How Are Initial Outbreaks Treated

Mothers of newborns who acquire neonatal herpes often lack histories of clinically evident genital herpes . The risk for transmission to the neonate from an infected mother is high among women who acquire genital herpes near the time of delivery and low among women with prenatal histories of recurrent herpes or who acquire genital herpes during the first half of pregnancy . Women who acquire HSV in the second half of pregnancy should be managed in consultation with maternal-fetal medicine and infectious disease specialists.
All pregnant women should be asked whether they have a history of genital herpes or genital symptoms concerning for HSV infection. At the onset of labor, all women should be questioned thoroughly about symptoms of genital herpes, including prodromal symptoms , and all women should be examined thoroughly for herpetic lesions. Women without symptoms or signs of genital herpes or its prodrome can deliver vaginally. Although cesarean delivery does not eliminate the risk for HSV transmission to the neonate , women with recurrent genital herpetic lesions at the onset of labor should have a cesarean delivery to reduce the risk for neonatal HSV infection.

Prevention Of Genital Herpes

Does Herpes Need To Be Treated
Treating herpes simplex symptoms, even for the first infection, may not be necessary. Herpes simplex infections clear up even if no treatment is given. But treating genital herpes can be helpful particularly if there are severe symptoms. Some people are affected more than others. See below for the rare occasions when it can be serious.

Neurological Signs In Long Term Survivors
Orientation in time and place and short term memory were evaluated in 27 of the 29 patients the two children could not be assessed. Two patients were disoriented in time and two patients were disoriented in time and place. Orientation was normal in the others. Short term memory was normal in eight patients , mildly impaired in 13 patients and severely affected in six patients . The score for the mini mental state examination was less than 25 in five of 27 patients tested.
Unilateral or bilateral anosmia was found in 65% of the patients. Eleven had a mild non-fluent dysphasia, but only one had a severe global aphasia. Four patients had a pseudobulbar palsy secondary to an anterior opercular syndrome. Minor upper motor neuron signs were noted in five patients. Two had a homonymous hemianopia, but in one this preceded herpes simplex encephalitis. One patient developed acute HSV retinitis two years after treatment for herpes simplex encephalitis.
What Treatments Are Available For Genital Herpes
The treatment for genital herpes varies between an initial outbreak and a recurrent attack. If you are suffering from a primary infection of genital herpes then it is likely you will be prescribed an antiviral medication, for example, Aciclovir 400mg tablets.
Aciclovir works by preventing the herpes simplex virus from multiplying. Aciclovir, and other antivirals, are unable to completely eradicate the infection from the body. The dose of Aciclovir when treating genital herpes is typically one 400mg tablet, five times a day after your inital outbreak. You will need to take Aciclovir for at least five days or longer depending on whether new blisters are still forming when you start treatment. For subsequent outbreaks of genital herpes, you only need to take one Aciclovir 400mg tablet, three times a day for five days. Other antiviral medications used to treat genital herpes include Valaciclovir, which works in a similar way to Aciclovir.
Lidocaine 5% ointment is an anaesthetic ointment used to know the area of genital herpes to relieve the pain and irritation of blistering during the outbreak. It does not treat the infection itself, however, it helps to relieve the symptoms whilst the outbreak clears on its own.
Recurrent outbreaks may require a course of Aciclovir however, if your symptoms are mild, you may wish to use some self-help techniques to ease the symptoms. These can include:
