What Triggers Herpes Outbreaks

If youve had herpes for a while, you might have identified things that cause your herpes to crop up. Some of the most well-known triggers for herpes are things that stress your body, like lack of sleep or even a common cold. While your body is off dealing with some other stress, your immune system is essentially distracted, allowing your herpes to wake up. Thats why theyre called cold sores or fever blistersbecause they often appear when you have a cold or fever.

Other common triggers include:

Iii1 Efficacy And Effectiveness In Persons 60 Years

The efficacy of the live attenuated injectable Oka/Merck herpes zoster vaccine in healthy 60 year olds was shown in the Shingles Prevention Study and discussed previously in the first NACI statement on herpes zoster immunization. The SPS showed a 61% reduction in the burden of illness caused by HZ in persons aged greater than or equal to 60 years who were vaccinated with the herpes zoster vaccine.Footnote 1 It also showed a reduction in the incidence of HZ by 51% and a reduction in the incidence of post-herpetic neuralgia by 66%.

Tseng et al. performed two retrospective studies to look at herpes zoster vaccine effectiveness and safety postmarketing in immunocompetent adults 60 years of age or older. In one study, they examined cohorts from the Kaiser Permanente Southern California health plan over a three year time period and compared the number of HZ cases in vaccine recipients compared to matched unvaccinated controls. The vaccinated group had a rate of 6.4 cases of herpes zoster per 1000 person years . This is in contrast to the control group which had a rate of herpes zoster occurrence of 13 cases per 1000 person years . This comparison yielded a hazard ratio of 0.45 for vaccine associated risk reduction of HZ.Footnote 3

Duration Of Genital Herpes

The first episode of genital herpes usually begins with inflammation, followed by a small group of blisters that break and weep after a few days and eventually crust over and heal. The first outbreak is usually the worst and ends within about three weeks. The CDC recommends antiviral medication for everyone having a first outbreak of genital herpes.

After the initial outbreak, outbreaks tend to last for less time and have less severe symptoms. Research has indicated that the number of symptomatic outbreaks may decrease over time.

Vi2 Herpes Zoster Vaccine Efficacy And Safety In Immunocompromised Or Those On Immunosuppressive Drugs

Although herpes zoster vaccine is not approved for use in immunocompromised patients, off label administration of the vaccine occurs. Several studies have looked retrospectively at the safety of herpes zoster vaccine in groups of patients with underlying autoimmune diseases or receiving immunosuppressant medication. Zhang et al. examined a claims database from Aetna of 44,115 individuals 50 years of age with rheumatoid arthritis, ankylosing spondylitis, psoriasis, psoriatic arthritis or inflammatory bowel disease. Of this large population, only 514 patients received herpes zoster vaccine and had also had complete claims history up to 30 days after vaccination. Of these subjects a total of 47 subjects used anti-TNF agents at some time in the 30 days before and 30 days after vaccination. None of these patients developed HZ in the thirty days following vaccination.Footnote 23 In this study, the authors also reviewed low dose immunosuppression with HZ vaccine. Of the 514 persons who received HZ vaccine and had complete claims history, 34 were using methotrexate, 1 was receiving azathioprine, 5 were receiving 6-mercaptopurine, and 48 used oral glucocorticoids. None of these patients developed HZ within one month of vaccination.

Other data on the length of stay, consultations per case, and PHN per case were derived from the literatureFootnote 11 Footnote 24 Footnote 63 Footnote 111 and MedEchoFootnote 110 and RAMQFootnote 109 . Economic data on the costs were inflated to 2015 Canadian dollars and obtained from the literature. Utility weights for each health state were used in the estimation of QALYs. These data were collected from the Monitoring and Assessing Shingles Through Education and Research studyFootnote 112 and from Brisson et alFootnote 11 .

Is Oral Herpes Permanent

Yes. Once a person contracts the herpes virus, they will always have it.

However, this doesnt mean theyll have active recurrences throughout their lives. There are things you can do to reduce the risk of recurrence. For example:

Treatment For Genital Herpes

There are three antiviral medications that are FDA-approved for the treatment of genital herpes:

  • Acyclovir: The oldest antiviral medication for herpes is acyclovir. It has been available since 1982 in a topical form and sold since 1985 in pill form. Acyclovir has been shown to be safe in persons who have used it continuously for as long as 10 years.
  • Valacyclovir: A newer drug, valacyclovir, actually uses acyclovir as its active ingredient. This medication delivers acyclovir more efficiently so that the body absorbs much of the drug, which has the advantage of taking the medication fewer times during the day.
  • Famciclovir: Famciclovir uses penciclovir as its active ingredient to stop HSV from replicating. Like valacyclovir, it is well absorbed, persists for a longer time in the body, and can be taken less frequently than acyclovir.

Antiviral medication is commonly prescribed for patients having a first episode of genital herpes, but they can be used for recurrent episodes as well. There are two kinds of treatment regimens: episodic therapy and suppressive therapy.

What Happens If I Do Not Get Treatment

Genital herpes can lead to painful ulcers in the genital area, and in people with a weakened immune system, it can be severe.

If you touch sores or fluid from a wound, you can transfer herpes to another part of your body, such as your eyes. To prevent cold sores from spreading to another part of your body, do not touch sores or fluids. If you touch sores or fluids, wash your hands immediately to prevent the spread of infection.

Treatment For Sexually Transmitted Diseases

Patients who have a bacterial STD such as gonorrhea or chlamydia will receive a prescription for antibiotics. It is critical to follow the instructions so the antibiotics kill the infection. Patients should undergo a second round of testing three months after finishing the antibiotics to ensure the infection did not reappear. If it reoccurs, patients must undergo a second round of treatment.

Genital herpes is a viral STD and cannot be cured. However, antiviral drugs limit the number of outbreaks and also reduce the symptoms. These drugs also make infected patients less likely to pass the disease to sexual partners. Antiviral drugs do not completely eliminate the risk, though, so safe sex is necessary.

HIV is also a virus, but instead of antiviral drugs, the doctor will likely treat it with antiretroviral therapy. The patient will take a series of HIV medications. The medications reduce symptoms and slow the progression of the disease. Antiretroviral therapy also reduces the risk of transmitting the disease to others, although it is still possible.

Iv2 Efficacy And Effectiveness

IV.2.1 Efficacy and effectiveness of LZV in the general population

There were 14 studies that assessed single-dose live zoster vaccine efficacy and effectiveness in the general population. Of these, 3 were good quality studies and 11 were rated as fair quality .

Efficacy of LZV in preventing HZ among adults 50-59 years of age was reported in a large RCT involving over 22,000 individualsFootnote 61 . Within 1.5 years of immunization, VE was estimated to be 69.8% . Lower vaccine effectiveness within one year of immunization in this age group was reported following an analysis of observational dataFootnote 62.

Immunization with LZV was also found to be protective against PHN and recurrent HZ. In clinical trials, three year effectiveness against PHN was reported to be 65.7% in adults 60-69 years of age and 66.8% in adults 70 years of age and olderFootnote 37 Footnote 63 . These estimates were similar to those reported in observational studiesFootnote 68 Footnote 70 . In individuals with previous HZ, protection within three years of immunization was only observed among adults less than 70 years of age , but not in those 70 years of age and older .

IV.2.2 Efficacy and effectiveness of RZV in the general population

Treatment Of Primary Infection

Antiviral therapy is recommended for an initial genital herpes outbreak, especially if the patient has systemic symptoms or is immunocompromised.2 Oral acyclovir is effective in reducing symptoms1. Intravenous administration may be required in immunocompromised patients and those with severe disseminated infection.1 Topical acyclovir reduces the duration of viral shedding and the length of time before all lesions become crusted, but this treatment is much less effective than oral or intravenous acyclovir.1
