Herpes Simplex Virus Infection

Epidemiology

Anogenital herpes is caused by herpes simplex virus type 1 or type 2 . Infections caused by this virus are common across the world but there are variations in prevalence between countries and population groups.

HSV disease is not a notifiable disease in Australia so there are limited data regarding disease prevalence. A nationwide population seroprevalence survey of HSV infection from 1999-2000 found an overall HSV2 seroprevalence of 12% . HSV2 seroprevalence was two-fold higher in females compared with males , 18% amongst indigenous Australians, and higher in metropolitan compared with remote areas . The overall seroprevalence of HSV1 infection was 76% .

Historically, HSV1 was the most common cause of orolabial herpes and HSV2 the most common cause of anogenital herpes. However, recent data from a large sexual health clinic in Melbourne suggests that HSV1 is now the most common cause of anogenital herpes among women, people younger than 28 years and MSM .

Clinical manifestations

Genital HSV disease typically presents in the anogenital area with blistering, ulceration and lymphangitis . It is a common cause of proctitis in MSM, although only one-third of men will have externally visible ulcers . Herpes proctitis is more common among HIV-positive, compared with HIV-negative, MSM .

This randomized, double-blind, placebo-controlled proof-of-concept trial will provide evidence for the efficacy of HSV-2 suppression with daily acyclovir on HIV transmission among HIV-discordant couples among whom the HIV-positive partner is also HSV-2 seropositive with CD4 > 250. The researchers hypothesis is that, by decreasing the frequency and amount of genital HIV shedding, standard doses of daily acyclovir 400 mg bid will reduce the rate of HIV transmission by 50% in HIV-discordant couples among whom the HIV-infected partner is HSV-2 positive.

How Is Genital Herpes Treated

There is no cure for the herpes simplex virus. The blisters usually heal and go by themselves, so you may not always need treatment. There is antiviral medicines for herpes, which can: shorten outbreaks, relieve discomfort and stop symptoms from getting worse.

The antiviral treatment is most effective when you take it within the first five days of symptoms appearing. Avoid touching the blisters as this can also increase the risk of spreading the infection. If your herpes treatment requires you to apply cream to a sore, just gently pat the cream on, being careful not to rub around the surrounding area.

You can ease your symptoms by:

• keeping the affected area clean using plain or salt water to prevent blisters or ulcers from becoming infected
• applying petroleum jelly, such as Vaseline, to any blisters or ulcers to reduce the pain when passing urine
• asking a healthcare provider to recommend painkilling creams
• avoiding tight clothing because it may irritate the blisters and ulcers.

Dont have sex until you or your partner have finished your treatment, and the blisters or ulcers have gone. If its your first outbreak, its sometimes advised that you check back with a healthcare provider before having sex again, to ensure that your symptoms have gone.

Special Considerations During Pregnancy

Laboratory testing to diagnose mucocutaneous HSV infections is the same for pregnant women as for non-pregnant women. Episodic therapy for first-episode HSV disease and for recurrences can be offered during pregnancy. Visceral disease following HSV acquisition is more likely to occur during pregnancy and can be fatal. Acyclovir is the antiviral drug with the most reported experience in pregnancy and appears to be safe, particularly during the second and third trimesters .51 One recent caseâcontrol study suggested a higher risk of gastroschisis associated with both genital herpes and acyclovir use during the first trimester of pregnancy.52 The use of valacyclovir and famciclovir during pregnancy has been described, and the antiviral drugs also appear to be safe and well tolerated during the third trimester.53 Given its simplified dosing schedule valacyclovir is an option for treatment and suppressive therapy during pregnancy .

One Option For Ongoing Therapy: Taking Drugs Regularly

Some people with genital herpes choose to take antiviral medication daily to reduce the number of symptom outbreaks they have. In some cases, daily therapy prevents outbreaks completely.
