Prognosis Of Genital Herpes
Genital herpes can be treated and managed. Baill stresses that for most people who have healthy immune systems, the high-risk times for giving a sex partner genital herpes are during the first outbreak and during later outbreaks of sores.
Without such an outbreak, the viral load is usually not sufficient to infect a normal, healthy partner, she says. Antiviral medication can be effective at controlling the frequency and the severity of genital herpes outbreaks, Baill says.
For some people, though, even the times when no symptoms are present can be risky ones for transmission.
If you have genital herpes and your partner is taking biologic drugs for rheumatoid arthritis or MS or psoriasis, you may need to be extra careful, says Baill. It may take a lower viral load to infect people taking those medications.
In these situations, she says, you may want to take antiviral drugs in an ongoing way to keep your viral load low and protect your partner.

- Being a woman. Women are more likely than men to become infected when exposed to genital herpes. And their symptoms tend to be more severe and longer-lasting. Women also are at a greater risk of having complications from a genital herpes infection.
Having herpes, especially if you have open sores, also increases your risk for becoming infected with HIV if you are exposed to HIV.
Condoms Reduce Womens Risk Of Herpes Infection But Do Not Protect Men
Using condoms during sexual intercourse significantly decreases the likelihood that men infected with herpes simplex virus type 2 will transmit the infection to their female partners, according to the first study to examine the effectiveness of condoms in preventing this infection.1Women are almost six times as likely as men to acquire HSV-2. Increased frequency of sexual intercourse, younger age and having a partner who is infected with both herpes simplex virus type 1 and HSV-2 increase the likelihood of acquiring HSV-2. Although using condoms more than 25% of the time offers women a high degree of protection against acquiring HSV-2, men do not receive the same benefits.
Overall, 528 couples were included in the study. Of the susceptible partners, 267 were women and 261 were men, with a median age of 36 years. Ninety-two percent were white, and 98% were in a heterosexual relationship. Participants median frequency of intercourse was twice weekly half said that they had used condoms no more than 10% of the time since becoming sexually active. Of the source partners, 62% were seropositive for only HSV-2, while 38% were seropositive for both HSV-1 and HSV-2.
During the studys observation period, 31 of the 528 susceptible partners acquired HSV-2: 26 of the women and five of the men. Women acquired the virus at a rate of 8.9 per 10,000 sex actsalmost six times the rate of men .

Frequent, severe recurring outbreaks might be a sign of a compromised immune system. If outbreaks are occurring often, a doctor might investigate whether theres an underlying issue affecting the immune system.
Having sex is OK if theres a genital herpes diagnosis, but its best to avoid sex if theres a herpes outbreak. To reduce the chances of transmitting HSV to a partner, use barrier methods such as condoms and dental dams during all sexual activity.
Genital herpes does not always require treatment. However, genital warts can be painful. Antiviral treatments can reduce the symptoms and severity of outbreaks.

As you age, the virus loses some of its ability to kind of be a badass, says Kelly-Jones. Youve been fighting it for a long time, and you have less of a chance of asymptomatic shedding. This means you’d be less likely to actively have an outbreak without knowing it, making you less likely to pass along the virus.
Its a phenomenon known as prodrome, says Kelly-Jones, and it happens when a patient experiences symptoms before a lesion appears. Some patients recognize them, and what happens is the herpes virus lives in the nerve root in your spinal cord. When it reactivates, it comes down that same nerve root and causes the lesion in one specific area. So patients will report itching, tingling, and burning .
If you notice those symptoms, Kelly-Jones says you can begin treatment right away. It probably wont prevent the lesion from happening, but it can decrease the longevity and severity of the outbreak, she says. And, of course, you should abstain from sex.
When youre not having an outbreak, its okay to have sexso long as you use a condom. Consistent use reduces your risk of passing along the virus by about 30 percent, according to a study in the Archives of Internal Medicine.
That said, remember that condoms break and not everyone knows how to use one perfectly. And because the virus is spread by skin-to-skin contact, that means that any slip-ups could leave an uninfected partner at risk. That’s why it’s important to talk to your doctors about medication, too.
