What Are Cold Sores

Cold sores are small blisters around the mouth, caused by the herpes simplex virus. They are sometimes called fever blisters. The most common strain of the virus causing cold sores is herpes simplex virus 1. It can be spread by kissing or sharing eating utensils or even sharing towels.

Herpes simplex is not curable, but may lie dormant for a long time. Episodes of the cold sores last no longer than 2 weeks. Hot sun, cold wind, a cold or other illness, or a weak immune system can cause an outbreak of herpes simplex virus.

Sleeping Herpesviruses Survive In The Body

The balance between the latent and active proliferation phases of the virus is one of the research focuses of Prof Luka Cicin-Sain, who is the head of the Immune Aging and Chronic Infections research group at the HZI. He aims to understand how the virus maintains its presence during the chronic phase in particular, how the

memory cells are busy with CMV? In general, proving a negative is much more difficult than proving that something occurs, cautions Luka Cicin- Sain. In one study, he was able to show that the immune systems of CMV-positive subjects appear to work similarly to those of CMV-negative subjects in terms of recognising and controlling influenza and other pathogens. The field seems to agree with this conclusion. However, other studies have shown that atherosclerosis and related diseases, such as heart attack and stroke, are more common in CMV-positive individuals. Whether CMV is a cause of atherosclerosis or simply a bystander in these patients is an open question. Scientists are still seeking experimental proof. Research collaborations, such as the RESIST Excellence Cluster in Hannover, in which Cicin-Sain works together with clinicians and epidemiologists, are on-going.

How these viruses evade the immune system remains poorly understood, but researchers at Washington University School of Medicine in St. Louis discovered that a mouse herpes virus uses molecules that mimic a cell’s own proteins to help thwart an immune attack.

St. Louis, August 20, 2002 — Herpes viruses enter the body and hide away in cells, often re-emerging later to cause illnesses such as shingles, genital herpes and cancer. How these viruses evade the immune system remains poorly understood, but researchers at Washington University School of Medicine in St. Louis discovered that a mouse herpes virus uses molecules that mimic a cell’s own proteins to help thwart an immune attack.

The findings also suggest that a branch of the immune system known as the complement system may play a more important role in controlling herpes virus infections than previously thought. The study is published in the August issue of the journal Immunity.

Virgin’s team found that one type of herpes virus makes its own version of RCA to trick the immune system and evade destruction by complement, but that the RCA mimic proteins help the virus only during acute infection.

Scientists Reveal How Herpes Virus Hijacks And Hides In Our Cells

New research reveals how the common virus that causes cold sores herpes simplex 1 invades our cells and takes control of their machinery to replicate itself and hide from the immune system.

The discovery is significant because it offers new information about how the virus affects processes in the cell rather than the immune system.

Researchers from the University of Cambridge in the UK, and two German institutions: the Julius-Maximilians- Universität Würzburg and the Ludwig-Maximilians-Universität München, report their findings in the journal Nature Communications.

They describe how HSV-1 inserts its DNA into our cell DNA, takes over and disrupts the production of cell proteins while using it to make perfect versions of its own proteins.

Herpes simplex virus 1 is very common and usually harmless. Most people become infected by their early 20s, and after the first infection, the virus remains dormant in the facial nerve tissues.

Now and again, the virus flares up and causes mild cold sore symptoms. But there are also cases where HSV-1 can lead to life-threatening disease. For instance, in intensive care units, it can cause severe lung problems.

And even in healthy people, there are rare instances where HSV-1 can spontaneously cause inflammation of the brain, resulting in irreversible brain damage.

You Get Sick Frequently And Take Longer Than Usual To Recover

Don’t be alarmed if you get the sneezes and sniffles through two or three colds a year. Most people bounce back to normal in about a week.

But if you’re constantly catching colds with symptoms that linger for weeks, or even get food poisoning often, it may be due to a sluggish response from your innate immune system.

Your innate immune system involves barriers that keep harmful materials from entering your body. Think of it as the first line of defense against all invaders and injury. Its components include:

  • Cough reflex, which helps us expel things that may irritate or infect us.
  • Mucus production, which traps bacteria and small particles and helps expel them from the body.
  • Stomach acid, which helps kill microbes that enter through our food and water.

How Are Cold Sores Treated

Cold sores cant be cured, but, if symptoms are severe, treatment may help relieve some symptoms. Treatment may include:

  • Antiviral topical ointments

  • Antiviral oral medications

  • Over-the-counter topical anesthetics or anti-inflammatory agents may help with symptoms

Cold sores take about 1 to 3 weeks to heal. The first time they appear, they can take up to 3 weeks to heal. But, when cold sores return, they usually take a week to heal if no medicines are used. Antiviral medications may help, but they need to be started before day 3 or 4 of the blister formation. Antivirals are usually not recommended for otherwise healthy people. Always consult your health care provider or dentist if the sore does not heal or becomes worse as time passes.

Treatment The First Time You Have Genital Herpes

You may be prescribed:

  • antiviral medicine to stop the symptoms getting worse you need to start taking this within 5 days of the symptoms appearing
  • cream for the pain

If you have had symptoms for more than 5 days before you go to a sexual health clinic, you can still get tested to find out the cause.

Complications Of Cold Sores

Cold sores are usually mild, but may cause complications in rare cases. People with weak immune systems caused by illness or treatments such as chemotherapy are particularly at risk of complications.

Dehydration sometimes occurs if drinking fluids becomes painful. Young children are particularly at risk of becoming dehydrated.

The herpes simplex virus can also spread to other parts of your body. Examples of when this can occur include:

  • skin infections these often occur if the virus comes into contact with broken skin, such as a cut or graze, or a skin condition such as eczema
  • herpetic whitlow this causes painful sores and blisters to appear on and around your fingers
  • herpetic keratoconjunctivitis this causes swelling and irritation of your eye area and sores to develop on your eyelids

Left untreated, herpetic keratoconjunctivitis can cause the cornea, the transparent layer at the front of your eye, to become infected, which can eventually lead to blindness.

It’s therefore important not to touch your eyes if you have an unhealed cold sore. If you must touch your eyes for example, to remove contact lenses wash your hands thoroughly first.

In very rare cases, encephalitis, a condition where the brain becomes inflamed and swollen, can be caused by the cold sore virus spreading to the brain. It can be treated with intravenous injections of antiviral medications, such as aciclovir.

You Get Cold Sores Often Or Had Shingles At A Young Age

The viruses that cause cold sores and shingles are both in the herpes virus family. Once you have contracted a herpes virus, it goes into a dormant state in the body.

However, when you are under stress or your cellular immunity weakens, the virus can replicate and reactivate again.

Seeing frequent reactivations can be a sign that your immune system needs a boosting.

Dna Sensors As Activators Of Host Antiviral Response

According to Zhang and colleagues DDX41 , a member of the DEAD-like helicases superfamily senses DNA in myeloid dendritic cells, which stimulates type I interferon production. It has been reported that the knockdown of the DDX41 gene inhibited mitrogen-activated protein kinase TBK1 and NF-B transcription factor by DNA, and it depends on STING to sense pathogenic DNA. HSV-1 capsid ubiquitination and Vp5 degradation occur in a proteasome-dependent manner . This liberates the DNA into the cytosol where they get sensed by cytosolic DNA sensors, subsequently inducing the innate immune response. A PYRIN protein interferon gamma inducible protein 16 , which functions as a cytosolic DNA sensor that directly binds DNA, has been reported. The study found that the knockdown of the IFI16 gene in mouse models inhibited the activation of transcription factors IRF3 and NF-B on infection by HSV-1 . This showed that IFI16 is essential for the activation of IRF3 and NF-B. IFI16 interacts directly with interferon-, inducing DNA to recruit IRF3 and NF-B for the transcription of genes involved in inflammatory response. In a recent publication, HSV-1 was shown to also activate the inflammatory response via the nucleotide-binding domain and leucine-rich repeat-containing receptor 3 .

protein known as STING. Normally, when a cell is infected, STING is activated, triggering several chain reactions or signalling pathways, and resulting in multiple immune responses. One of these signalling pathways is particularly effective at raising the emergency alert level in both the infected cell as well as its neighbours. The CMV protein m152 precisely targets this emergency alert pathway. STING also activates a second signalling cascade, but CMV needs this second pathway for its own replication and leaves it unaffected. Through m152, CMV outsmarts part of the immune response while at the same time utilising a separate part of the immune response for its own purposesleading to the aforementioned own goal of the

infection, the virus goes into a type of dormant state from which it will only occasionally wake up to reproduce itself, a stage known as reactivation. This can be observed in the case of cold sores caused by HSV-1: The cold sores are not permanently present, but rather only appear occasionally.

T Cells No Less Diverse In Old Age

This revelation also took the scientists by surprise for a long time, the assumption had been that T cell populations become less diverse with age, and this had been considered a main factor in the decreased effectiveness of immune responses.

Now, Dr. Nikolich-ugich and team saw that the T cells in aging mice were no less diverse than in younger animals.

The problem was that the T cells were not typically recruited to defend the systems. However, older CMV-infected mice did not seem to encounter this issue, and their T cells were more active.

Its as if CMV is issuing a signal that gets the best defenses out onto the field, Dr. Nikolich-ugich observes.

This shows that the ability to generate a good immune response exists in old age and CMV, or the bodys response to CMV, can help harness that ability, Smithey hypothesizes.

The scientists findings are now reported in PNAS. And already, they are planning to deepen their understanding of CMVs effect on the immune system in further studies.

In the future, the team hopes to be able to replicate its recent results in another study with human participants.

Should this happen, Dr. Nikolich-ugich and colleagues would aim to design a vaccine that could more effectively boost the immune systems of older adults.

THE herpes viruses, among the most ubiquitous, persistent and resistant of infectious organisms, are fast gaining notoriety as a cause of human ills. Indeed, every species of animal seems to have evolved with its own contingent of disease-causing herpes viruses, and few herpes infections in any species have thus far proved curable or preventable.

In fact, says Dr. Clyde Crumpacker of Harvard Medical School and Beth Israel Hospital in Boston, one of a legion of researchers currently battling these stubborn organisms, the five herpes viruses that infect human beings are now believed to cause more illness than any other group of viruses. Two members of the herpes family are leading suspects in research efforts to prove that viruses cause human cancers, including cancer of the cervix.

Though herpes is currently best known by name as the cause of a worldwide epidemic of venereal disease, this virus family plagues people of all ages and circumstances: The fetus may be devastated by cytomegalovirus, the child bespotted by chicken pox, the adolescent felled by infectious mononucleosis, the adult pestered by recurrent cold sores, the aged agonized by shingles.

”Until effective drugs are developed,” he wrote in Patient Care magazine, ”avoiding person-to-person contact is the only method we have for preventing the spread of herpes.”

Other Triggers For Genital Herpes Outbreaks

Other triggers of genital herpes may include:

  • Sexual intercourse. Some people find that the friction of sexual intercourse irritates the skin and brings on symptoms. Using a water-based lubricant can help reduce irritation. Don’t use one that contains the spermicide nonoxynol-9, however. Nonoxynol-9 can irritate mucous membranes, such as the lining of the . Oil-based lubricants are a no-no, too. They weaken latex, making condoms more likely to break. Even if the friction of intercourse seems to be a trigger for symptoms, it probably won’t cause a flare-up every time you have sex.
  • Colds and sunlight. The common cold and sunlight seem to trigger outbreaks of oral herpes , but no proof exists that they trigger genital herpes outbreaks.
  • Hormones. Hormonal changes, like those that occur in the menstrual cycle, can affect genital herpes outbreaks. No one knows why.
  • Surgery, weak immune system. Trauma to the body, such as having surgery, may make herpes symptoms appear. It’s possible that having a weakened immune system does, too. People whose immune systems are weakened by HIV, for example, tend to have outbreaks more often than people with normal immune functioning.

Remember that triggers may not be the same for everyone, and doctors are not certain how much lifestyle affects outbreaks of herpes symptoms. If you think something in particular triggers symptoms, ask your doctor what you should do about it.
