How Can You Test For Genital Herpes

Genital Herpes And Pregnancy

A Vaccine Pathway for Herpes Virus with Gregory Smith, PhD

Women with herpes before pregnancy can usually expect to have a healthy baby and a vaginal delivery.

If you have genital herpes during pregnancy, there’s a risk your baby could develop a serious illness called neonatal herpes.

This can be fatal, but most babies recover with antiviral treatment.

The risk of your baby getting neonatal herpes is low if you have had genital herpes before.

It’s higher if you get genital herpes for the first time in pregnancy.

How Much Does The Test Cost

The cost of testing for genital and oral herpes depends on the type of test that is performed, where the test is conducted, and whether the patient has medical insurance. Testing is often covered by insurance when ordered by a doctor, although patients may still be responsible for costs such as copays and deductibles.

What Can I Do About Genital Herpes

So heres the good news: herpes is rarely a serious health issue. Although there can be a negative stigma associated with those who have an outbreak and the blistering sores may change sexual habits, a majority of those infected have no symptoms at all.

If you do request a herpes blood test, ask if your doctor is going to be performing a type-specific test. These blood tests tend to be more reliable.

Should the blood test come back as positive, how your treatment plan develops will depend on other health conditions that may be affecting you. Suppressive therapies and other medication may be ordered, but in some patients, careful watching and waiting may be the best solution. If suppressive therapies are ordered, this will mean taking a drug every day that youll have to pay for.

Genital herpes can be detected with a herpes blood test. If no symptoms are being experienced, your doctor may wish to hold off on testing to avoid a false positive. And, if youre concerned about your sexual health for any reason, schedule an appointment with your doctor right away.

Type Of Tests Used To Diagnose Herpes

There are four main types of diagnostic tests that can be used to diagnose herpes. Your doctor will determine which type of test to use based on whether an outbreak is present or not.

If youre experiencing what you believe to be a herpes outbreak, your doctor can use a viral culture test or virus antigen detection test. If youre not experiencing symptoms, you can have an antibody test.

  • Viral culture test. This test is used to determine if a sore contains the herpes virus. This test can sometimes produce a false-negative, meaning that it may not detect the virus even though its present.
  • Virus antigen detection test. This test is used to determine if antigens to the herpes virus are present in a sore or lesion.
  • Antibody test. If youre not experiencing an outbreak yet but still believe you may have been exposed, you can opt to have an antibody test performed. This test will only show a positive result if the antibodies to the virus have been developed. Therefore, this test isnt necessarily recommended for recent exposure.
  • Polymerase chain reaction test. With this test, a healthcare provider can screen a sample of your blood or tissue from a sore. They can use this to determine if HSV is present and which type you have.

Is There Any Resistance To Genital Herpes Medication

Is this genital herpes

Resistance to drugs that treat genital herpes even after 20 years of use is very rare. Herpes medications may not work as well in patients who are very immunosuppressed and have been treated with these drugs for a long time. Each individuals response to treatment may vary. In some cases, patients may need more drugs to suppress their viral outbreaks than others.

Are Test Results Accurate

Tests that identify evidence of HSV infections are routine and commonly used medical tests. The ability of a test to accurately diagnose or rule out HSV infection depends on the test used for analysis and the stage of a patients infection.

The Tzanck smear and viral culture are less able to accurately identify patients who have an HSV infection than other tests. PCR testing and antibody testing are more sensitive, which means that they miss fewer cases of infection.

Blood tests that detect antibodies to HSV can distinguish between HSV-1 and HSV-2 but not the site of infection. This means that blood tests cannot tell whether a patient has a genital or oral herpes infection.

Although these tests are valuable for identifying HSV infection, no test is perfect. When learning of test results, patients should talk to their doctor about the type of test conducted, the reliability of test results, and if additional tests are necessary to confirm or rule out a diagnosis.

Challenges Of Testing For Genital Herpes

What could it be?

Genital herpes is a common, chronic sexually transmitted infection .1 It is caused by herpes simplex virus type 1 and type 2 . HSV is a lifelong infection characterized by periodic eruption of painful lesions in the genital area.2 Approximately 1 in 8 individuals between the ages of 13 and 49 years has a genital infection with HSV-2 in the United States.1,3 The proportion of genital infections caused by HSV-1 versus HSV-2 is increasing, especially among young persons.4 The Centers for Disease Control and Prevention estimates that 776,000 people in the United States become infected with genital herpes each year.1

Although severe sequelae are rare, genital herpes fosters a strong sense of fear, shame, and stigma among individuals. Because infections with HSV-1 and -2 have a wide spectrum of manifestation , infectious status is often underestimated. Studies consistently report 65% to 90% of those with genital HSV infection are unaware of its presence.5 Further, many individuals may assume they have been tested as part of routine STI testing, which is rarely the case.

Testing for herpes does present challenges for clinicians. There are direct testing options and indirect testing options . Each kind of test has its own set of indications and limitations. Clinicians can use appropriately ordered and interpreted tests to guide the counseling messages they provide.

The following three cases are used to explain the indications for and interpretation of the various test types.

Quick Answers For Clinicians

Virologic and serologic testing methods are available to confirm herpes simplex virus infection. Direct detection of the virus by polymerase chain reaction or viral culture is recommended for neonates and patients who have an active infection with lesions, to test for disseminated bloodborne infection in immunocompromised patients, and for cerebrospinal fluid testing. Serologic testing for immunoglobulin G can be used to determine if a person has ever been exposed to HSV but cannot determine exactly when the exposure occurred or the site of infection. Type-specific serologic testing for HSV-1- and HSV-2-specific glycoproteins can determine the subtype of the virus, which provides useful information about prognosis and informs treatment and counseling.

Genital herpes is most often caused by herpes simplex virus type 2 , but HSV-1-associated cases are increasing. Recurrence and subclinical viral shedding occur more frequently in cases caused by HSV-2 than in cases caused by HSV-1, which may impact treatment and counseling decisions. Therefore, subtyping HSV using polymerase chain reaction or serology testing can guide treatment decisions and patient counseling. Subtyping may also provide epidemiologic information about transmission of the virus.

In fact, you can learn a ton about the research on genital herpes and transmission where they discuss the reasoning for their decision.

As well as increasing awareness about HSV infection and its symptoms, improved access to antiviral medications and heightened HIV prevention efforts for those with genital HSV symptoms are needed globally.

In addition, development of better treatment and prevention interventions is needed, particularly HSV vaccines. WHO and partners are working to accelerate research to develop new strategies for prevention and control of genital and neonatal HSV-1 and HSV-2 infections. Such research includes the development of HSV vaccines and topical microbicides. Several candidate vaccines and microbicides are currently being studied.
