Whereas most research on herpes has focused on suppressing the recurrence of painful symptoms, the Fred Hutch gene therapy work addresses the root cause of reactivation: the presence of latent virus in infected nerve cells.
âI hope that this study changes the dialogue around herpes research and opens up the idea that we can start thinking about cure, rather than just control of the virus,â Jerome said.
It will still take a long time before these experiments lead to the first human trials of gene therapy to cure herpes. Jerome estimates that will be at least three years away.
Herpes simplex viruses afflict billions of human beings around the globe. According to the World Health Organization, two-thirds of the world population under the age of 50 carry herpes simplex virus type 1, or HSV-1, which primarily causes cold sores, while 491 million people aged 15-49 are infected with closely related HSV-2, which is the cause of sexually transmitted genital herpes.
Ganciclovir Cidofovir And Foscarnet
In the last decade, alternatives to ACV for herpesvirus treatment have emerged and become commercially available as therapeutic drugs. Ganciclovir is a nucleic acid analog that does not require viral proteins for its activation in the cell . These compounds differ from each other either, in their molecular processing into an active form of an acyclic guanosine analog, or bioavailability which significantly determines the frequency of administration .
Ganciclovir is indicated for the treatment of CMV, particularly for systemic and ocular infections in immunosuppressed patients . However, ganciclovir has also been reported to have antiviral activity against HSV-1 and HSV-2 and may be used for the treatment of herpetic keratitis . At present, there is an ongoing clinical study that is recruiting patients for assessing the effects of oral ganciclovir, together with femtosecond laser-assisted corneal debridement in the treatment of herpes simplex virus epithelial keratitis . Of note, ganciclovir has been reported to produce considerable adverse side effects in a high percentage of individuals , such as nephrotoxicity, neutropenia, myelosuppression, confusion, altered mental status, anxiety, ataxia, tremors, convulsions, fever, abnormal levels of liver enzymes in serum, diarrhea, and nausea, among others .

New Genital Herpes Drug Proves More Promising Than Existing Treatment In Clinical Trial
A new genital herpes drug, pritelivir, has demonstrated greater virus suppression than the present standard treatment in a clinical trial.
As what may be one of the most promising treatments for the herpes simplex virus 2 in two decades, pritelivir, has demonstrated in a recent trial that it provides greater viral suppression than the present standard treatment, valacyclovir.
In the study, conducted by a research team led by Anna Wald, MD, medical director of virology research at the University of Washington, Fred Hutchinson Cancer Research Center in Seattle, patients who took pritelivir not only experienced less HSV shedding than those who took valacyclovir , but they also had fewer lesions , less pain, and fewer treatment-emergent adverse events .
In the double-blind study, Dr. Wald and her team studied 91 adultswho reported having had between four and nine annual genital HSV-2 recurrencesrandomized into two groups. One group received pritelivir for 28 days, followed by a 28-day washout period, and then received valacyclovir for 28 days. The second group received valacyclovir first, and then the 28-day washout period, followed by pritelivir.

For anyone who suffers from herpes or has done antiviral research, the headline above is a serious attention grabber. Positive results announced by Synergy Pharmaceuticals about a potential breakthrough therapy that not only treated the symptoms but actually eradicated the virus from the volunteers. Amazing!

Not so fast.
First, a grand total of zero cases of herpes have been cured in the history of the world. Second, the premise of the biological strategy for eradication of the virus was shaky, at best. Third, the stuff in Synergy’s “cure” consists of “Rhodiola, Tribulus, Astragalus, Lysine and the Amino Acid Formula.” Please.

My interpretation of the bolded phrase: “Well, this crap doesn’t do anything, so let’s at least sell it as a dietary supplement. They don’t do anything either but plenty of people spend beaucoup bucks on them anyhow.

About half a billion people worldwide are living with genital herpes, and several billion have an oral herpes infection, new estimates show, highlighting the need to improve awareness and scale up services to prevent and treat herpes.
About 13% of the worlds population aged 15 to 49 years were living with herpes simplex virus type 2 infection in 2016, the latest year for which data is available. Currently, several new anti-herpetic drugs are being assessed in clinical trials, such as brincidofovir , amenamevir , pritelivir , and nelfinavir mesylate .
Brincidofovir is an acyclic nucleotide phosphonate, similar to cidofovir, yet it is conjugated to a lipid . When brincidofovir enters the cell, the lipid sidechain is cleaved and the compound is phosphorylated, acting as a substrate inhibitor for the viral DNA polymerase. Noteworthy, brincidofovir accumulates within the cell significantly more than cidofovir, and has up to 1,000-fold higher antiviral activity as compared to the latter . Brincidofovir was evaluated in phase III clinical trial that has concluded, yet to our knowledge the results have not been reported .
On the other hand, amenamevir and pritelivir target the viral DNA helicase/primase complex . There are three finished clinical studies for pritelivir, yet similar to brincidofovir, the results have not been reported to the best of our knowledge . There is also one clinical study that is currently ongoing and is in the recruitment phase for immunocompromised subjects with acyclovir-resistant mucocutaneous HSV-1 or HSV-2 infection . Amenamevir is an oxadiazolephenyl derivate that belongs to the helicase-primase group of inhibitors and has been evaluated in at least three clinical trials, although the results have not been published . Regretfully, a study indicates that amenamevir displayed adverse events in an early clinical phase against HSV-1 and HSV-2 .
A Cure For Herpes There Is Progress To Report
It takes a persistent scientist to stop a persistent virus.
A decade ago, Fred Hutchinson Cancer Research Center virologist Dr. Keith Jerome began exploring the idea that lifelong infections with herpes viruses might be cured by using the DNA-cutting tools of gene therapy.
Initial research showed these techniques could knock out small quantities of latent virus, and the work of improving the results fell to Jeromeâs senior staff scientist, Dr. Martine Aubert. Five years ago, the team reported they had damaged the genes of 2%-4% of herpes virus in infected mice. Aubertâs work was an important proof of principle, but far short of a cure.
Nevertheless, she persisted.
On Aug. 18, the team led by Jerome and Aubert published a paper in Nature Communications showing that, through a series of incremental improvements on their original method, they had destroyed up to 95% of herpes virus lurking in certain nerve clusters of mice.
âThis is the first time that anybody has been able to go in and actually eliminate most of herpes in a body,â said Jerome, who is also spearheading research at Fred Hutch and the University of Washington on COVID-19. âIt is a completely different approach to herpes therapy than anybodyâs ever had before.â
