Genital herpes is one of the most common co-infections for people with HIV. It can be a more serious condition if youre HIV positive – outbreaks may last longer and blisters can be more severe. If youre having recurrent outbreaks of genital herpes, you should have an HIV test, as this may be a sign of a weakened immune system caused by HIV.

Having an STI such as genital herpes can increase your risk of getting and passing on HIV. The blisters and sores provide an easy way for HIV to get into your body and cause an infection.

People with HIV who arent on treatment or who have a lower CD4 count are more likely to get other infections, like herpes.

If youre taking antiretroviral treatment for HIV, its important to discuss this with your doctor. They can advise how treatment for herpes may interact with your HIV drugs.

How Long Do Herpes Outbreaks Last

When you hear the word herpes, the image that comes to mind is probably one of a sore at the edge of your lip that crops up every time you have a cold or a fever. But herpes is a little more complicated that that. Believe it or not, most people who have herpes have absolutely no idea. Thats because, for the most part, the virus can be entirely asymptomatic. Usually the first infection when it comes to oral herpes happens sometime during childhood and not from sexual contact, and the symptoms are a lot more like a cold. In severe cases, the initial outbreak can involve a fever, swollen lymph nodes, swollen gums, and sores in your mouth, throat, or on your tongue. An initial herpes outbreak lasts around 14 days but can take up to six weeks for sores to fully heal .

While oral herpes is most commonly caused by herpes simplex virus 1 , genital herpes is more frequently cause by herpes simplex virus 2 .

Much like oral herpes, an initial genital herpes infection can last a lot longer than subsequent outbreaks and may be accompanied by fever and other whole-body symptoms and some symptoms may persist for weeks. Recurrent genital herpes outbreaks cause symptoms like itching, redness, and irritation in the genital area, but like oral herpes, genital herpes can also be asymptomatic . A genital herpes outbreak may begin with pain in the genitals and shooting pains or tingling in your legs, hips, or butt. These symptoms usually occur before you can see any blisters or lesions .

Can You Get Herpes On Your Tongue

While you may associate herpes type 1 with cold sores on your lips, it is possible to suffer from the symptoms of herpes on your tongue. This is referred to as oral herpes and usually affects more of your mouth than just your tongue. More than half of all adults in the United States are thought to have oral herpes.

Treatment The First Time You Have Genital Herpes

You may be prescribed:

  • antiviral medicine to stop the symptoms getting worse you need to start taking this within 5 days of the symptoms appearing
  • cream for the pain

If you have had symptoms for more than 5 days before you go to a sexual health clinic, you can still get tested to find out the cause.

What Happens At A Sexual Health Clinic

Painless rash/blisters

The doctor or nurse at the sexual health clinic will:

  • ask about your symptoms and your sexual partners
  • use a small cotton bud to take some fluid from 1 of your blisters or sores for testing

The test cannot:

  • be done if you do not have visible blisters or sores
  • tell you how long you have had herpes or who you got it from

Symptoms might not appear for weeks or even years after you’re infected with the herpes virus.

How Long Do Sores From Herpes Simplex Last

If youre infected with HSV-1, commonly known as oral herpes, you may notice tingling or burning around your mouth in the days before a cold sore appears. These blisters break open and ooze fluid before forming a crust. Usually, sores last for seven to 10 days.

If youre infected with HSV-2, commonly known as genital herpes, your first outbreak may last between two to four weeks. Recurrent outbreaks usually last between three to seven days.

Can Genital Herpes Be Treated

Your physician can prescribe different medications to help reduce your symptoms and speed up the healing of an outbreak. These medicines work best when you start them soon after an outbreak occurs.

To reduce pain during an outbreak:

  • Sit in warm water in a portable bath or bathtub for about 20 minutes. Avoid bubble baths.
  • Keep your genital area clean and dry, and avoid tight clothes.
  • Take over-the-counter medications, such as acetaminophen or ibuprofen. Avoid aspirin.

Let your physician know if you are worried about your genital herpes. He or she can recommend a support group to help you cope with the virus.

Monkeypox Can Cause A Rash On The Genitals

Monkeypox is a rare disease caused by the monkeypox virus, according to the CDC. The monkeypox virus belongs to the same family of viruses as variola virus, the virus that causes smallpox. While monkeypox symptoms are similar to smallpox symptoms, they are less severe, and monkeypox is rarely fatal.

Additional symptoms of monkeypox can include:

  • Respiratory symptoms, such as coughing, nasal congestion, or sore throat

Monkeypox symptoms usually appear within three weeks of exposure to the virus, per the CDC. The illness typically lasts two to four weeks. Some people get a rash first, followed by other symptoms, while others get flu-like symptoms first and then develop a rash one to four days later. How Long After Herpes Outbreak Is It Still Contagious

For oral herpes, cold sores are usually contagious for up to 15 days and are generally no longer contagious once all symptoms have cleared, which includes the blister itself and any scabbing.

Someone carrying genital herpes should wait until 7 days after the sore heals to begin engaging in sexual activity, as the herpes virus can spread from sores not covered by a condom.

Can I Breastfeed If I Have Genital Herpes

Yes, you can breastfeed if you have genital herpes, but not if you have a herpes sore on one of your breasts. If you have genital herpes, it is possible to spread the infection to any part of your breast, including your nipple and areola.

The herpes virus can be spread to babies and children if they are kissed by someone that is infected and has active cold sores. Although rare, it is also possible to spread herpes to a child or infant by touch if a person touches their cold sore and then touches the baby or child.

How Is Herpes Treated

There is no cure for the herpes simplex virus. The blisters usually heal and go by themselves, so you may not always need treatment. If treatment is needed, there are antiviral medicines which can help. These can shorten outbreaks, relieve discomfort and stop symptoms from getting worse.

The antiviral treatment is most effective when you take it within the first five days of symptoms appearing. Avoid touching the blisters as this can spread the infection. If treatment requires you to apply cream to a sore, gently pat the cream on, being careful not to rub around the surrounding area.

You can ease your symptoms by:

  • keeping the affected area clean using plain or salt water to prevent blisters or ulcers from becoming infected

  • applying petroleum jelly, such as Vaseline, to any blisters or ulcers to reduce pain when passing urine

  • asking a healthcare provider to recommend painkilling creams

  • avoiding tight clothing because it may irritate the blisters and ulcers.

Dont have sex until you or your partner have finished your treatment, and the blisters or ulcers have gone. Recurrent outbreaks are usually milder than the first episode of genital herpes and over time they tend to occur less often.

Do Condoms Help Prevent The Spread Of Genital Herpes

Yes. To prevent transmission of herpes, we recommend that condoms be used 100% of the time. Many patients will shed the virus and be contagious when they dont have symptoms. Studies have shown that asymptomatic shedding occurs between 1% and 3% of the time in patients with HSV II genital infections. Many new herpes infections occur from partners who are shedding the virus asymptomatically, so condoms are highly recommended.

Condoms may not be an attractive option for monogamous couples or for couples who desire to become pregnant. Couples may opt to have serological tests to determine if either partner has an asymptomatic infection. In close monogamous relationships, the risks of transmission can be weighed against other relationship issues, such as intimacy and pregnancy.

How Can Herpes Be Prevented

Correct and consistent use of latex condoms can reduce, but not eliminate, the risk of transmitting or acquiring genital herpes because herpes virus shedding can occur in areas that are not covered by a condom.25,26

The surest way to avoid transmission of STDs, including genital herpes, is to abstain from sexual contact, or to be in a long-term mutually monogamous relationship with a partner who has been tested for STDs and is known to be uninfected.

Persons with herpes should abstain from sexual activity with partners when herpes lesions or other symptoms of herpes are present. It is important to know that even if a person does not have any symptoms, he or she can still infect sex partners. Sex partners of infected persons should be advised that they may become infected and they should use condoms to reduce the risk. Sex partners can seek testing to determine if they are infected with HSV.

Daily treatment with valacyclovir decreases the rate of HSV-2 transmission in discordant, heterosexual couples in which the source partner has a history of genital HSV-2 infection. 27 Such couples should be encouraged to consider suppressive antiviral therapy as part of a strategy to prevent transmission, in addition to consistent condom use and avoidance of sexual activity during recurrences.

How Is Genital Herpes Spread

  • Saliva from a partner with an oral herpes infection
  • Genital fluids from a partner with a genital herpes infection
  • Skin in the oral area of a partner with oral herpes or
  • Skin in the genital area of a partner with genital herpes.

You also can get genital herpes from a sex partner who does not have a visible sore or is unaware of their infection. It is also possible to get genital herpes if you receive oral sex from a partner with oral herpes.

You will not get herpes from toilet seats, bedding, or swimming pools. You also will not get it from touching objects, such as silverware, soap, or towels.

When To Call The Doctor

Even though HSV sores can hurt and be unsightly, most cases of herpes do not cause serious illness. The sores go away in a few days. Contact the health provider if:

  • Sores are present and you think it might be a first herpes infection. The diagnosis is usually made by examining the sores. If the sores have not healed, sometimes a laboratory test is done.
  • Sores do not heal by themselves or show signs of infection such as pus, spreading redness or fever.
  • Sores are near the eyes.
  • You have another health condition with a weakened immune system.

Contact Dermatitis Can Be Mistaken For Herpes

Contact dermatitis is a skin condition that can cause an itchy rash, cracked, dry, or scaly skin, bumps and blisters, leathery patches, or swelling, burning, or tenderness, according to the Mayo Clinic. Its not an STD, but when it appears in the mouth or genital area, it may be mistaken for herpes.

However, unlike herpes, contact dermatitis can happen anywhere on the skin and is caused by direct contact with an irritant or an allergen. It is not caused by a virus.

Contact dermatitis usually occurs within days of exposure to an irritant or allergen. Common causes include poison ivy and reactions to medication, cosmetics, detergents, and jewelry. The skin rash from contact dermatitis usually goes away in two to four weeks.

The best way to prevent contact dermatitis is to identify whats causing it and avoid touching that item or substance again. Allergy testing can help identify the cause.

While contact dermatitis is generally not a serious condition, it can be itchy and uncomfortable. When the rash is present, try using cool, wet compresses or anti-itch creams to soothe it. Antihistamine pills can help reduce the itchiness. Avoid scratching the area: Doing so can make symptoms worse and could open the door to a bacterial or fungal infection. If your itching is severe, you should see your primary care provider or a dermatologist for topical steroid medication.

How Do You Get Herpes
