Does Herpes Antibodies Mean I Have Herpes

Increased Risk Of Hiv

How Herpes Can Actually Be Good For You

As reported by the National Institute of Health , scientists have linked HSV-2 outbreaks with an increased risk of contracting HIV. The magnified risk â said to be 2-to-3-fold over non-HSV-2 infected individuals â is due to the ideal spreading mechanism open lesions present for the HIV virus. HIV targets the bodyâs CD4+T cells, which commonly appear at the site of healed genital herpes lesions at 2 to 37 times greater frequency than unaffected genital skin.

What Is Genital Herpes

Genital herpes is a sexually transmitted infection that is caused by the herpes simplex virus the same type of virus that causes cold sores.

As many as 1 in 8 sexually active Australian adults have genital herpes.

If you think you have genital herpes, it is important to see a doctor as soon as possible to confirm the diagnosis and start treatment.

Can You Be Falsely Diagnosed With Herpes

False positive results can occur with many diagnostic tests, including STD tests. The chances of false positive results increase as the likelihood of the infection decreases in the person being tested. False positive HSV-2 results can happen, especially in people who are at low risk for a herpes infection.

What If A Person Has Never Had Chickenpox Or The Vaccine For It

Shingles doesnt spread from one person to another. And those whove never had chickenpox or received the chickenpox vaccine cant get shingles.

The varicella-zoster virus that causes shingles can be transmitted, however. Those who dont have the virus can contract it from exposure to active shingles blisters, and then develop chickenpox as a result.

Following are a few precautions to take to reduce risk of contracting the varicella-zoster virus:

  • Try to avoid exposure to people with chickenpox or shingles.
  • Be especially careful to avoid direct contact with the rash.
  • Ask a healthcare professional about getting the vaccine.

There are two shingles vaccines available. The newest vaccine contains inactivated virus, which wont cause a shingles infection and so can be given to people whose immune system is severely compromised. The older vaccine contains the live virus and may not be safe in this case.

Consult a healthcare professional to find out if they recommend getting vaccinated against shingles.

Those with HIV might get a more severe case of shingles and are also at increased risk of complications.

Treating The Symptoms Of Herpes

Theres currently no cure for herpes, but there are treatments that can reduce the symptoms and make it less likely to transmit the virus to a sex partner.

Antiviral medications are prescribed to help clear up symptoms or reduce the frequency of outbreaks. Some antivirals that may be prescribed include:

Why Do I Need This Test

If you suspect that you have herpes but do not have an active infection, the antibody blood test can help make the diagnosis.

You may also have this test if you have HIV, or are pregnant or hope to become pregnant. The herpes simplex virus antibodies test screens for current or previous HSV infections.

In some cases, the herpes simplex virus antibodies test can be used to diagnose an active HSV infection. But more often, a herpes culture is used.

The antibodies test is valuable because many initial herpes infections show no symptoms. If symptoms do occur, they can include tenderness, as well as pain or burning at the site of the infection. This usually occurs before the outbreak of sores. You may also have headache, fever, achiness, or pain.

Does A Cold Sore On My Mouth Mean I Have Genital Herpes

No, a cold sore on your mouth usually means you have herpes simplex virus type 1 . You can get HSV-1 by kissing someone or sharing utensils, towels, razors, or lipstick with someone who has HSV-1.

HSV-1 cannot turn into HSV-2 , but you can get a cold sore on your mouth from HSV-2 if you give oral sex to someone with HSV-2. Cold sores caused by HSV-1 or HSV-2 are contagious. You can spread it to other people or other parts of your body if you touch an open sore and then touch another part of your body. That means if you have a cold sore and give oral sex to someone, that person will get the herpes virus on his or her genitals.

Avoid touching your cold sore as much as possible. If you touch your cold sore, wash your hands right away to avoid spreading the infection to other parts of your body or other people.

When To See A Medical Provider

If you have never been formally diagnosed with HSV-2 and you notice signs or symptoms of possible infection, visit your doctor to confirm the diagnosis.

They will do a physical examination and may wish to do additional testing to determine if you have any other STDs.

Visit a doctor or STD clinic frequently to protect yourself and your partners from STDs.

Get regular sexual health checkups to reduce the chances of exposing yourself or others to potentially harmful STDs.

If you test positive for HSV-2, you are at a greater risk of contracting HIV.

Herpes also increases the number of immune cells in the lining of the genitals which HIV targets to enter the bloodstream.

If you are experiencing a flare-up, a doctor can prescribe you antiviral medications to help alleviate your symptoms.

For the safety of your sexual partners, you should abstain from sexual activity until the sores have cleared up.

Doctors Don’t Advise Getting A Blood Test For Herpes Without Any Symptoms

Understanding Herpes Testing

Hilda Hutcherson, assistant professor of obstetrics and gynecology at Columbia University Medical Center in New York, is one of those doctors. “I usually dont do tests for herpes someone has a lesion or a sore, something I can see, I will do a swab and take it to a culture or check the DNA on the materials I removed,” she says. “If theres nothing there and I do a blood test and the blood test comes back positive, lets say its positive for type 1, what does that really mean? Most people are positive for type 1.” She adds that someone could be HSV-1-positive and either have or not have genital herpes. The result “doesnt mean much, and it makes people go crazy.”

Hutcherson’s motto is pretty simple: Why stress yourself over something if you haven’t seen anything especially when it’s something as common as herpes? “Let’s say they have a blood test come in positive but theyve never had a lesion. Now what? You may never get a lesion,” she says. It’s for this reason that she recommends people do not get tested for herpes unless they have a visible lesion that can be cultured and sent to a lab. Let’s say you get tested anyway and it comes back positive, despite the fact that you have no symptoms. What do you do?

There Are Two Common Blood Tests Used To Identify Herpes And They’re Often Misleading

“I had a patient once who underwent this test, which was positive, and was told she has herpes, which to her was devastating and life-altering,” Friedman says. “She burst into tears when I explained the significance or lack thereof with this test, which is very misleading.” There are two common blood tests used to identify herpes: IgG and IgM. The IgM test is still used by some doctors even though it has been denounced by the Centers for Disease Control and Prevention . According to the American Sexual Health Association , IgM tests can lead to deceptive results, as well as false assumptions about how and when a person acquired HSV.

And that’s not all: IgM tests fail to accurately distinguish between HSV-1 and HSV-2 antibodies, leading them to provide false-positive results for HSV-2. In other words, if you’ve been exposed to oral herpes , results of the IgM test could lead you to believe you’re positive for genital herpes, when in fact you simply have antibodies against HSV-1.

IgG tests, on the other hand, are more accurate, but aren’t without their own issues. For instance, ASHA says that while an IgG test can distinguish between HSV-1 and HSV-2, the problem is that the time it takes for IgG antibodies to reach a detectable level varies from person to person. This means that even with accurate results, someone could get a false-negative if they have the test administered too soon after contracting the virus .

What Happens During An Hsv Test

HSV testing is usually done as a swab test, blood test, or lumbar puncture. The type of test you get will depend on your symptoms and health history.

  • For a swab test, a health care provider will use a swab to collect fluid and cells from a herpes sore.
  • For a blood test, a health care professional will take a blood sample from a vein in your arm, using a small needle. After the needle is inserted, a small amount of blood will be collected into a test tube or vial. You may feel a little sting when the needle goes in or out. This usually takes less than five minutes.
  • A lumbar puncture, also called a spinal tap, is only done if your provider thinks you may have an infection of the brain or spinal cord. During a spinal tap:
  • You will lie on your side or sit on an exam table.
  • A health care provider will clean your back and inject an anesthetic into your skin, so you won’t feel pain during the procedure. Your provider may put a numbing cream on your back before this injection.
  • Once the area on your back is completely numb, your provider will insert a thin, hollow needle between two vertebrae in your lower spine. Vertebrae are the small backbones that make up your spine.
  • Your provider will withdraw a small amount of cerebrospinal fluid for testing. This will take about five minutes.
  • Your provider may ask you to lie on your back for an hour or two after the procedure. This may prevent you from getting a headache afterward.

Hsv Infection And Symptoms

Most individuals infected with HSV have either no symptoms or mild symptoms that go unnoticed. When symptoms do appear, they initially present with tingling and/or redness, followed by blister-like lesions that rapidly merge into open, weeping sores. The sores are often quite painful and can be accompanied by a fever and swollen lymph glands.

Both oral and genital herpes cycle between periods of active disease, which can last from two days to three weeks, followed by a period of remission. After the initial infection, the viruses attach themselves to sensory nerve cells, where they remain for a lifetime. HSV can reactivate at any time , although the frequency and severity of outbreaks tend to subside over time.

Diagnosis is generally made by clinical examination of the patient, although genital herpes is often difficult to diagnose as symptoms can be mild and easily confused with other conditions . Laboratory tests are sometimes used to make a definitive diagnosis, including newer generation HSV antibody tests which can identify HSV-1 or HSV-2 with greater than 98% specificity, but cannot detect new infections, as antibodies to the virus have not yet been made.

Some research has shown that two-step testing might improve the specificity that is avoid false positive results for the blood test which would be a great thing because these diagnoses create great anxiety and concern for people, he said. In a commentary accompanying the USPSTF guidelines, Hook expressed disappointment that herpes testing had barely improved over the past decade.

There is no perfect test but there are ways to reduce the inaccuracies and reduce the number of equivocal results and those are actively used in other diseases, he said in an interview with STAT. But they havent been used very aggressively for the purpose of herpes.

In the absence of sure-fire test advances, education and a lessening stigma surrounding herpes might help, not only by reducing test-related confusion, but potentially by bringing discussion of the virus out into the mainstream, said Hook.

Theres no major herpes advocacy group, he said. People call attention to diseases that they suffer from, but people with herpes dont feel they can call attention to it. And that creates a lot of suffering.

Herpes Might Not Have Any Symptoms

You or your partner may not have any herpes symptoms that you can see or feel, or the signs of herpes may be so mild you dont even notice them. Sometimes people confuse herpes symptoms with other things, like pimples, ingrown hairs, and the flu.

Herpes symptoms come and go, but that doesnt mean the infection goes away or that you cant spread it to other people. Once you have herpes, it stays in your body for life.

Is Genital Herpes Infection Related To Hiv

A Vaccine Pathway for Herpes Virus with Gregory Smith, PhD

Herpes and HIV are caused by different viruses. But patients infected with these viruses are more likely to transmit either disease to their sexual partners. Patients with herpes are more vulnerable to HIV infection. People newly diagnosed with herpes should be tested for HIV infection and other sexually transmitted infections.

Patients who are infected with both herpes and HIV also may have a higher concentration of HIV viruses in their body because of the interaction between the herpes virus and the HIV virus. When HIV damages a persons immune system, the person may be more likely to shed herpes simplex virus asymptomatically.

Can Genital Herpes Be Treated

Your physician can prescribe different medications to help reduce your symptoms and speed up the healing of an outbreak. These medicines work best when you start them soon after an outbreak occurs.

To reduce pain during an outbreak:

  • Sit in warm water in a portable bath or bathtub for about 20 minutes. Avoid bubble baths.
  • Keep your genital area clean and dry, and avoid tight clothes.
  • Take over-the-counter medications, such as acetaminophen or ibuprofen. Avoid aspirin.

Let your physician know if you are worried about your genital herpes. He or she can recommend a support group to help you cope with the virus.

When tests fall between 1.1 and 3.5, more testing is necessary, said Johnston. This recommendation is also noted in the 2015 CDC Sexually Transmitted Diseases Treatment Guidelines. But some patients will never be referred for a second test.

I think most clinicians are unaware and perhaps labs dont have this available and/or it is not straightforward to order, Johnston said of second-step tests.

But while her facilitys Western blot is considered highly accurate, it is expensive and cumbersome to perform. Each test costs over $200 and the University of Washington is the only lab that provides it.

Other confirmatory tests also exist, for instance Biokits HSV-2 Rapid Test and Quests own HSV-2 IgG Inhibition assay. The latter, which adds only $4 to the price of the HerpeSelect test, performed well in a study conducted over a decade ago. Rick Pesano, the medical director for infectious disease at Quest, believes that with more awareness, the test could stand in for the Western blot. But the test was not mentioned in the USPSTF guidelines because it still has not been evaluated in asymptomatic individuals, according to Cindy Feltner, associate director of the RTI-UNC Evidence-based Practice Center, who helped prepare the science review for USPSTF.

We need better diagnostic testing. That is where we are stuck at this point, said Johnston. We dont have a good test thats inexpensive, high throughput, and reliable.
