How Herpes Is Treated
There is no cure for herpes.
- The health care provider may prescribe antiviral medicine to help speed up the healing process. It also shortens the time when the virus can spread from the herpes sores.
- Pain may be treated with medicine such as Tylenol or Motrin or by applying an ice pack wrapped in a towel to the affected area.
- If sores are in the mouth, avoid foods high in acid, like citrus fruits and juices.
- The infected area should be kept clean and dry.
- Wash hands often and avoid touching the sores to prevent spreading the virus.
- It is important to stay healthy by getting enough rest, having proper nutrition and exercise and managing stress well.
What Is Herpes Simplex
Herpes simplex is a virus that causes skin infections. The infection lasts your lifetime, and it causes painful or itchy sores and blisters that come and go. Herpes simplex virus typically doesnt cause severe problems. But it can be dangerous in infants and people with weakened immune systems. There are two types of herpes simplex:
- Herpes simplex 1 tends to affect your mouth or face. It causes cold sores. HSV-1 spreads through contact with saliva .
- Herpes simplex 2 is a sexually transmitted disease . It causes sores on skin that comes in contact with the genitals of an infected person.
Sometimes a herpes infection can affect other parts of your body, such as your eyes or other parts of your skin.
Clinical Herpes Simplex Infection
Primary infection with herpes simplex involves grouped vesicles on an erythematous base. Mucocutaneus manifestations include eczema herpeticum, herpetic whitlow, herpes gladiatorum, folliculitis, or a severe/chronic mucocutaneous infection. Extracutaneous manifestations include ocular HSV keratoconjunctivitis. Herpes Encephalitis, Proctitis, and Neonatal HSV. These manifestations are often the ones with prominent morbidity and mortality .
Ocular HSV infection has complications that include corneal ulceration and scarring, globe rupture, and subsequent blindness. Epithelial keratitis has strong links to viral replication, while stromal keratitis is primarily immunopathologic and is promoted mainly by T cells . Consequently, HSV is one of the main causes of infectious blindness in developed countries .
Herpes encephalitis has > 70% mortality without treatment, with residual neurological defects in most surviving patients . Similarly, disseminated neonatal HSV has 50% mortality without treatment, 15% with treatment and with neurologic deficits in many recovering patients .

Is There A Cure Or Treatment For Genital Herpes
There is no cure for genital herpes. However, daily use of antiviral medicines can prevent or shorten outbreaks. Antiviral medicines also can reduce the chance of spreading it to others.
Though several clinical trials have tested vaccines against genital herpes, there is no vaccine currently available to prevent infection. One vaccine trial showed efficacy among women whose partners had HSV-2, but only among women not infected with HSV-1. Efficacy was not observed among men whose partners had HSV-2. Another trial testing the same vaccine showed some protection from genital HSV-1 infection, but none from HSV-2.
Why Genital Herpes Comes Back
Genital herpes is caused by a virus called herpes simplex. Once you have the virus, it stays in your body.
It will not spread in your body to cause blisters elsewhere. It stays in a nearby nerve and causes blisters in the same area.
If you can, avoid things that trigger your symptoms.
Triggers can include:
- surgery on your genital area
- a weakened immune system for example, from having chemotherapy for cancer

Is There A Cure Or Treatment For Herpes
Theres no cure for the herpes virus yet. A herpes vaccine doesnt currently exist, as the herpes virus has more complicated DNA than most infections, creating challenges for researchers.
However, medication can help with symptoms, like sores, and outbreaks. Medication also lowers the risk of transmission to others. You can take most medications for herpes orally, though you may also apply some as a cream or receive them by injection.
Slicing And Dicing On A Molecular Level: Meganuclease And Crispr
Wilder: The article you published with your colleagues discusses the cutting enzymeits called meganuclease?
Jerome: Thats right.
Wilder: What exactly is meganuclease? Has it been used in any other kind of research, or is it something your team is pioneering?
Jerome: Meganucleases are essentially a fancy name for the molecular scissors that we use to locate the virus. They recognize a specific type of DNAin this case, 20 letters of DNA.The letters have to be in the perfect order the perfect combination. If the meganuclease sees the 20 letters in exactly the right order, it will make a cut in the DNA. Thats what we use to destroy the herpes.
Meganucleases have been around a very long time they actually evolved in yeast. Weve come to appreciate their biology over the last 20 years or so, and theyve been used for gene editing purposes for over a decade. Thats why we got into this.
Part of the reason you dont hear about meganucleases now is that, to some degree, theyve been sort of replaced by the CRISPR system. One of the things that we deal with, with any type of molecular scissorswhether its CRISPR or the meganucleases: You have to change it so that it will recognize the 20 letters in the right order.
The proteins that evolve naturally in yeast dont have anything to do with herpes. So we need to change the letters that theyre looking for. For a meganuclease, its quite a difficult process that can take months. For CRISPR, you can actually do that overnight.

New Approach Uses Mrna
Early efforts to produce a protein-based vaccine for herpes failed. But a new mRNA approach has outperformed the efficacy of the past vaccines in preclinical trials and is expected to be introduced in clinical trials in the second half of 2022, investigators say. This new approach for a prophylactic genital herpes vaccine showed great promise in early studies.1
Research into the mRNA herpes vaccine began long before the COVID-19 mRNA vaccines were developed, says Harvey M. Friedman, MD, professor of medicine/infectious diseases at the University of Pennsylvania Perelman School of Medicine.
Were pretty far along in the research, he says. We are using the mRNA technology in a vaccine effort for preventing genital herpes. Its not, at this point, intended for a treatment.
The vaccine, which was studied in mice and guinea pigs, showed exceptionally good prevention. Were hopeful it will make it into humans this year, Friedman says. Sometime in the summer of 2022 is our target.
This vaccine is good news for everyone at risk of STIs. More than one out of 10 Americans, age 49 years and younger, are infected with genital herpes, says Sita Awasthi, PhD, associate research professor in the infectious disease division at Perelman School of Medicine.
This is important for so many people who are devastated by the infection. Its a step in the right direction, she says. Hopefully, it will be proven in clinical trials to be effective.
Is There Any Resistance To Genital Herpes Medication
Resistance to drugs that treat genital herpes even after 20 years of use is very rare. Herpes medications may not work as well in patients who are very immunosuppressed and have been treated with these drugs for a long time. Each individuals response to treatment may vary. In some cases, patients may need more drugs to suppress their viral outbreaks than others.

How Often Do Herpes Outbreaks Occur
Outbreak frequency varies greatly from person to person. The average number of outbreaks for a person with the HSV-2 virus is four to five per year, while the average for HSV-1 is less than one outbreak per year. Usually, outbreaks are more common during the first year and become less frequent over time.
Genital Herpes During Pregnancy
Prevention of neonatal herpes depends both on preventing acquisition of genital herpes during late pregnancy and avoiding exposure of the neonate to herpetic lesions and viral shedding during delivery. Mothers of newborns who acquire neonatal herpes often lack histories of clinically evident genital herpes . The risk for transmission to the neonate from an infected mother is high among women who acquire genital herpes near the time of delivery and low among women with prenatal histories of recurrent herpes or who acquire genital herpes during the first half of pregnancy . Women who acquire HSV in the second half of pregnancy should be managed in consultation with maternal-fetal medicine and infectious disease specialists.
All pregnant women should be asked whether they have a history of genital herpes or genital symptoms concerning for HSV infection. At the onset of labor, all women should be questioned thoroughly about symptoms of genital herpes, including prodromal symptoms , and all women should be examined thoroughly for herpetic lesions. Women without symptoms or signs of genital herpes or its prodrome can deliver vaginally. Although cesarean delivery does not eliminate the risk for HSV transmission to the neonate , women with recurrent genital herpetic lesions at the onset of labor should have a cesarean delivery to reduce the risk for neonatal HSV infection.

- shooting pain at the site of the infection
You may notice some tingling, burning, or itching at the site of the infection before small, painful blisters appear. There could be one blister or a small cluster. These blisters will eventually burst and crust over before they begin to heal.
The blisters that develop during a primary infection may take to fully heal. These blisters can still transmit the virus until theyve healed completely.
Sores often itch, and genital sores may cause pain during urination.
Who Is At Risk For Developing Herpes Simplex Infections
Anyone can contract HSV, regardless of age. If youre exposed to HSV, youre likely to contract the virus.
Remember, HSV is very common. But because its often asymptomatic, plenty of people living with the virus never have an episode or realize theyve contracted HSV.
You may have a higher chance of contracting the virus if you:
- have a sexual partner who lives with HSV
- were assigned female at birth . suggests more AFAB folks than people assigned male at birth develop HSV, but this could also mean AFAB folks are more likely to experience symptoms.
- are immunocompromised
Some older research suggests HSV-1 antibodies may offer AFAB folks some protection against contracting HSV-2. Still, many people living with one type of the virus do contract the other type later on. You wont contract the same type of the virus again, though, since it remains dormant in your body once you acquire it.
You may have a slightly higher chance of contracting genital HSV if you have sex without using condoms or other barrier methods. Keep in mind, though, that condoms and other barrier methods wont always cover the infection site, since sores can appear on the buttocks or inner thighs.

Further Considerations For The Future Of Herpes Simplex Virus Vaccine Development
Our understanding of the complexities of herpes simplex virus pathogenesis and immune evasion is consistently evolving, along with our understanding of viral latency. Designing an effective therapeutic or prophylactic vaccine requires further understanding these processes.
Trials that had been successful in the pre-clinical realm with murine model and guinea pig models have been effective, but have not translated well in the clinical realm. Other trials like the GSK gD subunit vaccine and the GEN-003 vaccine have been suspended due to dwindling financial support . To us, the HSV-529 and G103 appear to be promising from early studies in the pre-clinical stage. Additionally, they are likely to have less issues with safety as they are not live-attenuated vaccines.
The rapid development of an mRNA vaccine for COVID-19 has re-vitalized interest in mRNA vaccines as viable options to produce viral immunity. The classical problems involving the creation of mRNA vaccines include mRNA stability and delivery systems. It remains to be seen whether mRNA vaccines might have a utility for HSV however, initial studies with the trivalent vaccine have been promising, showing increased efficacy compared to a subunit formulation .
The advantages of an mRNA vaccine were previously discussed: It does not integrate within the host genome, translates in both proliferating and non-proliferating cells, with immediate protein production for a controllable amount of time .

New Gene Therapy Approach Eliminates At Least 90% Latent Herpes Simplex Virus 1
- Fred Hutchinson Cancer Research Center
- Infectious disease researchers have used a gene editing approach to remove latent herpes simplex virus 1, or HSV-1, also known as oral herpes. In animal models, the findings show at least a 90 percent decrease in the latent virus, enough researchers expect that it will keep the infection from coming back.
Infectious disease researchers at Fred Hutchinson Cancer Research Center have used a gene editing approach to remove latent herpes simplex virus 1, or HSV-1, also known as oral herpes. In animal models, the findings show at least a 90 percent decrease in the latent virus, enough researchers expect that it will keep the infection from coming back.
The study, published August 18 in Nature Communications, used two sets of genetic scissors to damage the virus’s DNA, fine-tuned the delivery vehicle to the infected cells, and targeted the nerve pathways that connect the neck with the face and reach the tissue where the virus lies dormant in individuals with the infection.
“This is the first time that scientists have been able to go in and actually eliminate most of the herpes in a body,” said senior author Dr. Keith Jerome, professor in the Vaccine and Infectious Disease Division at Fred Hutch. “We are targeting the root cause of the infection: the infected cells where the virus lies dormant and are the seeds that give rise to repeat infections.”
The team did other comparisons to fine-tune the gene editing approach:

Home Remedies And Lifestyle
At-home treatments for herpes are used to relieve pain. They do nothing to alter the course of the infection but can make you more comfortable as the herpes sores begin to heal.
There are several things you can do if have an outbreak of oral or genital herpes:
- Use a cold compress. Place a cloth-covered ice pack on the sores for 15 minutes several times daily to reduce pain. Keep the ice pack moving to prevent frostbite.
- Do not touch or scratch. Avoid touching and rubbing the sores. Doing so can spread the infection to other areas of the skin.
- Keep the sores clean. Herpes sores can become infected with bacteria if touched with soiled hands. Wash your hands thoroughly with soap and water after going to the bathroom or before applying medications.
- Reduce stress. Stress can increase your sensitivity to pain during a herpes outbreak. You can reduce stress by performing gentle exercises or practicing mind-body therapies like meditation, yoga, and guided imagery.
To reduce the risk of spreading herpes, avoid kissing and sharing objects that come into contact with saliva, like drinking straws and eating utensils.
It’s important to note that oral herpes can be spread to the genitals, and that genital herpes can be spread to the mouth. To reduce the risk of infection, use condoms and dental dams during oral sex. Better yet, avoid sex altogether if you are having an active herpes outbreak .
How Is Herpes Simplex Treated
Although there is no cure for herpes, treatments can relieve the symptoms. Medication can decrease the pain related to an outbreak and can shorten healing time. They can also decrease the total number of outbreaks. Drugs including Famvir, Zovirax, and Valtrex are among the drugs used to treat the symptoms of herpes. Warm baths may relieve the pain associated with genital sores.
