Recurrences of herpes are often mild and infrequent, and most patients do not seek treatment. Drug therapy to prevent recurrences is available and effective, but because of cost and inconvenience issues, it is traditionally reserved for use in patients who have more than six outbreaks per year. Acyclovir resistance has not been a problem in immunocompetent patients but has been documented in 4 percent of HIV-infected persons.1
