, compared with HIV-negative, MSM .
Management
Orolabial HSV disease
- Valaciclovir 500mg – 1 g PO BID
- Famciclovir 250 – 500 mg PO BID
- Acyclovir 400 mg PO TID
Initial or recurrent genital lesions
Hsv Infection And Symptoms
Most individuals infected with HSV have either no symptoms or mild symptoms that go unnoticed. When symptoms do appear, they initially present with tingling and/or redness, followed by blister-like lesions that rapidly merge into open, weeping sores. The sores are often quite painful and can be accompanied by a fever and swollen lymph glands.
Both oral and genital herpes cycle between periods of active disease, which can last from two days to three weeks, followed by a period of remission. After the initial infection, the viruses attach themselves to sensory nerve cells, where they remain for a lifetime. HSV can reactivate at any time , although the frequency and severity of outbreaks tend to subside over time.

Gum Disease And Dry Mouth
Although these arent sores, gum disease and dry mouth are common problems.
Gum disease causes the gums to swell, and can be painful. In severe cases, it can lead to gum or teeth loss in as quick as 18 months. Gum disease may also be an indication of inflammation, which increases the risk for heart disease and stroke.
Dry mouth occurs when a person doesnt produce enough saliva. Saliva can help protect the teeth as well as prevent infections. Without saliva, the teeth and gums are vulnerable to plaque development. This can also make gum disease worse.
Treatment: Drink water, floss, and brush consistently to keep the mouth clean and hydrated. For gum disease, a dentist will remove the plaque with a deep cleaning method.
If dry mouth persists, ask a healthcare provider about saliva substitutes.

Treating The Symptoms Of Hpv
Most cases of HPV dont require any treatment. The virus will go away on its own in many people. However, there are treatment options available for treating the symptoms of HPV.
Genital warts from HPV may occasionally go away without medication. Sometimes, medications are used to help lessen the effects of the warts. These include:
Your doctor may also apply trichloroacetic acid or bicloroacetic acid, or cryotherapy to help treat genital warts.
Sometimes a doctor will remove the warts, though this removes the wart not the virus itself. If a high-risk HPV is found, your doctor may monitor you to ensure that cancer doesnt occur, or is caught early.

Im Pregnant How Could Genital Herpes Affect My Baby
If you are pregnant and have genital herpes, it is very important for you to go to prenatal care visits. Tell your doctor if you have ever had symptoms of, or have been diagnosed with, genital herpes. Also tell your doctor if you have ever been exposed to genital herpes. There is some research that suggests that genital herpes infection may lead to miscarriage, or could make it more likely for you to deliver your baby too early.
Herpes infection can be passed from you to your unborn child before birth but is more commonly passed to your infant during delivery. This can lead to a potentially deadly infection in your baby . It is important that you avoid getting herpes during pregnancy. If you are pregnant and have genital herpes, you may be offered anti-herpes medicine towards the end of your pregnancy. This medicine may reduce your risk of having signs or symptoms of genital herpes at the time of delivery. At the time of delivery, your doctor should carefully examine you for herpes sores. If you have herpes symptoms at delivery, a C-section is usually performed.

Is There A Cure Or Treatment For Herpes
There is no cure for herpes. Antiviral medications can, however, prevent or shorten outbreaks during the period of time the person takes the medication.11 In addition, daily suppressive therapy for herpes can reduce the likelihood of transmission to partners.11
There is currently no commercially available vaccine that is protective against genital herpes infection. Candidate vaccines are in clinical trials.

Complications Of Oral Herpes
Theres no current cure for oral herpes . The virus remains in your system once its transmitted.
Blisters can disappear and reappear throughout your life. When you dont have visible symptoms, it means the virus is inactive, but you can still transmit it to others. Many people dont develop visible symptoms.
For the most part, oral herpes is a mild infection. Sores usually clear up on their own without treatment.
In rare cases, complications can occur. This is more likely to happen in people who have a weakened immune system, perhaps due to age or a chronic illness.
Possible complications can include dehydration if drinking becomes painful due to oral blisters. If left untreated, dehydration can lead to serious problems. This certainly isnt likely to occur. Just make sure youre drinking enough, even if its uncomfortable.
Another incredibly rare complication of oral herpes is encephalitis. This occurs when the viral infection travels to the brain and causes inflammation. Encephalitis isnt usually life-threatening. It may only cause mild flu-like symptoms.
Minor complications of oral herpes include a skin infection if the virus comes in contact with broken skin. This can occur if you have a cut or eczema. It can sometimes be a medical emergency if cold sores cover widespread areas of skin.
Children with oral herpes may develop herpes whitlow. If a child sucks their thumb, blisters can form around the finger.

- HIV infection is often mistaken for the flu, mononucleosis, or another viral infection, even syphilis or hepatitis. That’s not surprising. Many of the symptoms are the same, including pain in the joints and muscles and swollen lymph glands.
- Lymph nodes are part of your body’s immune system and tend to get inflamed when you have an infection. Many lymph nodes are located in your armpit, groin, and neck.

In the proportional hazards model , 70 participants met the primary outcome based on having 2 consecutive CD4 count measurements 350 cells/mm3, and 43 met the primary outcome based on actual HAART initiation the remaining 105 were right-censored. What Happens If Herpes Is Not Treated
Herpes can be painful, but it generally does not cause serious health problems like other STDs can.
Without treatment, you might continue to have regular outbreaks, or they could only happen rarely. Some people naturally stop getting outbreaks after a while. Herpes typically does not get worse over time.

Special Considerations During Pregnancy
Laboratory testing to diagnose mucocutaneous HSV infections is the same for pregnant women as for non-pregnant women. Episodic therapy for first-episode HSV disease and for recurrences can be offered during pregnancy. Visceral disease following HSV acquisition is more likely to occur during pregnancy and can be fatal. Acyclovir is the antiviral drug with the most reported experience in pregnancy and appears to be safe, particularly during the second and third trimesters .51 One recent caseâcontrol study suggested a higher risk of gastroschisis associated with both genital herpes and acyclovir use during the first trimester of pregnancy.52 The use of valacyclovir and famciclovir during pregnancy has been described, and the antiviral drugs also appear to be safe and well tolerated during the third trimester.53 Given its simplified dosing schedule valacyclovir is an option for treatment and suppressive therapy during pregnancy .

What Is The Connection To Hiv
People with HIV have been found to have higher rates of shingles than the general population. They are also at risk of experiencing complications as a result of shingles.
HIV targets the immune system by depleting it of its CD4 T lymphocytes . CD4 cells are lymphocytes, a type of white blood cell that helps the body fight off infections.
While shingles can be triggered in adults with HIV at any CD4 count, the risk of infection is higher in people with lowered CD4 counts, as in less that 200 cells per cubic millimeter of blood . Lowered CD4 counts are linked to untreated HIV and stage 3, or advanced, HIV. This is why shingles may indicate HIV since untreated HIV and stage 3 HIV are associated with a higher chance of developing shingles.
A person with a compromised immune system is more susceptible to getting shingles. This includes people with untreated HIV and stage 3 HIV. This may have to do with depleted levels of CD4 cells, meaning the body is more vulnerable to infection.

How Can I Reduce My Risk Of Getting Genital Herpes
If you are sexually active, you can do the following things to lower your chances of getting genital herpes:
- Be in a long-term mutually monogamous relationship with a partner who is not infected with an STD
- Using latex condoms the right way every time you have sex.
Be aware that not all herpes sores occur in areas that are covered by a latex condom. Also, herpes virus can be released from areas of the skin that do not have a visible herpes sore. For these reasons, condoms may not fully protect you from getting herpes.
If you are in a relationship with a person known to have genital herpes, you can lower your risk of getting genital herpes if:
- Your partner takes an anti-herpes medication every day. This is something your partner should discuss with his or her doctor.

How Do I Know If I Have Genital Herpes
Most people with genital herpes have no symptoms or have very mild symptoms. Mild symptoms may go unnoticed or be mistaken for other skin conditions like a pimple or ingrown hair. Because of this, most people do not know they have a herpes infection.
Herpes sores usually appear as one or more blisters on or around the genitals, rectum or mouth. This is known as having an outbreak. The blisters break and leave painful sores that may take a week or more to heal. Flu-like symptoms also may occur during the first outbreak.
People who experience an initial outbreak of herpes can have repeated outbreaks, especially if they have HSV-2. However, repeat outbreaks are usually shorter and less severe than the first outbreak. Although genital herpes is a lifelong infection, the number of outbreaks may decrease over time.
Ask a healthcare provider to examine you if:
